Uterine Fibromyoma and Intravascular Thrombosis—Eight cases

Objectives: We present eight cases of vascular thrombosis of the pelvic vessels and deep venous thrombosis (DVT) of the lower limb, secondary to compression of the pelvic vessels by the uterine fibroid, managed over a span of twelve years from 2001 to 2013. We discuss the mean size of the leiomyoma, the prediliction for DVT of the left lower limb, the role of oral contraceptives when used in women with large fibroids, to increase the risk of DVT and present a brief literature review.?Results:?Age,?the mean age of our patients was—41.12 yrs, range—18 yrs to 50 yrs.?Parity and Menopausal Status: Seven were parous women. One was unmarried, nulligravid.?Seven were premenopausal and one was postmenopausal.?Vascular Thrombosis:?In one it was arterial thrombosis and in seven it was venous thrombosis.?DVT was on the left side in -6/8?-?75%. clinical complaints:?The presenting complaints were heavy menstrual bleeding in three,?severe dysmenorrhoea and heavy bleeding in one,?mass per abdomen in one, heavy menses and abdominal mass in one, no complaints apart from DVT in one.?Uterine Enlargement:?The size of the uterus was between 12 to 26 weeks. The mean size was 20 weeks.?It was a single fibromyoma in six and multiple, two in two.?Use of Oral contraceptives:?Oc pills were used to control heavy menstrual bleeding in three cases—3/8, 37.5%.?Discussion:?The DVT was on the left side in six of our cases,?75% were on the left side in our series. Menstrual problems like heavy bleeding can be secondary to fibromyoma. Three of the eight—37.5%,?women used Oc pills to control menorrhagia. One developed DVT after one cycle of use of Oc pills;?the second developed arterial thrombosis of the pelvic vessels after four cycles of Oc pill use;?the third used Oc pills for a longer period, on and off. The woman who developed arterial thrombosis was using Oc pills with 50 mcgs of oestrogen and 0.5 mg norgestrel. conclusions: Uterine leiomyoma can cause vascular thrombosis secondary to compressio

Synthesis of nano crystalline spatulae of lead zirconate titanate (PbZr0.52Ti0.48O3)

A simple and effective method for the synthesis of nano-crystalline PZT spatulae has been re-ported near MPB via a new Solution-Ignition Synthesis route and has been characterized by FT-IR, XRD, TG/DTG/DTA and SEM techniques. X-ray line broadening and Scherrer formula show crystallite size to be^20 nm. Densities of nano-crystalline spatulae of PZT in pellet form made by using 2% PVA and without PVA have been found to be 5.35 and 7.51 gm/cm3 respec-tively compared to the theoretical value of 7.78 gm/cm3. Dielectric constants of 83 and 227 of these spatulae with dielectric loss 0.118 and 0.0609 at 1 MHz and a high resistivity value of 3.043 * 107 Ω cm for PZT pellets made without PVA suggest these nano-crystalline PZT spatu-lae to be the potential candidates for high fre-quency applications.

Active Flow control of a Low Reynolds Number S809 Wind Turbine Blade Model under Dynamic Pitching Maneuvers

A low Reynolds number wind turbine blade model based on the S809 airfoil was tested in a subsonic wind tunnel to study the structural vibration of the blade under dynamic pitching maneuvers. Piezoelectric-based synthetic jet actuators were embedded inside the blade and activated with a synthetic jet momentum coefficient, cμ of 2.30 × 10-3. Structural vibration was quantified for a range of unsteady angles undergoing “pitch up and down” and “sinusoidal pitch” maneuvers at a Reynolds number of 5.28 × 104. The blade tip deflection amplitude and frequency were acquired utilizing a pair of strain gauges mounted at the root of the model. Using active flow control vibration reduction was more effective during the pitch up portion of the blade motion cycle compared to the pitch down portion. This effect is due to dynamic stall, where a leading edge vortex is shed during the pitch up motion and contributes to higher lift compared to static angles of attack and lower lift when the blade is pitched down. Dynamic stall was measured with phase-locked stereoscopic particle image velocimetry (SPIV), where global mean flow measurements reveal a shift in location and reduction in the size of a recirculating flow structure near the suction surface of the blade during the pitch up motion compared to the pitch down.

Study of Heavy Quarkonium with Energy Dependent Potential

Heavy quark systems (c and b ) have been studied in the nonrelativistic framework using energy dependent interquark potential of the form harmonic oscillator with a small linear term as energy dependent as perturbation plus a inverse square potential. This potential admits exact analytical solution of the Schrodinger equation. Mass spectra, leptonic decay width, root mean square radii (), the expectation value of the radius (r) and have been estimated for different quantum mechanical states for and systems. It is observed that energy dependent term in the potential leads to saturation of the mass spectra and degree of saturation is governed by the magnitude of perturbation. The calculated values of leptonic decay widths for 1s state are in very good agreement with the experimental data both for c and b systems.

Use of the tumor necrosis factor-blockers for crohn's disease

The use of anti-tumor necrosis factor-α therapy for inflammatory bowel disease represents the most important advance in the care of these patients since the publication of the National co-operative crohn＇s disease study thirty years ago. The recommendations of numerous consensus groups worldwide are now supported by a wealth of clinical trials and several meta-analyses. In general, it is suggested that tumor necrosis factor-c~ blockers （TNFBs） are indicated （1） for persons with moderately-severe crohn＇s disease or ulcerative colitis （Uc） who have failed two or more causes of glucocorticosteroids and an acceptably long cause （8 wk to 12 wk） of an immune modulator such as azathioprine or methotrexate; （2） non-responsive perianal disease; and （3） severe Uc not responding to a 3-d to 5-d course of steroids. Once TNFBs have been introduced and the patient is responsive, therapy given by the IV and Sc rate must be continued. It remains open to definitive evidence if concomitant immune modulators are required with TNFB maintenance ther- apy, and when or if TNFB may be weaned and discon- tinued. The supportive evidence from a single study on the role of early versus later introduction of TNFB in the course of a patient＇s illness needs to be confirmed. The risk/benefit profile of TNFB appears to be accept- able as long as the patient is immunized and tested for tuberculosis and viral hepatitis before the initiation of TNFB, and as long as the long-term adverse effects on the development of lymphoma and other tumors do not prone to be problematic. Because the rates of ben- efits to TNFB are modest from a population perspec- tive and the cost of therapy is very high, the ultimate application of use of TNFBs will likely be established by cost/benefit studies.

Selective cluster-Based Temperature Monitoring System for Homogeneous Wireless Sensor Networks

Over the past few decades, there has been a revolution in IcT, and this has led to the evolution of wireless sensor networks （WSN）, in particular, wireless body area networks. Such networks comprise a specialized collection of sensor nodes （SNs） that may be deployed randomly in a body area network to collect data from the human body. In a health monitoring system, it may be es-sential to maintain constant environmental conditions within a specific area in the hospital. In this paper, we propose a tempera-ture-monitoring system and describe a case study of a health-monitoring system for patents critically ill with the same disease and in the same environment. We propose Enhanced LEAcH Selective cluster （E-LEAcH-Sc） routing protocol for monitoring the tem-perature of an area in a hospital. We modified existing Selective cluster LEAcH protocol by using a fixed-distance-based thresh-old to divide the coverage region in two subregions. Direct data transmission and selective cluster-based data transmission ap-proaches were used to provide short-range and long-distance coverage for the collection of data from the body of ill patients. Ex-tensive simulations were run by varying the ratio of node densities of the two subregions in the health-monitoring system. Last Node Alive （LNA）, which is a measure of network lifespan, was the parameter for evaluating the performance of the proposed scheme. The simulation results show that the proposed scheme significantly increases network lifespan compared with traditional LEAcH and LEAcH-Sc protocols, which by themselves improve the overall performance of the health-monitoring system.

Soil erosion is a complex process involving multiple natural and anthropic agents,causing the deterio-ration of multiple components comprising soil ***,we provide an estimate of the spatial pat-terns of cropland susceptibility to erosion by sheet and rill,gully,wind,tillage,and root crops harvesting and report the co-occurrence of these processes using a multi-model *** addition,to give a global overview of potential future changes,we identify the locations where these multiple concurrent soil erosion processes may be expected to intersect with projected dry/wet climate changes by *** a modelled 1.48 billion hectares(B ha)of global cropland,our results indicate that 0.56 B ha(-36%of the total area)are highly susceptible(classes 4 and 5)to a single erosion process,0.27 B ha(-18%of the total area)to two processes and 0.02 B ha(1.4%of the total area)to three or more *** estimated 0.82 B ha of croplands are susceptible to possible increases in water(0.68 B ha)and wind(0.14 B ha)*** contend that the presented set of estimates represents a basis for enhancing our founda-tional knowledge on the geography of soil erosion at the global *** generated insight on multiple erosion processes can be a useful starting point for decision-makers working with ex-post and ex-ante policy evaluation of the UN Sustainable Development Goal 15(Life on Land)***,this work provides the hitherto most comprehensive assessment of soil erosion risks at the global scale,based on state-of-the-art models.

Morbidly Adherent Placenta (MAP): Lessons learnt

context: Once a rare occurrence, MAP is becoming an increasing threat to maternal lives. Aims: To summarize our experience in the management of patients with morbidly adherent placenta. Introduction: MAP is a potentially life threatening hemorrhagic condition responsible for 7% - 10% maternal mortality. Settings and Design: Tertiary care center. Methods and Material: Retrospective study in which data of twelve patients with clinical diagnosis of morbidly adherent placenta was reviewed from Jan 2009 till Sept 2012. Results: The incidence of placenta accreta was found to be increasing every year. Out of twelve cases with clinical diagnosis of MAP, placenta previa was present in 10/12 patients with MAP. All patients had history of previous section. Two patients with preoperative diagnosis of MAP on USG/MRI were found to be normal intra-operative and in one patient focal accreta was diagnosed intraoperatively. Nine patients of MAP underwent caesarean hysterectomy due to excessive bleeding during placental separation and were confirmed histo-pathologically (3 accreta vera, 3 increta and 3 percreta). Internal iliac artery ligation was done in 2 patients. Two patients with placenta percreta had bladder rupture which was repaired and these two patients subsequently expired. conclusions: The incidence of placenta accreta is increasing due to higher cesarean section (c/S) rate. Key to successful outcome is awareness, anticipation, preoperative counseling, planning and multidisciplinary approach.
