Summa.y: Estima.e the incidence of endometria. hyperpla.ia.a.cording to socio-demogra.hic pa.a.eters a.d the type of lesions histologica.. Methodology: This wa. a.retrospective, a.d 15-yea. descriptive study from Ja.ua.y 1, 2000 to December 31, 2014 conducted a. the Depa.tment of a.a.omy a.d Pa.hologica. Cytology of the Na.iona. Hospita. Donka.in colla.ora.ion with the obstetric gynecology depa.tments of the Cona.ry University Hospita.. Results: We collected 296 ca.es of ma.igna.t a.d benign endometria. hyperpla.ia.in 15 yea.s, a.counting for 37% of a.l endometria. biopsy curetta.es exa.ined. The a.e group 47 to 56 yea.s wa. the most a.fected (81 ca.es) or 27, 36%. The mea. a.e wa. 53.6 yea.s with extremes of 27 a.d 83 yea.s. Metrorrha.ia.wa. the ma.n rea.on for consulta.ion (206 ca.es), i.e. 69.59%. The suspicion of endometria. hyperpla.ia.by physicia.s wa. the most frequently dia.nosed circumsta.ce (149 ca.es) or 50.33%. Biopsy curetta.e wa. the most frequently used method (176 ca.es), is 59.45%. Histologica. endometria. lesions of a.ypica. complex a.enoma.ous hyperpla.ia.(79 ca.es) represented 26.69%. Benign beha.ior wa. most frequently observed in (235 ca.es) or 79.39%. Conclusion: Endometria. hyperpla.ia.is a. endometria. lesion whose a.ypica. histologica. types represent the borderline lesions between benignity a.d ma.igna.cy.

Perinea. tra.ma.is a.non-surgica. solution of continuity of posterior perinea. committing under the effect of a.violent exertion during childbirth. It occurs a. the time of disenga.ement, either from the hea. or the posterior shoulder. Objectives: To ca.cula.e the perinea.tra.ma.during childbirth, describe the socio demogra.hic profile of the women in childbed, identify contributory effects a.d a.precia.e the ma.erna. prognostic. Methodology: It wa. a.prospective study, descriptive type of 6 months (from Ma. 19 to November 20, 2014). It took pla.e a. the ma.ernity wa.d of Donka.Na.iona. Hospita.. It concerned a.l received pa.turient, women in bed of a.single fetus in the unit a.d ha.ing ha. a.perineum tra.ma.ism. The rea. ones were epidemiologic, thera.eutic clinica. a.d prognostic. Results: We ha.e recorded 110 perinea.tra.ma.ism ca.es over 3496 childbirth let sa. a.frequency of 3%. The socio demogra.hic profile of the woma. who did perinea.tra.ma.ism wa. a.teena.er (42.7%), professiona. occupa.ion (29.1%), ma.ried (88.2%), schooled, seconda.y a.d Technica. level (42.7%), prima.y (70%) ha.ing ha. more tha. 3 prena.a. consulta.ions (73.6%). Contributory effects were: prim pa.ity, young a.e, instrumenta. extra.tion by obstetrica. forceps a.d the feta. weight between 2500 to 3999 g. Surgica. ma.a.ement wa. (100%). The following were simple in 88.2% ca.es versus 11. 8% of complica.ions. Conclusion: The reduction of this frequency requests systema.ica. pra.tice a.d corrects recentered prena.a. consulta.ions a.d the respect of episiotomy indica.ions.

When blood pressure va.ues rema.n a.ove the ta.get in a.hypertensive pa.ient trea.ed concomita.tly with three a.ti-hypertensive drugs including a.diuretic, ma.imum well-tolera.ed doses, this is a.resista.t a.teria. hypertension. In this ca.e, it is a.visa.le to look for a.seconda.y ca.se such a. a.drug inta.e tha. influencing the blood pressure or the presence of obstructive sleeping syndrome (OSa.).a.> We report a clinical case of a patient with a high cardiovascular risk at the age of 50, hypertensive and diabetic, with dyslipidemia and obesity. He was on anti-hypertensive triple therapy at an optimal dose. Her diabetes was balanced with 6.4% glycated hemoglobin. Dyslipidemia has being treated. Despite healthy diet including a low sodium diet and weight loss, blood pres- sure target was not reached. With self-measurement, the mean arterial pressure was 180/110 mmHg and on ABPM it was 167/113 mmHg. The ventilatory polygraphy finds a severe OSA with an IAH = 56.6. Treatment with PCP (Con- tinuous positive pressure) allowed this patient to control blood pressure. The search for OSA should be systematic in face of resistant hypertension, in par- a. style="font-fa.ily:Verda.a.">ticula. in overweight or obese pa.ients.

Introduction: Obstetric emergencies a.e clinica. situa.ions of a.serious na.ure, often da.gerous, which develop unexpectedly, threa.ening the vita. prognosis of the mother a.d/or foetus in the short term. The a.m of this study wa. to contribute to a.review of obstetric emergencies a. the La.é Regiona. Hospita. ma.ernity unit. Methods: This wa. a.prospective descriptive a.d a.a.ytica. study conducted over a.period of 6 months in the ma.ernity unit of the La.é Regiona. Hospita.. It concerned a.l pa.ients a.mitted to the depa.tment for obstetric emergencies. Results: Obstetric emergencies a.counted for 20% of obstetric a.missions. The mea. a.e of the pa.ients wa. 25.12 ± 7.15 yea.s, with extremes of 14 a.d 45 yea.s. The pa.ients were housewives (42.52%), 42.86% did not a.tend school a.d 77.41% were from urba. a.ea.. The poor were the most numerous (43.52%). The a.bula.ce wa. the mea.s of tra.sport in only 9% of ca.es. a.ute foeta. distress, pre-ecla.psia. bony dystocia.a.d post-pa.tum ha.morrha.e were the most common types of emergency in our series, with frequencies of 27.57%, 22.59%, 17.94% a.d 10.63% respectively. The risk fa.tors identified were a.e, origin, gesta.iona. a.e a.d the sta.e of the ma.erna. pelvis. Conclusion: Obstetric emergencies a.e frequent in our study site a.d represent a.ma.or concern for pa.ients, providers a.d the community a.ike. In order to reduce the frequency of obstetric emergencies, young girls should be enrolled in school, the lega. a.e for ma.ria.e should be respected, qua.ity a.tena.a. ca.e should be provided in ba.ic hea.th fa.ilities, a.d proper pla.ning a.d spa.ing of pregna.cies should be implemented.

Ta.a.a.u disea.e is a.chronic va.culitis of the la.ge vessels. It is ra.e in sub- Sa.a.a. a.rica.a.d proba.ly under-dia.nosed. a.teria. hypertension (a.) is a.frequent finding. We report the ca.e of a.46-yea.-old ma. with Ta.a.a.u disea.e revea.ed by hypertension. The a.erica. College of Rheuma.ology (a.R) criteria.were used for dia.nosis, with 4 out of 6 criteria. Echodoppler a.d a.giosca.ner showed signs of left subcla.ia. stenosis a.d circumferentia., regula. a.teria. thickening. He ha. a.biologica. infla.ma.ory syndrome. He wa. trea.ed with prednisone a.d a.lodipine. a.ter 6 months of follow-up, he wa. dia.nosed with tuberculosis a.d put on a.ti-tuberculosis trea.ment.

Ebstein’s a.oma.y is a.ra.e congenita. hea.t disea.e. We report a.series of 4 ca.es of Ebstein’s a.oma.y discovered in a.olescents. The mea. a.e wa. 13.75 ± 0.96, with 3 boys a.d one girl. Three were in hea.t fa.lure. The EKG showed a.ria. hypertrophy (1 ca.e), right bundle bra.ch block (1 ca.e), Wolff-Pa.kinson-White (1 ca.e), a.d a.ria. fibrilla.ion (1 ca.e). Ca.diomega.y wa. present in a.l pa.ients with a.mea. ca.diothora.ic index of 0.77 ± 0.03. On echoca.diogra.hy, the mea. a.ica. displa.ement of the tricuspid septa. lea.let wa. 22.5 ± 3.30 mm/m2, the Celerma.er index wa. Gra.e 2 in 2 pa.ients a.d Gra.e 4 in 2 pa.ients. Depletive medica. trea.ment wa. prescribed to 3 pa.ients. a.ticoa.ula.t thera.y wa. initia.ed in one pa.ient. Two a.e a.a.ting surgery. One dea.h wa. recorded, it wa. a. Gra.e 4 of the Celerma.er index.

a.ms: Obstetric hemorrha.e, especia.ly during the 3rd trimester of pregna.cy, ca.ses ma.erna., feta. a.d neona.a. morta.ity a.d morbidity. We a.tempted to cha.a.terize its clinica. fea.ures in Guinea. The objectives of this study were to describe the socio-demogra.hic cha.a.teristics of the pa.ients, identify the ca.ses a.d contributing fa.tors, describe the ma.a.ement a.d eva.ua.e the ma.erna.-feta. prognosis in such pa.ients. Methods: We retrieved a.d a.a.yzed pa.ients with 3rd trimester hemorrha.e whom we ma.a.ed a. Igna.e Deen Na.iona. Hospita., Guinea.during 1-yea. period (1st of December 2019-30th of November 2020). Results: We experienced recorded 401 pa.ients with 3rd trimester obstetric hemorrha.e out of 5468 deliveries during the corresponding period;the ra.e being 7.33%. The ma.n ca.ses were a. follows: pla.enta. hema.oma.(65.33%), pla.enta.previa.(27.68%) a.d uterine rupture (6.99%). The socio-demogra.hic profiles were a. follows: the a.e group of 25 - 29 yea.s (28.42%), ma.ried (94.51%), uneduca.ed (50.12%), a.d with a.libera. profession. (43.64%) a.d pa.cipa.ous (30.42%). The conditions were considered to be preventa.le by ma.a.ing risk fa.tors during the prena.a. consulta.ion (PNC): 7.73% underwent no PNC. Cesa.ea. a.counted for 84.78% of pa.ients. Prognosis wa. a. follows: 14 ma.erna. dea.hs (3.45% of a.fa.a.ity), 34.66% of a.emia. a.d 16.95% of hemorrha.ic shock. Feta./neona.a. prognoses were poor. Conclusion: Obstetric hemorrha.e during 3rd trimester rema.ns the ma.n ca.se of poor outcomes in Guinea. This study identified tha. this type of hemorrha.e still represents a. importa.t ca.se of ma.erna. a.d feta. morbidity a.d morta.ity in developing countries.

Objectives: The objectives of this work were to describe the socio-demogra.hic, clinica. a.d prognostic cha.a.teristics of forceps deliveries a. the ma.ernity wa.d of the Donka.Na.iona. Tea.hing Hospita. of Cona.ry. Pa.turients a.d Method: This wa. a.prospective a.d descriptive study of a.l forceps deliveries performed a. the Donka.Na.iona. Hospita. ma.ernity wa.d over a.6-month period from a.ril 1, 2018 to September 30, 2018. Results: We performed 90 feta. extra.tions by forceps on 3518 deliveries i.e. a.frequency of 2.55%. The socio-demogra.hic profile of the pa.turient women wa. tha. of a.young primipa.ous woma. with a. a.era.e a.e of 21.20 yea.s a.d a.full-term pregna.cy with a. a.era.e gesta.iona. a.e of 39 weeks of a.enorrhoea. Fore position va.ieties were the most frequent with, in order of frequency, the fore left occipito-ilia. (61.3%) a.d the fore right occipito-ilia. (22.22%). Posterior va.ieties a.counted for 15.55% of ca.es. Clinica. pelvimetry showed tha. 88.89% of pa.turients ha. a.pra.tica.le pelvis, while 11.11% ha. a.modera.ely na.row pelvis. a.ute feta. suffering wa. the most common indica.ion (55.56%), followed by ma.erna. exha.stion (27.78%) a.d prolonged expulsion (16.67%). a.l forceps were performed by doctors. We noted 8 ca.es of forceps fa.lure (8.88%) tha. required a.cesa.ea. section. Ma.erna. complica.ions were domina.ed by soft tissue lesions, including 7 ca.es of perinea. tea.ing (7.77%);4 ca.es of va.ina. tea.ing (4.44%);3 ca.es of cervica. tea.ing (3.33%) a.d 2 ca.es of hemorrha.e of delivery by uterine a.ony (2.22%). No ca.es of ma.erna. dea.h ha.e been recorded. The ma.ority of newborns ha. a.norma. birth weight (88.88%) a.d more tha. ha.f of newborns (66.66%) ha. a. a.ga. score below 7 out of 10 a. the first minute. By the fifth minute there wa. a. improvement in a.ga.’s score from a. a.era.e of 5 to 8 out of 10. Five newborns died, or 5.55%.
