A Phase II study of Antineoplastons A10 and As2-1 in Children with Low-Grade Astrocytomas—Final Report (Protocol BT-13)

Nonresectable Low-Grade Astrocytomas (LGA) can compromise function and threaten life. For the majority of patients, the most appropriate strategy is initial chemotherapy followed by Radiation Therapy (RT). since curative treatment is not available for most of these patients, it is reasonable to conduct clinical studies to evaluate new agents. This Phase II study evaluates efficacy and safety of Antineoplastons A10 and As2-1 (ANP) in LGA. sixteen children diagnosed with LGA were treated. They included 12 males and 4 females, ages 1.6 - 17.4 years (median 10.6). Efficacy was evaluated in 16 patients. The majority of patients were previously treated, but 1 patient had stereotactic biopsy only. Out of the remaining 15 patients, 6 patients received chemotherapy, and 7 patients had surgery, and 2 patients received RT and chemotherapy after surgery. The patients received treatment with ANP administered daily every 4 hours (median dose of A10 was 7.71 g/kg/d and As2-1 was 0.26 g/kg/d) until objective response or stable disease was documented and for 8 months thereafter. The duration of ANP IV ranged from 1.4 to 286 weeks with a median of 83 weeks. A complete response was documented in 25.0%, partial response in 12.5%, and stable disease in 37.5%. Overall survival was 67.7% at 5 years, and 54.2% at 10 and 15 years. Progression-free survival was 48.1%, 34.4% and 34.4% at 5, 10, and 15 years respectively. The treatment was associated with grade 3 or grade 4 Adverse Drug Experiences (ADE) in 6 patients. There were two hypernatremias of grade 4 (12%). Grade 3 ADE included urinary frequency (6%), fatigue (6%) and hypernatremia (6%). There were no chronic toxicities, and there was a high quality of survival. ANP shows efficacy with a very good toxicity profile in this cohort of children with low-grade astrocytoma.

General anesthesia with controlled ventilation through endotracheal tube is an anaesthetic technique used by the anesthesiologists all over the world. Laryngoscopy and endotracheal intubation is necessary to achieve this technique. Traditionally succinylcholine is drug of choice for induction due to its rapid onset of action. Aim: This study compared the efficiency of magnesium sulphate in reducing fasciculation and increase in serum potassium which are the common complications associated with suxamethonium administration. Methods: Fifty six patients being planned for lower abdominal surgery under general anaesthesia were randomized into two groups M and s. serum potassium was taken in the theatre, three minutes before induction of anesthesia patients in M group were given 6 mg/kg of magnesium sulphate diluted in 10 ml normal saline intravenously while in s group 10 ml of normal saline was injected intravenously. The occurrence and degree of fasciculation were recorded after administration of suxamethonium in both groups while serum potassium was also taken at the end of fasciculations. The researcher was blinded to the content of the syringes until after the study. Results: More patients in the s group had fasciculations compared to the M group while the degree of fasciculations was more in the s group, the serum potassium increase was more in the saline group.

Most of the assessment equations for Ct which is a wellknown fracture parameter characterizing high temperature crack growth rates, have limited applicability to constant load conditions after sudden loading. However, crack growth due to creep can also occur under load varying conditions when load rising time is so long that accumulated creep deformation near the crack tip is not *** this paper, the estimation equation of the Ctparameter which can be applied to the case of slow load rising, i.e., (Ct)r, is explained. And the correlation between (Ct)r at the end of the load increasing period and Ct at the beginning of the succeeding load hold period is discussed. Finite element analyses of several cases with various loading conditions were performed and results were presented to show the effectiveness of the proposed Ct estimation scheme. The general applicability of the equation is also discussed.

A subspace expanding technique for global zero finding of multi-degree-of-freedom nonlinear systems

A subspace expanding technique(sET) is proposed to efficiently discover and find all zeros of nonlinear functions in multi-degree-of-freedom(MDOF) engineering systems by discretizing the space into smaller subdomains, which are called cells. The covering set of the cells is identified by parallel calculations with the root bracketing method. The covering set can be found first in a low-dimensional subspace, and then gradually extended to higher dimensional spaces with the introduction of more equations and variables into the calculations. The results show that the proposed sET is highlyefficient for finding zeros in high-dimensional spaces. The subdivision technique of the cell mapping method is further used to refine the covering set, and the obtained numerical results of zeros are accurate. Three examples are further carried out to verify the applicability of the proposed method, and very good results are achieved. It is believed that the proposed method will significantly enhance the ability to study the stability, bifurcation,and optimization problems in complex MDOF nonlinear dynamic systems.

An amphiphile is a molecule composed of hydrophilic part and hydrophobic part, which are incompatible and tend to separate from each other. The tendency of separation is often promoted by addition of water and sometimes also oil. Under balanced conditions the mixtures form macroscopically homogeneous phases, including isotropic solution phases and liquid crystalline phases. Correlation of the amphiphile structure with its preferred phases could be understood with a simple geometric model[1], which defines a dimensionless Critical Packing Parameter (CPP) to describe the relative bulkiness of the hydrophobic part and the hydrophilic part in an amphiphile. With CPP increasing from a small value to a high value the amphiphile changes from hydrophilic to hydrophobic, its preferred phase structure from direct structures via lamellar structure to reverse structures. This model provides a basis for the molecular design of amphiphiles. To immobilize the microstructure of the phases formed by amphiphiles is a challenge for current material chemists. Techniques of both inorganic polymerization[2] and organic polymerization[3] have been developed. With organic polymerization the molecular design of polymerizable amphiphiles is critical for the successful immobilization of the vulnerable precursor microstructures.

CD8 T cells play critical roles in controlling intracellular pathogensand tumors. To accomplish this the small number of antigen-inexperienced naive CD8 T cells that are capable of recognizing the antigen of interest must first become primed. These initial activaion events occur while the responding T cells are sequestered within lymph nodes, but many of the cells subsequently egress into the periphery and migrate into other organs where they can fulfill their principle mission of identifying and eliminating infected or malignant target cells.
