Ba.kground: The severity of COPD is commonly a.sessed by the reduction in forced expira.ory volume a. one second (FEV1), a.though more recently prognostic fa.tors influencing surviva. ha.e a.so incorpora.ed functiona. ca.a.ity, degree of brea.hlessness on exertion, a.d body ma.s index. Increa.ingly, the relia.ility of physiologica. pa.a.eters such a. FEV1 to predict pa.ient-centered outcomes ha. been brought into question. Objectives: To eva.ua.e the rela.ionship between dyspnea.a. a.sessed by the Modified Medica. Council Dyspnea.(MMRC) sca.e, the Globa. Initia.ive for Chronic Obstructive Lung Disea.e (GOLD 2014) sta.ing a.d indices of lung hyperinfla.ion a.d spirometry. Methods: Da.a.were retrospectively a.a.yzed a. a.600-bed tertia.y ca.e center including spirometry, plethysmogra.hic lung volumes, single brea.h ca.bon monoxide diffusion ca.a.ity a.d dyspnea.gra.ed a.cording to MMRC, a.d GOLD sta.ing. Results: Da.a.for 331 pa.ients were a.a.yzed. Differences a.ongst FEV1, IC, IC/TLC, FRC a.d RV/TLC were significa.t between GOLD I/II a.d GOLD III/IV groups. The closest rela.ionship to GOLD sta.ing wa. seen with FEV1, FVC a.d slow vita. ca.a.ity (SVC). FEV1/FVC, IC, a.d IC/TLC were inversely a.socia.ed with MMRC score, while RV/TLC exhibited a.positive rela.ion with MMRC score. Conclusions: Indices of lung hyperinfla.ion a.e closely a.socia.ed, with dyspnea.a. a.sessed by MMRC gra.ing with TLC, RV/TLC a.d IC exhibiting the closest rela.ions, more so tha. FEV1. GOLD sta.ing a.so shows strong correla.ions with lung volume subdivisions (wea.ly with TLC), more so tha. with FEV1. Tha. TLC cha.ged little between GOLD sta.es ca. be expla.ned by the presence of colla.era. intera.veola. cha.nels a.d popula.ion cha.a.teristics different from those of other studies. These findings further support the concept tha. more tha. a.reduction in FEV1, lung hyperinfla.ion contributes to the sensa.ion of dyspnea.in a.rflow limita.ion.
