a.ute Kidney Injury with Rha.domyolysis: 25 Yea.s Experience from a.Tertia.y Ca.e Center

Objective: To describe pa.ients presenting with a.ute kidney injury a.ter rha.domyolysis a. a.tertia.y rena. ca.e center in Pa.ista.. Pa.ients a.d Methods: a. observa.iona. cohort of pa.ients identified a. ha.ing a.ute kidney injury (a.I) with rha.domyolysis, which wa. dia.nosed by rise in crea.inine phosphokina.e (CK) a.d la.ta.e dehydrogena.e (LDH) more tha. 4 times the reference ra.ge wherea. a.I wa. defined a.cording to RIFLE criteria. On ultra.onogra.hy, a.l pa.ients ha. norma. size non obstructed kidneys, a.d no other co morbid. Results: Between Ja.ua.y1990 to December 2014, 334 pa.ients with rha.domyolysis a.d a.I registered to this hospita.. Mea. a.e wa. 28.22 ± 11.22 yea.s with M:F ra.io of 3.33:1. Mea. va.ues of CK a.d LDH were 597,749.790 ± 180,461.360 a.d 4077.026 ± 5050.704 U/L with reference ra.ge of 26 - 174 U/L a.d 91 - 180 U/L respectively. We divided the study popula.ion into 4 groups over timeline. Rha.domyolysis etiology wa. divided in 3 groups;1) tra.ma.ic, 2) non-tra.ma.ic exertiona., a.d 3) non-tra.ma.ic non-exertiona.. In the la.t group, which spa.s from 2010-2014, we trea.ed ma.y ca.es with toxic rha.domyolysis a.d ma.n toxin wa. pa.a.henylenedia.ine (PPD). The other ca.ses showed more or less sa.e preva.ence over two a.d a.ha.f deca.e, except non-tra.ma.ic exertiona. which ha. decrea.ed during la.t 5 yea.s without a.y expla.na.le ca.se. Rena. repla.ement thera.y (RRT) wa. required on a.riva. in 94% ca.es. Complete rena. recovery wa. observed in 70%, while 15.86% died a.d 10% were lost during recovery pha.e. a.sma.l number 2.69% left a.a.nst medica. a.vice during a.ute pha.e of illness a.d 0.8% developed chronic kidney disea.e (CKD). Conclusion: The common clinica. conditions found a.socia.ed with rha.domyolysis a.d a.I includes tra.ma. immobiliza.ion, sepsis, overexertion, a.d drugs a.d toxins. In recent yea.s, we ha.e seen ma.y young pa.ients with PPD poisoning;we ha.e found good rena. recovery in pa.ients who survived initia. 2 - 3 weeks.

Ba.kground: Poplitea. cysts a.e common a.d present a. a.ymptoma.ic lumps in the media. poplitea. fossa. Some ha.e complex interna. cha.a.teristics such a. septa.a.d loose-bodies. However, not a.l a.e poplitea. cysts a.d ca. be a.gressive. These lesions need to be differentia.ed by the a.sence of the communica.ing neck with the joint on ultra.ound. Presence of Doppler flow of non-communica.ing cysts requires further eva.ua.ion on MRI, prior to performing a.biopsy. Using a.ca.e series, we propose a. a.gorithmic a.proa.h tha. is simple a.d will help identify the ma.igna.t lesions a.d institute a.propria.e ma.a.ement. Ca.e-Presenta.ion: Poplitea. Cyst: On ultra.ound: cha.a.teristic neck communica.ing with knee joint. Synovia. Sa.coma. Ga.olinium enha.cement, with a.ea. of low-, iso- a.d hyper-intense signa. to fa. on T2. Synovia.-Osteochondroma.osis: Non-minera.ized: T1-low/intermedia.e intensity;T2-high intensity. Minera.ized type: low intensity on T1 & T2. Thrombosed Poplitea. a.eurysm: La.ella.ed a.pea.a.ce-high/low signa. intensity on T2. Myxoid-Liposa.coma.: Inhomogeneous a.pea.a.ce;homogenous with ga.olinium. Usua.ly require a.biopsy for dia.nosis. Conclusion: The cystic lesions in the media. a.pect of the poplitea. fossa.ca. be misdia.nosed. Our a.ticle reitera.es the importa.ce of the communica.ing neck tha. sepa.a.es poplitea. cysts from other mimics. We ha.e proposed a. a.gorithm to identify these mimics.

Ba.KGROUND Coa.ulopa.hy a.d thromboembolic events a.e a.socia.ed with poor outcomes in corona.irus disea.e 2019(COVID-19)*** is conflicting evidence on the effects of chronic a.ticoa.ula.ion on morta.ity a.d severity of COVID-19 *** To summa.ize the body of evidence on the effects of pre-hospita. a.ticoa.ula.ion on outcomes in COVID-19 *** a.Litera.ure sea.ch wa. performed on LitCovid PubMed,WHO,a.d Scopus da.a.a.es from inception(December 2019)till June 2023 for origina. studies reporting a. a.socia.ion between prior use of a.ticoa.ula.ts a.d pa.ient outcomes in a.ults with *** prima.y outcome wa. the risk of thromboembolic events in COVID-19 pa.ients ta.ing *** outcomes included COVID-19 disea.e severity,in terms of intensive ca.e unit a.mission or inva.ive mecha.ica. ventila.ion/intuba.ion requirement in pa.ients hospita.ized with COVID-19 infection,a.d *** ra.dom effects models were used to ca.cula.e crude a.d a.justed odds ra.ios(a.Rs)with 95%confidence interva.s(95%CIs).RESULTS Forty-six observa.iona. studies met our inclusion *** una.justed a.a.ysis found no a.socia.ion between prior a.ticoa.ula.ion a.d thromboembolic event risk[n=43851,9 studies,odds ra.io(OR)=0.67(0.22,2.07);P=0.49;I2=95%].The a.socia.ion between prior a.ticoa.ula.ion a.d disea.e severity wa. non-significa.t[n=186782;22 studies,OR=1.08(0.78,1.49);P=0.64;I2=89%].However,pre-hospita. a.ticoa.ula.ion significa.tly increa.ed a.l-ca.se morta.ity risk[n=207292;35 studies,OR=1.72(1.37,2.17);Pa.justed estima.es revea.ed a.sta.istica.ly non-significa.t a.socia.ion between pre-hospita. a.ticoa.ula.ion a.d thromboembolic event risk[a.R=0.87(0.42,1.80);P=0.71],morta.ity[a.R=0.94(0.84,1.05);P=0.31],a.d disea.e severity[a.R=0.96(0.72,1.26);P=0.76].CONCLUSION Prehospita. a.ticoa.ula.ion wa. not significa.tly a.socia.ed with reduced risk of thromboembolic events,improved surviva.,a.d lower disea.e seve
