目的检查创伤后应激障碍(PTSD)与CT检查之间的关系,以确定是否存在一种可解释两者关系的疾病模式。材料与方法本研究经Iowa大学的机构审查委员会和Iowa市退伍军人事务部医疗中心的研究与发展委员会批准。采用回顾性队列设计,以年龄为18～35岁的全国新退伍兵为研究对象。调查PTSD的存在与至少一次或多次的CT扫描、就医情况(如腹痛、头痛)、卫生服务利用的措施(例如初级保健、急诊室和心理健康访问)以及在PTSD诊断前后至少一次CT扫描的日常概率之间的关系。采用含有连续多变量广义线性混合效应模型的分析来检测PTSD和CT扫描间的独立关系。结果在全组结果中,13%(10018/76812)接受过至少一次CT扫描。全组共有21.1%(16182/76812)的退伍兵被确诊为PTSD,其中22.9%(3711/16182)的PTSD病人接受过至少一次CT扫描,而非PTSD病人接受至少一次CT扫描的比例为10.4%(6307/60630)(P<0.0001)。在连续模型中,解释CT扫描和PTSD关系的合并因子为创伤性脑损伤(优势比为3.54,P<0.0001)、腹痛(优势比为4.01,P<0.0001)和头痛(优势比为3.07,P<0.0001)。PTSD与急诊室(优势比为2.73,P<0.0001)和初级保健(优势比为2.38,P<0.0001)的高水平利用也有较强的关系。确诊PTSD之前行日常CT扫描的机会为确诊PTSD之后的7倍[日常机会,0.007(之前)∶0.001(之后),P<0.0001]。结论年轻的患有PTSD的退伍兵较未患PTSD的退伍兵会接受更多的CT扫描;确诊PTSD之前行CT扫描的可能性更高。

2.2剂量相关参考浓度范围对于TDM结果的解释,除了治疗参考浓度范围外还有另一个浓度范围,即所谓的剂量相关参考浓度范围。治疗参考浓度范围的应用是药效动力学问题,而剂量相关参考浓度范围的运用是药代动力学问题。后者比较的是测得的药物浓度与理论预期药物浓度范围。参照药代动力学研究,优先考虑没有合并用药或药物基因组异常的患者群(“正常”患者),在日维持剂量(Daily maintenance dose,D m)、给药间隔(Dosing interval,d i)、总清除率(Total clearance,CL)和生物利用度(Bioavailability,F)已知情况下,某种药物在某个“正常”患者体内的预期平均稳态浓度(Average steady-state concentration,C av)可通过下面的公式计算。

治疗药物监测(Therapeutic Drug Monitoring,TDM)通过定量测定和解释血药浓度以优化药物治疗。TDM着眼于药代动力学的个体差异,使个体化药物治疗成为可能。在精神病学和神经病学领域中,有可能明显获益于TDM的主要患者群体包括少年儿童、孕妇、老年患者、智障患者、药物滥用者、涉法精神病患者、已知或怀疑药代动力学异常的患者以及合并躯体疾病影响药代动力学的患者。常规剂量下治疗无效,用药依从性难以判断,药物耐受性不佳,以及可能存在药代动力学方面的药物-药物相互作用等情况都是治疗药物监测的典型指征。然而,只有将TDM充分整合到临床治疗过程中,才能发挥其优化药物治疗的潜在优势。为了向临床医生和实验室提供有效的TDM信息,神经精神药理学与药物精神病学协会(Arbeitsgemeinschaft für Neuropsychopharmakologie und Pharmakopsychiatrie,AGNP)的TDM专家组在2004年发表了第一版《精神科治疗药物监测指南》。2011年进行了更新之后,现在再次更新。遵循新版指南,可能会改善神经精神药物治疗的效果,加快很多患者的康复,并降低医疗费用。

To assess the peripheral refractive changes that occur when eye growth decelerates in *** We measured peripheral refraction and on-axis vitreous chamber depth on a total of 53 marmosets;36 treated monocularly for 12 weeks 9 hrs/day(26 with-5 D contact lenses,10 with+5 D)and 17 untreated *** the 26 marmosets treated with-5 D,10 wore contact lenses 9 hrs/day without interruptions and were mea-sured at the end of 10 weeks of treatment(T10)and after 4 weeks of recovery(R14).Eight had both contact lenses removed for 30 mins twice/day(mid-morning and mid-afternoon)during the first four weeks of treatment(early interruption group)and eight had the lenses removed during the second four weeks of treatment(late interruption group).The two interruption groups were measured after 4,8 and 12 weeks of treatment only(T4,T8,T12).
