a.quired immune deficiency syndrome (a.DS)-rela.ed lymphoma.(a.L) rema.ns the ma.n ca.se of a.DS-rela.ed dea.hs in the highly a.tive a.ti-retrovira. thera.y (Ha.RT) era. Recently, rea.ra.gement of MYC is a.socia.ed with poor prognosis in pa.ients with diffuse la.ge B-cell lymphoma. Here, we report a.ra.e ca.e of ga.trointestina. (GI)-a.L with MYC rea.ra.gements a.d coinfected with Epstein-Ba.r virus (EBV) infection presenting with va.ious endoscopic findings. a.38-yea.old homosexua. ma. who presented with a.emia.a.d wa. dia.nosed with a. huma. immunodeficiency virus infection for the first time. GI endoscopy revea.ed multiple dish-like lesions, ulcera.ions, bloody spots, nodula. ma.ses with a.tive bleeding in the stoma.h, erythema.ous fla. lesions in the duodenum, a.d multiple nodula. ma.ses in the colon a.d rectum. Ma.nified endoscopy with na.row ba.d ima.ing showed a.honeycomb-like pa.tern without irregula. microvessels in the dish-like lesions of the stoma.h. Biopsy specimens from the stoma.h, duodenum, colon, a.d rectum revea.ed diffuse la.ge B-cell lymphoma.concomita.t with EBV infection tha. wa. detected by high tissue EBV-polymera.e cha.n rea.tion levels a.d Epstein-Ba.r virus sma.l RNa. in situ hybridiza.ion. Fluorescence in situ hybridiza.ion a.a.ysis revea.ed a.fusion between the immunoglobulin hea.y cha.n (IgH) a.d c-MYC genes, but not between the IgH a.d BCL2 loci. a.ter 1-mo of trea.ment with Ha.RT a.d R-CHOP, endoscopic a.pea.a.ce improved rema.ka.ly, a.d the histologica. fea.ures of the biopsy specimens revea.ed no evidence of lymphoma. However, he died from multiple orga. fa.lure on the 139 th da. a.ter dia.nosis. The ca.se of his poor outcome ma. be rela.ed to MYC rea.ra.gement. The GI tra.t involvement in a.L is ra.ely reported, a.d its endoscopic findings a.e va.ious a.d ma. be different from those in non-a.DS GI lymphoma. thus, we a.so conducted a.litera.ure review of GI-a.L ca.es.

Identifying Cha.a.teristics of a.ults a.sent from a.Meta.olic Syndrome Checkup in Ja.a. Using CHa.D Dendrogra.s a.d Insura.ce Cla.m Da.a

To prevent the development of meta.olic syndrome a.ong a.ult members of the community, improving the pa.ticipa.ion ra.e in the specific medica. checkup (a.medica. exa.ina.ion focused on fa.tors a.socia.ed with meta.olic syndrome prior to the onset of lifestyle-rela.ed disea.e) is a. importa.t public hea.th issue. This study used cla.m da.a.a.d hea.th checkup da.a.a.d a.med to detect those community-dwelling a.ults who were lea.t likely to pa.ticipa.e in the specific medica. checkup. a.a.ysis included the medica. a.d hea.th checkup cha.t da.a.of 61,753 a.ults a.ed 60 - 69 yea.s (a. of a.ril 2011) who lived in Fukui prefecture, Ja.a.. The chi-squa.ed intera.tion wa. used to a.a.yze da.a. If a.person did not pa.ticipa.e in the specific medica. checkup, individua. wa. ca.egorized a. “a.sent.” Between a.ril 2012 a.d Ma.ch 2013, 66.3% of subjects were a.sent from the specific medica. checkup. Those most likely to be a.sent included those who were a.so a.sent a. the previous yea.’s checkup, those who were men, a.d those who did not ha.e a. exa.ina.ion for hypertension;87.9% of pa.ients who met a.l of these criteria.were a.sent. a.ong women who were a.sent a. the previous yea.’s checkup, the a.sentee ra.e differed by a.out 10.0% between those whose municipa.ities did (74.8%) or did not (84.2%) ha.e a.free checkup progra.. Our findings ma. help public hea.th professiona.s detect those who require intervention a.d to effectively a.d efficiently improve pa.ticipa.ion in the specific medica. checkup.

Objective: Most of the western music consists of a.melody a.d a. a.compa.iment. The melody is referred to a. the foreground, with the a.compa.iment the ba.kground. In visua. processing, the la.era. occipita. complex (LOC) is known to pa.ticipa.e in foreground a.d ba.kground segrega.ion. We investiga.ed the role of LOC in music processing with use of positron emission tomogra.hy (PET). Method: Musica.ly na.ve subjects listened to unfa.ilia. novel melodies with (a.compa.iment condition) a.d without the a.compa.iment (melodic condition). Using a.PET subtra.tion technique, we studied cha.ges in regiona. cerebra. blood flow (rCBF) during the a.compa.iment condition compa.ed to the melodic condition. Results: The a.compa.yment condition wa. a.socia.ed with bila.era. increa.e of rCBF a. the la.era. a.d media. surfa.es of both occipita. lobes, media. pa.ts of fusiform gyri, cingula.e gyri, precentra. gyri, insula. cortices, a.d cerebellum. During the melodic condition, the a.tiva.ion a. the a.terior a.d posterior portions of the tempora. lobes, media. surfa.e of the fronta. lobes, inferior fronta. gyri, orbitofronta. cortices, inferior pa.ieta. lobules, a.d cerebellum wa. observed. Conclusions: The LOC pa.ticipa.es in recognition of melody with a.compa.iment, a.phenomenon tha. ca. be rega.ded a. foreground a.d ba.kground segrega.ion in a.ditory processing. The fusiform cortex which wa. known to pa.ticipa.e in the color recognition might be a.tiva.ed by the recognition of flourish sounds by the a.compa.iment, compa.ed to melodic condition. It is supposed tha. the LOC a.d fusiform cortex pla. simila. functions beyond the difference of sensory moda.ities.

a.62-yea.-old ma. who ha. a.ute recta. obstruction due to a.la.ge recta. ca.cer is presented. He underwent emergency la.a.oscopic colostomy. We used the la.a.oscopic puncture needle to inject a.a.gesia.with the novel tra.speritonea. a.proa.h. In this procedure, both ultra.ound a.d la.a.oscopic ima.es a.sisted with the a.cura.e injection of a.a.gesic to the correct la.er. The combina.ion of la.a.oscopic visua.iza.ion a.d ultra.ound ima.ing ensured infiltra.ion of a.a.gesic into the correct la.er without ca.sing da.a.e to the bowel. Twentyfour hours postopera.ively, the pa.ient's pa.n intensity a. a.sessed by the numeric ra.ing sca.e wa. 0-1 during coughing, a.d a.continuous intra.enous a.a.gesic wa. not needed. Colostomy is often necessa.y in colon obstruction. Epidura. a.esthesia.for postopera.ive pa.n ca.not be used in pa.ients with a.coa.ula.ion disorder. We report the use of a.novel la.a.oscopic rectus shea.h block for colostomy. There ha. been no litera.ure described a.out the nerve block with tra.speritonea. a.proa.h. The la.a.oscopic rectus shea.h block wa. performed sa.ely a.d ha. enough a.a.gesic effica.y for postopera.ive pa.n. This technique could be considered a. a. optiona. a.esthetic regimen in a.ute situa.ions.

a.giolipoma.is a.common benign tumor with a.cha.- a.teristic va.cula. component tha. usua.ly occurs in subcuta.eous tissue. a.though lipoma. a.e frequently encountered a. colonoscopy a. submucosa. tumors, a.- giolipoma. a.e ra.ely found in the ga.trointestina. tra.t including the la.ge intestine. Here we report a.77-yea. old Ja.a.ese ma. who underwent tra.sa.a. resection of a.tumor tha. wa. dia.nosed tenta.ively a. a.leiomyoma. Histologica.ly, the tumor consisted of ma.ure fa. cells a.d blood vessels. Immunohistochemica.ly, the tumor cells were nega.ive for c-kit a.d HMB-45, which a.e consis- tently expressed in tumors such a. ga.trointestina. stro- ma. tumor a.d a.giomyolipoma. The tumor wa. there- fore dia.nosed a. a. a.giolipoma. This is thought to be the first ca.e report of a.surgica.ly resected a.giolipoma.of the rectum.

Using sea.a.er in concrete ca. be considered a. one of the susta.na.le a.proa.hes in construction industry not only to sa.e the freshwa.er resource but a.so to promote the use of a.a.doned sea.a.er resource, especia.ly in the construction a. the uninha.ited a.ea.close to the sea.where the procurement of fresh wa.er is difficult. In this study, dura.ility a.a.nst chloride a.ta.k of sea.a.er mixed concrete with different repla.ement ra.io of BFS （bla.t furna.e sla.） a.d Fa.（fly a.h） is discussed a.d the life time until the occurrence of corrosion cra.k is eva.ua.ed. The results show tha.：（1） Chloride penetra.ion ra.e of sea.a.er mixed specimens with BFS a.d Fa.is lower tha. tha. of freshwa.er mixed OPC （ordina.y Portla.d cement） specimens; （2） Oxygen permea.ility of sea.a.er mixed specimens with BFS a.d Fa.is a.most the sa.e or lower tha. tha. of freshwa.er mixed OPC specimens; （3） Tota. life time （corrosion incuba.ion period a.d propa.a.ion period） of sea.a.er mixed specimens with BFS a.d Fa.is a.most the sa.e or only slightly shorter tha. tha. of freshwa.er mixed OPC specimens. From the results, it wa. confirmed tha. the usa.e of sea.a.er in concrete mixing is fea.ible in concrete with the a.propria.e BFS a.d Fa.repla.ement ra.io.

a.M: To determine the effect of discontinuing nonsteroida. a.tiinfla.ma.ory drugs(NSa.Ds) on recurrence in long-term follow-up pa.ients with colonic diverticula. bleeding(CDB).METHODS: a.cohort of 132 pa.ients hospita.ized for CDB exa.ined by colonoscopy wa. prospectively enrolled. Comorbidities, lifestyle, a.d medica.ions(NSa.Ds, low-dose a.pirin, a.tipla.elet a.ents, a.ticoa.ula.ts, a.eta.inophen, a.d corticosteroids) were a.sessed. a.ter discha.ge, pa.ients were requested to visit the hospita. on scheduled da.s during the followup period. The Ka.la.-Meier method wa. used to estima.e ***: Media. follow-up wa. 15 mo. The proba.ility of recurrence a. 1, 6, 12, a.d 24 mo wa. 3.1%, 19%, 27%, a.d 38%, respectively. Of the 41 NSa.D users on a.mission, 26(63%) discontinued NSa.D use a. discha.ge. Ma.y of the pa.ients who could discontinue NSa.Ds were intermittent users, a.d could be switched to a.terna.ive thera.ies, such a. a.eta.inophen or a. a.tiinfla.ma.ory a.a.gesic pla.ter. The proba.ility of recurrence a. 12 mo wa. 9.4% in discontinuing NSa.D users compa.ed with 77% in continuing users(P a.k test). The ha.a.d ra.io for recurrence in the discontinuing NSa.Ds users wa. 0.06 a.ter a.justing for a.e > 70 yea.s, right-sided diverticula. history of hypertension, a.d hemodia.ysis.
