Percutaneous image guided thermal ablation has become a cornerstone of therapy for patients with oligometastatic disease and primary liver *** from percutaneous ethanol injection(PEI),thermal ablation utilizing radiofrequency ablation(RFA)and microwave ablation(MWA)have become the standard approach in the treatment of isolated lesions that fit within the size criteria for curative intent therapy(typically 3-4 cm).With the evolution of more intense thermal ablation,such as MWA,the dramatic increase in both the size of ablation zone and intensity of heat generation have extended the limits of this *** a result of these innovations,intra-procedural and post-procedural pain have also significantly increased,requiring either higher levels of intravenous sedation or,in some institutions,general *** addition to the increase in therapeutic intensity,the use of intravenous sedation during aggressive ablation procedures carries the risk of over-sedation when the noxious insult(*** ablation)is removed,adding further difficulty to post-procedural recovery and ***,high subdiaphragmatic lesions become challenging in this setting due to issues relating to sedation and compliance with breath hold/breathing *** general anesthesia may mitigate these complications,the added resources associated with providing general anesthesia during ablation is not cost effective and may result in substantial delays in *** reduction of Aerosol Generating Medical Procedures(AGMP),such as intubation due to the COVID-19 Pandemic,must also be taken into *** to the potential increased risk of infection transmission,alternatives to general anesthesia should be considered when safe and *** abdominal regional nerve block techniques have been used to manage pain related to trauma,surgery,and cancer;however,blocks of this nature are not well described in the interventional radiology ***

Dysbetalipoproteinemia: Two cases report and a diagnostic algorithm

Dysbetalipoproteinemia is a rare familial dyslipidemia characterized by approximately equally elevated serum cholesterol and triglyceride levels due to accumulated remnant lipoproteins in apolipoprotein E2/E2 *** is associated with an increased risk for premature cardiovascular disease. Thus, making a diagnosis of dysbetalipoproteinemia aids in assessing cardiovascular risk correctly and allows for genetic counseling. However, the diagnostic work-up can be challenging. Diagnosis of dysbetalipoproteinemia should be considered in patients mixed dyslipidemia when the apolipoprotein B concentration is relatively low in relation to the total cholesterol concentration or when there is significant disparity between the calculated low density lipoprotein(LDL) and directly measured LDL cholesterol concentrations. Other indices are also informative in the diagnostic process. We present herein two phenotypically different cases(a 44-year-old man with severe hypertriglyceridemia and a 49-year-old woman with mixed dyslipidemia) of genotypically proven familial dysbetalipoproteinemia and a diagnostic algorithm of the disease.

Objective: To investigate the presence of autoantibody deposition against type 2 tissue transglutaminase (TG2; a reliable marker of the whole spectrum of gluten sensitivity) in the jejunal tissue and brain of patients with gluten ataxia and in control subjects. Methods: The authors evaluated jejunal biopsy samples from nine patients with gluten ataxia and seven patients with other causes of ataxia for the presence of TG2- related immunoglobulin deposits using double- color immunofluorescence. Autopsy brain tissue from one patient with gluten ataxia and one neurologically intact brain were also studied. Results: IgA deposition on jejunal TG2 was found in the jejunal tissue of all patients with gluten ataxia and in none of the controls. The intestinal IgA deposition pattern was similar to that seen in patients with overt and latent celiac disease and in those with dermatitis herpetiformis. Widespread IgA deposition around vessels was found in the brain of the patient with gluten ataxia but not the control brain. The deposition was most pronounced in the cerebellum, pons, and medulla. Conclusions: Anti- tissue transglutaminase IgA antibodies are present in the gut and brain of patients with gluten ataxia with or without an enteropathy in a similar fashion to patients with celiac disease, latent celiac disease, and dermatitis herpetiformis but not in ataxia control subjects. This finding strengthens the contention that gluten ataxia is immune mediated and belongs to the same spectrum of gluten sensitivity as celiac disease and dermatitis herpetiformis.

Background and objective:To report significant sociodemographic and behavioral outpatient characteristics associated with the diagnosis of genital warts. Methods: Cross-sectional hospital-based study (1990-96). Results: Genital warts (n = 2988, 51.2%) represent the leading sexually transmitted disease (STD) among 5831 consecutive symptomatic Greek and foreign immigrant STD outpatients. In Greek heterosexuals a low rate of partner change was the main patient characteristic at diagnosis (median: one partner in the past 6 months). Lower detection rate and riskier behavior characterized immigrants. Although associated with risky behavior, homo/bisexual orientation in males and injecting drug use were not significantly associated with condyloma diagnosis in the context of STDs. Conclusion: Broader health education and secondary prevention are needed to control this infection, whereas low-risk behavior in Greek heterosexuals with condylomata facilitates further preventive interventions.

Gluten sensitivity can manifest with ataxia. The metabolic status of the cere bellum was investigated in 15 patients with gluten ataxia and 10 controls using proton MR spectroscopy. Significant differences were present in mean N- acetyl aspartate levels at short echo time and N- acetyl aspartate/ choline ratios at long echo time between the patient and control groups. These data support the hy pothesis that cerebellar neuronal physiology differs between patients with glute n ataxia and healthy controls..

目的：在1型糖尿病患者中，下消化道症状不常出现，而且血糖控制和生活质量对其影响尚不清楚。本研究的目的是确定血糖控制和生活质量对下消化道症状患病率的影响，并研究其潜在的病因。研究设计和方法：次级治疗的前瞻性队列研究。1型糖尿病患者完成胃肠症状问卷，并填写36份V．2生活质量调查表，并进行血红蛋白Alc测定。腹泻患者按国家指南进行重新评估和调查。对照组为非糖尿病患者，比较消化道症状患病率和生活质量评分。
