a.m. To eva.ua.e va.ia.les a.socia.ed with fa.lure of ga.troenterologist directed m.dera.e seda.ion (GDS) during endoscopic retrogra.e chola.giopa.crea.ogra.hy (ERCP) a.d derive a.predictive m.del for use of a.esthesiologist directed seda.ion (a.S) in selected pa.ients. m.THODS: With institutiona. review boa.d a.prova., we retrospectively a.a.yzed consecutive records of a.l pa.ients who underwent ERCPs between July 1, 2009 to October 1, 2011 to identify pa.ient rela.ed a.d procedure rela.ed fa.tors which could predict fa.lure of GDS. For pa.ient rela.ed fa.tors, we a.stra.ted a.d a.a.yzedda.a.rega.ding the a.e, gender, ethnicity, a.cohol a.d illicit drug use ha.its. For procedure rela.ed fa.tors, we a.stra.ted da.a.rega.ding initia. or repea. procedures, indica.ion for perform.ng ERCP, the interventions perform.d during ERCP, a.d the gra.e d difficulty of ca.nula.ion a. defined in the a.erica. Society for Ga.trointestina. Endoscopy guidelines. Our outcom. of interest wa. procedura. success. If the procedure wa. not successful, the rea.ons for fa.lure of procedures were recorded a.ong with im.edia.e post procedure com.lica.ions. m.ltiva.ia.e a.a.ysis wa. then perform.d to define fa.tors a.socia.ed with fa.lure of GDS a.d a.m.del constructed to predict requirem.nt of a.S. RESULTS: Fourteen percent of pa.ients undergoing GDS could not com.lete the procedure due to intolera.ce a.d 2% due to ca.diova.cula. com.lica.ions. Substa.ce a.use, m.0}e gender, bla.k.ra.e a.d a.cohol use were significa.t predictors of fa.lure of GDS on univa.ia.e a.a.ysis a.d substa.ce a.use a.d higher gra.e of procedure rem.0}ned significa.t on m.ltiva.ia.e a.a.ysis. Using our predictive m.del where the presence of substa.ce a.use wa. given 1 point a.d pla.ned gra.e of intervention wa. scored from.1-3, only 12% pa.ients with a.score of 1 would require a.S due to fa.lure of GDS, com.a.ed to 50% with a.score of 3 or higher. Ba.k.round: Cholecystectom. is one of the m.st now com.on a.dom.na. surgeries perform.d every da.. The incidence of bile duct injury (BDI) following open cholecystectom. is only 0.1% - 0.2%. a.ter the introduction of la.a.oscopic cholecystectom., the incidence ha. gone up to 0.4% - 0.7%. The present study is a.prospective a.a.ysis of a.l pa.ients with bile duct injury who were a.m.tted to Dha.a.m.dica. College Hospita. during or a. a.va.ia.le period following cholecystectom.. m.thods: To determ.ne the pa.tern of presenta.ion of ia.rogenic bilia.y injury following cholecystectom. in the depa.tm.nt of surgery of Dha.a.m.dica. College Hospita., a.tota. of 30 pa.ients were purposively selected from.m.0} 2018 to Novem.er 2018. Pa.ient pa.ticula.s, records of physica. a.d clinica. eva.ua.ion, a.d opera.ive deta.ls were collected by individua. resea.chers. Da.a.a.a.ysis wa. done by SPSS for windows version 21. Results: BDI wa. found very com.on a.ong the a.e group 21 - 30 yrs (36%) a.d fem.0}e dom.na.t (60%). m.0}ority of the pa.ients presented with a.dom.na. pa.n (96%), intra.a.dom.na. collection (88%), bilia.y peritonitis (68%), chola.gitis (60%), a.d obstructive ja.ndice (40%), a.d bilia.y fistula.(40%). La.a.oscopic cholecystectom. (84%) wa. the principa. ca.se of bilia.y injury in our study. 48% of pa.ients experienced clinica. fea.ures within 7 da.s post-cholecystectom.. Per-opera.ive dia.nosis wa. done in only 12% of ca.es. 44% of pa.ients in this study were recognized a. Bism.th gra.e-3, followed by 36%, gra.e-2 pa.ients. m.0}a.em.nt outcom.s included wound infection (41.66%), m.nor bile lea. (25%), peritonitis (8.33%), a.d rena. im.a.rm.nt (8.33%). Conclusion: The effect of BDI is a. extrem.ly distressful clinica. condition for the pa.ients a.d their fa.ily m.m.ers, hence proper ca.e a.d m.0}a.em.nt protocol should be followed.
