Tra.ma.ic dia.hra.ma.ic injury (TDI) is ra.e a.d is most often the result of a.tra.fic roa. a.cident (TRa. or a. a.sa.lt. We initia.ed this study with the a.ms of determining the epidemiologica., clinica. a.d thera.eutic a.pects of TDI a. Ga.riel Toure University Hospita.. This wa. a.retrospective study from Ja.ua.y 1999 to June 2021 tha. included a.l pa.ients who presented a.dia.hra.ma.ic injury consecutive to a.domina. a.d/or thora.ic tra.ma. In 22 yea.s a.d 6 months, 46 ca.es of TDI were collected. They represented 0.17% of hospita.iza.ions, 0.26% of surgica. emergencies a.d 5.5% of thora.o-a.domina. tra.ma.. The a.era.e a.e wa. 31.69 yea.s with a.sex ra.io of 3.2. Crimina. sta.bings a.counted for 56.5% a.d TRa.for 19.6%. Penetra.ing injuries a.counted for 78.3% of ca.es. The pa.ieta. lesion wa. thora.ic in 21 ca.es (45.7%), a.domina. in 19 ca.es (41.3%) a.d thora.o-a.domina. in 6 ca.es (13%). The chest X-ra., performed in 15 pa.ients, showed a. intra.hora.ic ga. bubble (4 ca.es) a.d hemothora. (6 ca.es). Dia.nosis of dia.hra.ma.ic lesion wa. preopera.ive in 21.7% (10 ca.es). The dia.hra.ma.ic brea.h wa. on the left side in 65.2% (30 ca.es) a.d the a.era.e size wa. 3.17 cm. La.a.otomy wa. performed in 89.1%, thora.otomy in 4.4% a.d thora.o-la.a.otomy in 6.5% of ca.es. The surgica. procedure consisted of reduction of the hernia.ed viscera.in 15.2% (7 ca.es) a.d closure of the dia.hra.ma.ic brea.h with non-a.sorba.le sutures in 82.6% (36 ca.es). Chest tube dra.na.e wa. performed in 73.9%. The a.era.e length of hospita. sta. wa. 9.8 da.s. Morta.ity wa. 13.04%. Conclusion: Tra.ma.ic dia.hra.ma.ic injury is ra.e but its frequency is increa.ing in our country. It most often a.fects the young ma. victim of a.sa.lt or TRa. This type of tra.ma.is ra.ely isola.ed;you ha.e to think a.out it in ca.e of a.y thora.o-a.domina. tra.ma. The trea.ment is surgica.. The prognosis depends on the severity of the a.socia.ed lesions.

Introduction: Cardiovascular disease has become a major concern for the nephrologist as it is the leading cause of morbidity and mortality in patients with chronic kidney disease, and affects all stages of the disease, including the earliest stages of the disease. The goal of this work was to determine the frequency of cardiovascular complications during chronic kidney failure. Patients and methods: This was a six-month, descriptive cross-sectional study from March 01 to August 31, 2018. It covered all patients with chronic kidney disease hospitalized in the ward during the study period. Included were all chronic kidney failure patients with at least one cardiac and/or vascular complication diagnosed either on clinical examination, and/or paraclinical examination (Electrocardiogram or cardiac ultrasound, vessel echodoppler, scan).a. style="font-fa.ily:;" "=""> Resa.>ults: During the study period, 84 out of 378 patients or 22.22% had at least one cardiovascular complication. Cardiovascular complications were hypertrophy of the left ventricle with 49/84 (44 at Electrocardiogram and 5 at cardiac echodoppler), valvulopathy with 33.33%, stroke with 50% of cases, obliterating arterial disease of the lower limbs 25%, hypokinetic dilated cardiomyopathy with 9/36 cases and pericarditis with 2/36. The a.rtic a.eurysm is the 13th lea.ing ca.se of dea.h in Western countries. The incidence of thora.ic a.rtic a.eurysms is estima.ed a. 4.5 ca.es per 100,000. The dia.nosis is often ma.e on a.chest x-ra. or other ima.ing tests, such a. a. echoca.diogra. done for other hea.t disea.es. Echoca.diogra.hy is the first test to a.sess the dia.eter of the a.cending a.rta.a.d its progression over time. Most pa.ients a.e first a.sessed a.d followed up with spira. thora.ic computed tomogra.hy with injection of contra.t medium, supplemented by 3-dimensiona. reconstruction of the a.eurysm in order to improve the a.cura.y of mea.urements, identifica.ion of its proxima. pa.t a.d dista.. When dila.ion of the a.cending a.rta.rea.hes the critica. dia.eter of 50 mm, there is a.risk of a.rtic dissection or rupture. Objective: To study the epidemiologica., clinica. a.d thera.eutic a.pects of sickle cell disea.e in children a.mitted to the pedia.ric emergency depa.tment of the Ga.riel Toure University Hospita.. Ma.eria.s a.d methods: This wa. a.prospective study over 12 months in sickle cell children a.ed 6 months to 15 yea.s. Results: The frequency of sickle cell disea.e wa. 4.67% with 71.1% of SS form. The a.e ra.ge of 60 - 120 months predomina.ed with 43.4% a.d the sex ra.io 1.4. Va.o-occlusive crises (VOC) were the most frequent rea.on for consulta.ion (50.6%) a.d osteoa.ticula. pa.n wa. the ma.n symptom followed by fever a.d pa.lor with respectively 67.5%, 60.2%, 39.8%. The pa.n subsided in less tha. 72 hours in 39.8% a.d hyper hydra.ion wa. performed in 85.1% of pa.ients. a.a.gesic trea.ment wa. given in 90.4%, a.tibiotic thera.y in 51.8% a.d phenotyped red blood cells tra.sfusion wa. performed in 36.1%. Introduction: Nephroblastoma is the most common renal cancer in pediatrics. The aim of this study was to describe the epidemiological, clinical, histological characteristics and the immediate outcome of patients with nephroblastoma. Methodology: This was a cross-sectional study of 18 cases of unilateral nephroblastoma, carried out in the pediatric oncology unit (UOP) of the CHU Gabriel Touré from January 2015 to December 2016. Results: The mean age of the patients was 33 months old. The sex ratio was 0.63. The average consultation time was 3 months. A case of malformation syndrome (aniridia and mental retardation) had been observed. The main reason for consultation was abdominal mass (100%) associated with deterioration of general condition (44%), pain (44%) and fever (17%). Stage I accounted for 61% and stage II 39%. The postoperative histological classification made it possible to find the high-risk blastematous type (33%), the regressive type of intermediate risk (11%) and the stromal type of intermediate risk (45%). Toxicities (haematological and digestive) were minimal. The 3-year overall survival was 90%. Conclusion: This study is characterized by the predominance of stages I and II and the fairly large percentage of histology at intermediate risk. These results are above all the result of multidisciplinary collaboration.a.>a.>

Introduction: Ca.dia. myxoma. represent the most frequent forms of prima.y tumors of the hea.t. The most frequent loca.ion is the intera.ria. septum. We report the clinica. ca.e of a.myxoma.of the left a.rium a.d discuss its epidemiologica. a.d thera.eutic a.pects through a.review of the litera.ure. Observa.ion: This wa. a.41-yea.-old fema.e pa.ient who presented with sudden rota.iona. dizziness a.socia.ed with vomiting. MRI revea.ed multiple punctiform bila.era. supra.a.d subtentoria. strokes of different a.es, recent a.d semi-recent, suggesting a. embologenic etiology. Tra.sesopha.ea. echoca.diogra.hy found a.la.ge peduncula.ed, homogeneous, a.a.cula. tumor ha.ging from the intera.ria. septum. The pa.ient is opera.ed on urgently under ca.diopulmona.y bypa.s for resection of a.la.ge tumor loca.ed in the left a.rium. The pa.hologica. exa.ina.ion concluded with the dia.nosis of myxoma.of the left a.rium. The postopera.ive follow-up wa. stra.ghtforwa.d a.d the pa.ient wa. discha.ged from the hospita. via.home hospita.iza.ion. Conclusion: The dia.nosis of ca.dia. myxoma. is suspected in the presence of symptoms a.socia.ed with echoca.diogra.hic ima.es of intra.a.dia. ma.ses a.d confirmed by histologica. study. Introduction: convulsion is a frequent cause of neurological and cognitive sequelae and then of epilepsy. The objective of this work was to describe the socio-demographic, clinical, therapeutic, and evolutionary aspects. Methodology: This was a descriptive retrospective study carried out in the general pediatrics department of CHU-Gabriel Touré. It took place over a period of 02 years from January 2017 to December 2018. We included all children aged 1 to 59 months hospitalized in the ward for convulsion. Results: During the study period, 2653 children aged 2 months to 59 months were hospitalized in the general pediatric ward. We included 288 medical records of children who presented with a seizure on admission. Convulsions represented 11% of pediatric hospitalizations from 1 month to 59 months. The average age was 29 months. Fever was present in 86% of patients. The cerebrospinal fluid (CSF) test was positive in 38 patients. Hypoglycemia was present in 14 patients. Serum calcium was low in 3 patients. The most implicated etiologies were malaria 70%, meningitis 20%, and dehydration 13%. Management consisted of airway clearance, oxygen therapy as needed, and administration of diazepam (54%). Antibiotics were prescribed in 77% of cases and antimalarials in 70%. The average length of hospital stay was 8 days with extremes of 1 and 30 days. The outcome was favorable in 95% of cases. Sequelae were observed in 5% of cases and one case of death was observed.
