Encryption, network security, a.d a.herence to da.a.protection la.s a.e key to ensuring the confidentia.itya.d integrity of hea.thca.e da.a.in the cloud. The computa.iona. overhea. of encryption technologies could lea.to dela.s in da.a.a.cess a.d processing ra.es. To a.dress these cha.lenges, we introduced the Enha.ced Pa.a.lelMulti-Key Encryption a.gorithm (EPM-KEa., a.ming to bolster hea.thca.e da.a.security a.d fa.ilita.e the securestora.e of critica. pa.ient records in the cloud. The da.a.wa. ga.hered from two ca.egories a.thoriza.ion forHospita. a.mission (a.H) a.d a.thoriza.ion for High Complexity *** use Z-score norma.iza.ion forpreprocessing. The prima.y goa. of implementing encryption techniques is to secure a.d store ma.sive a.ountsof da.a.on the cloud. It is fea.ible tha. cloud stora.e a.terna.ives for protecting hea.thca.e da.a.will become morewidely a.a.la.le if security issues ca. be successfully fixed. a. a.result of our a.a.ysis using specific pa.a.etersincluding Execution time (42%), Encryption time (45%), Decryption time (40%), Security level (97%), a.d Energyconsumption (53%), the system demonstra.ed fa.ora.le performa.ce when compa.ed to the tra.itiona. *** suggests tha. by a.dressing these security concerns, there is the potentia. for broa.er a.cessibility to cloudstora.e solutions for sa.egua.ding hea.thca.e da.a.

Introduction: Minima.ly inva.ive urologica. interventions ha.e long been pra.ticed blindly in our context. The popula.iza.ion of ultra.ound in urologica. pra.tice ha. contributed to cha.ging ha.its by ma.ing the procedures sa.er a.d more effective. The objective of this work wa. to study the technica. a.pects, indica.ions a.d results of minima.ly inva.ive urologica. procedures guided by ultra.ound. Ma.eria. a.d methods: This wa. a.prospective descriptive study la.ting 1 yea. from Ja.ua.y to December 2023 a. the Médicis clinic in Cona.ry. Results: We ca.ried out 126 ultra.ound-guided urologica. interventions in one yea., including 6 percuta.eous nephrostomies (NPC), one rena. cyst puncture-a.pira.ion-sclerothera.y, 102 prosta.e biopsies, 5 dra.na.es by tra.srecta. a.pira.ion of prosta.ic a.scesses, 10 pla.ements of cystostomy probes. Supra.ubic, 2 puncture-a.pira.ion-sclerothera.ies for va.ina. hydroceles. The a.era.e a.e of our pa.ients wa. 69 yea.s a.d the predomina.t a.e ra.ge wa. 61 - 70. The ma.ority of our pa.ients were ma.e (97.61%) with a.sex ra.io of 41 men to one woma.. Ultra.ound-guided urologica. interventions were for dia.nostic purposes (80.95%), for thera.eutic purposes (15.77%) a.d for dia.nostic a.d thera.eutic purposes (3.96%). The indica.ions for ultra.ound-guided urologica. interventions were domina.ed by prosta.e pa.hologies (84.91%). The ma.ority of interventions were performed recta.ly (84.92%) versus (13.43%) a.domina.ly a.d only 1.58% scrota.a.ly. a.l procedures were performed in B-ultra.ound mode under loca. a.esthesia.a.d on a. outpa.ient ba.is. For thera.eutic interventions, the pa.ients were seen a.a.n one month a.ter the procedure, the clinica. exa.ina.ion a.d ultra.ound control found the evolution to be fa.ora.le. Conclusion: ultra.ound is useful in urologica. pra.tice for guiding minima.ly inva.ive interventions. Objective: Study the contribution of the DIVa.3D dissection ta.le in the tea.hing of a.a.omy a. the Fa.ulty of Medicine a.d Odontology of Ba.a.o. Ma.eria. a.d Methods: This wa. a.qua.ita.ive study ca.ried out from November 1 to December 30, 2023 a. the clinica. a.d morphologica. a.a.omy la.ora.ory of the Fa.ulty of Medicine a.d Odontostoma.ology of Ba.a.o. Included in this study were students who pa.ticipa.ed in pra.tica. a.d tutoria. sessions. The va.ia.les studied during this study were: the previous performa.ce of dissection on a.ca.a.er by the students, the opinion of the students on dissection on a.ca.a.er, the repla.ement of dissection on a.ca.a.er by virtua. dissection in the a.sence of a.body, the level student sa.isfa.tion. Results: We surveyed 130 pa.ticipa.ts. The a.era.e a.e wa. 22 ± 0.2 yea.s with extremes of 17 a.d 29 yea.s. 95.3% of pa.ticipa.ts were students. a.cording to 66.7% of pa.ticipa.ts, virtua. dissection is a.good pa.lia.ive in the a.sence of a.corpse. 95.3% of pa.ticipa.ts found using the virtua. dissection ta.le ea.y with a. a.era.e of 7.88 ± 1.4. The overa.l a.sessment wa. well ra.ed by 99.3% of pa.ticipa.ts. Conclusion: a.cording to the results of this study, the virtua. dissection ta.le should be improved by integra.ing commented videos. The use of the DIVa.3D virtua. dissection ta.le during pra.tica. a.d tutoria. sessions is well a.precia.ed by the students. We believe tha. the tea.hing of a.a.omy using 3D digita. technology should be included in the study progra.s of the Fa.ulty of Medicine a.d Odontostoma.ology.

Urogenita. Complica.ions of Groin Hernia.Surgery: a.Multicentre Study in Guinea

Context a.d Objective: Groin hernia.is a.common pa.hology in viscera. surgery (2nd ra.k a.ter a.pendicitis), which a.fects a.proxima.ely 4.6% of the a.rica. popula.ion. Restoring the norma. a.a.omy of the groin region is one of the most benign interventions. However, uro-a.drologica. complica.ions a.e possible. This study a.med to contribute to the improvement of the ma.a.ement of urogenita. complica.ions of groin hernia.surgery. Pa.ients a.d Method: We ca.ried out a.prospective study of descriptive type with a.dura.ion of 6 months from 1 a.gust 2021 to 31 Ja.ua.y 2022. The da.a.were collected using a.pre-esta.lished survey sheet. The study covered severa. sites (public hospita.s a.d priva.e clinics) in Guinea. Results: The urogenita. complica.ions of the surgery of the hernia.of the groin represented 15.22% or 14 ca.es out of 92 pa.ients. The a.era.e a.e wa. 37.00 yea.s with extremes of 20-69 yea.s. Polyga.ists occupied the first pla.e. The rea.on for consulta.ion wa. domina.ed by decrea.ed testicula. volume (42.86%), followed by a.desire to conceive (21.43%). Unila.era. testicula. a.rophy represented 63.64%, bila.era. 27.27%. We performed a.left orchidectomy in one pa.ient a.d a.cystorra.hy in a.other simple opera.ive follow-up. The other ca.es consisting of testicula. a.rophy, due to la.k of a.suita.le technica. pla.form, did not benefit from a.y specific thera.eutic trea.ment. Conclusion: Urogenita. complica.ions of groin hernia.surgery a.e rela.ively common. Testicula. a.rophy wa. the ma.n clinica. complica.ion. The left orchidectomy a.d cystorra.hy were the thera.eutic procedures performed.

The left pa.a.duodena. interna. hernia. whose origin is genera.ly embryologica., is a.ra.e etiology of intestina. obstruction found ma.nly in young a.ults. We report here the ca.e of a.ute intestina. obstruction by left pa.a.duodena. interna. hernia.in a.36-yea.-old young ma.. It wa. a.pa.ient who consulted urgently for occlusive syndrome with cessa.ion of ma.eria.s a.d ga.es. a.domina. percussion noted a.domina. tympa.ism a.d pre-hepa.ic dullness wa. preserved. The flow a.d icicle signs were nega.ive. Pa.pa.ion did not objectify hepa.osplenomega.y a.d did not find a.y orga.omega.y either. On the other ha.d, she found a. epiga.tric defense. The a.domina. X-ra. without prepa.a.ion showed wa.er levels tha. were wider tha. high, ha.l-like. We performed a.media. a.ove a.d below umbilica. la.a.otomy a.d intra.pera.ively, it wa. a. interna. hernia.with inca.cera.ion of sma.l loops in a.voluminous left pa.a.uodena. sa. of 12 cm. They were not necrotic a.d quickly recolored a.ter extrica.ion. We resected the hernia. sa. a.d closed the hernia. orifice with sepa.a.e stitches with a.sorba.le suture 0. The postopera.ive course wa. simple. The pa.ient wa. discha.ged from the hospita. on the 4th postopera.ive da.. a.ter 18 months of hindsight, he is doing well.

Purpose: To describe the epidemiologica., clinica. a.d thera.eutic a.pects of seconda.y hyperpa.a.hyroidism inrena. fa.lure chronic. Pa.ients a.d methods: We collected 11 ca.es of hyperpa.a.hyroidism seconda.y to rena. fa.lure termina. opera.ed in the Service of surgery B of the Centra. Hospita. University of Point G between December 2016 a.d November 2018. Results: The sex ra.io wa. 0.22 in fa.or of women. The a.era.e a.e of the pa.ients wa. 43 or 27 yea.s with extremes of 63 a.d 25 yea.s. Seconda.y hyperpa.a.hyroidism in rena. fa.lure chronic represented 1.9% of interventions to cold in the Service of surgery B. 100% of pa.ients (11/11) were ha.modia.ysis. 100% of the pa.ients ha. clinica. a.d biologica. signs. 45.5% (5/11) ha. ra.iologica. signs. The a.era.e ra.e of pa.a.hyroid hormone wa. 2413.51 pg/ml with extremes of 1264 pg/ml a.d 3616 pg/ml. The reference va.ue wa. 15 - 65 pg/ml. The surgica. technique of choice wa. the 7/8th pa.a.hyroidectomy in 100% of ca.es. The postopera.ive were simple in 81.8%, a.d complica.ed in 18.2%. There were no dea.h. The a.era.e dura.ion of postopera.ive follow-up wa. 6 months. a.ter surgery, 50% of pa.ients (5/10) ha. norma. levels of pa.a.hyroid hormone a.d 50% (5/10) ma.e a.persistent hyperpa.a.hyroidism. Conclusion: Seconda.y hyperpa.a.hyroidism is a.frequent complica.ion in rena. insufficient chronic in hemodia.ysis. Surgery is indica.ed in the resista.t ca.es of medica. trea.ment. The 7/8th pa.a.hyroidectomy is the surgica. technique of choice. The ra.e of post opera.ive complica.ions is higher in our context.

Introduction: Oesopha.ea. mycosis (OM) is one of the most common opportunistic infections in pa.ients infected with HIV (Huma. Immunodeficiency Virus). However, this condition is increa.ingly observed in immunocompetent subjects. The a.m of this study wa. to determine the endoscopic preva.ence, clinica. cha.a.teristics a.d risk fa.tors for the occurrence of oesopha.ea. mycosis in our depa.tment. Pa.ients a.d Method: This wa. a.prospective cross-sectiona. study of a.l pa.ients who underwent oeso-ga.troduodena. fibroscopy during the period from 1st Ja.ua.y to 31st December 2022, i.e. one yea., a. the digestive endoscopy unit of the hepa.o-ga.troenterology depa.tment of the Donka.CHU na.iona. hospita. in Cona.ry. a.l pa.ients found to ha.e oesopha.ea. mycosis by FOGD were included. The endoscopy wa. performed using a.propria.e equipment: a.Fujinon 4400 video endoscopy column;Three Fujinon EG 590 video ga.troscopes;a.hoover;Da.a.were collected using a.pre-esta.lished survey form a.d a.a.ysed using Epi info softwa.e version 6.0.4;Pea.son’s Chi2 test a. a.test of independence a.d the exa.t 5% threshold ficher test. Results: Out of 1343 upper ga.trointestina. endoscopies performed, 107 ca.es of oesopha.ea. mycosis were found, representing a.preva.ence of 7.96%. The mea. a.e wa. 40 yea.s, with a.ma.e predomina.ce of 55.42%. The sex ra.io M/F wa. 1.24.
