＜正＞ High-quality crystals of large polycyclic aromatic molecules such as tetracene and pentacene are often used as charge transport layers of organic electronic devices. Such polycyclic aromatic molecules due to their large πsystems have the ability to accept an excess electron in a lowest unoccupied molecular orbital （LUMO）. Accurate experimental adiabatic electron affinity （EAa） values of those molecules are important for the energetics of electron transport processes and for comparison with theoretical EAa. In this work, we for the first time studied the electronic properties of tetracene anion and its homogenous clusters using anion photoelectron spectroscopy with a magnetic bottle photoelectron spectrometer. Tetracene anion and its homogeneous clusters were produced by using a high-pressure, high-temperature Even-Lavie pulsed valve combined with electron source. For tetracene anion, the most dominant peak was observed at 1.03±0.04 eV, which is attributed to the origin （vibrationless） transition from the anion ground state to the neutral one. This value corresponds to the EAa of isolated tetracene molecule and agrees well with that calculated using the method of density functional theory. This result shows that the excess electron occupies the LUMO of tetracene molecule. The excess electron states and energetics of the tetracene cluster anions will be also discussed in detail on the basis of the results of anion photoelectron spectroscopy.

Solitary Bone Cyst of the Proximal Humerus with a Concomitant Stiff Shoulder Treated with both Arthroscopic Capsular Release and Arthroscope-Assisted Resection of the Bone Cyst: A Case Report

Solitary bone cysts are benign bone lesions that usually occur in the humerus, calcaneus, and femur of children and adolescents. When present in adults, they are usually diagnosed as incidental findings. In this report, we present an adult case of a solitary bone cyst of the proximal humerus with concomitant refractory stiff shoulder treated with both arthroscopic capsular release and arthroscope-assisted resection of the bone cyst. A 73-year-old woman was referred with a complaint of persistent left shoulder pain and limited range of motion. Magnetic resonance imaging showed an approximately 4.5-cm long diameter cystic bone lesion from the humeral neck to the shaft. At first, arthroscopic synovectomy and pan-capsular release were performed. Next, two 1-cm skin incisions and 8-mm drill holes into the lateral wall of the cyst were created with fluoroscopic guidance. After insertion of the arthroscope through the holes, the white membranous cyst wall was resected with a shaver until bone marrow was observed. Finally, an 8-mm cannulated hydroxyapatite pin was inserted. This is a simple technique that allows direct visualization and complete resection of the cyst wall with two small incisions. Such a technique may be superior to conventional open procedures or those under image guidance.

Utility of “loco-check,” self-checklist for “locomotive syndrome” as a tool for estimating the physical dysfunction of elderly people

Aim: A new concept of locomotive syndrome has been proposed by the Japanese Orthopaedic Association. The aim of this study is to clarify the utility of its self-checklist, “loco-check,” as a tool for estimating the physical dysfunction of elderly people. Methods: Subjects were 1124 community-dwelling Japanese people, 557 men and 567 women, aged 40-89 years. Information about the seven “loco-check” items was obtained from present inquiry sheets. Physical functions were examined by grip strength, knee extension strength, walking speed and one-leg standing time with open eyes. The averages of these test values, controlled for age and BMI, were compared between the “loco-check” (+) group and the “loco-check” (-) group. Also we examined about the trend of decline of physical function, together with SF36 physical function subscale score, as the number of the items chosen increased. Results: Adjusted average values of all four physical function examinations in the “lococheck” (+) group were significantly lower than those of the “loco-check” (-) group (all, p . Also the adjusted average values of the majority of four tests were significantly lower in those who checked each of the “loco-check” items than those who did not, for most of the items. It was also revealed that the more items subjects checked, the lower the adjusted average values were, except for one-leg standing time. It was also the case with SF36 physical function subscale score. Conclusion: We showed the utility of “loco-check” as a simple tool not only for noticing the physical dysfunction of elderly people, but also for estimating the extent of it, except for balancing ability, particularly by counting the number of checked items.

Perception of Reasons of Decision Making for Home Hospice, Benefits and Difficulties in Home Hospice Care by Family Caregivers

Objective: Home hospice is one of the most important places to spend time at the end of life. It is necessary to clarify the perception of home hospice for family caregivers in order to choose home hospice. The aim of the study was to examine the perceptions of family caregivers about the reasons of decision making for home hospice care, benefits, difficulties with home hospice, and copings. Methods: Ten family caregivers who were taking care of a patient with a terminal illness at home at the time of the study participated voluntary. They participated in two interviews in the narrative approach. In the first session, they narrated reasons of decision making for home hospice, benefits, difficulties, and copings. In the second session, they narrated importance or growth in self. We analyzed narratives in the first session using qualitative analysis. This study was approved by the Ethical Board at St. Mary’s College. Results: Categories of the reasons for the decision making to undergo home hospice were “Being natural to take care of a loved one at home” or “Home hospice matching a family caregiver’s life”. Categories of benefits at home hospice were “To strengthen family’s ties” or “Both a loved one and a family spending time together”. Categories of difficulties were “Difficulty of economic problems” or “Difficulties of mental and physical symptoms”. Conclusion: We concluded that family caregivers perceived benefits and difficulties, and they could cope with difficulties, though spiritual pain remained. We need continuous support for family caregivers at home hospice.

Acceptance Process Model of Patients’ Life with Terminally Ill in Home Hospice and Development of a Narrative Approach for Nurses

The prime aim of this study was to find a psychological process model of patients with terminally ill in home hospice investigating the contents of a narrative, and the secondary aim was to create a narrative approach program for nurses. Ten patients narrated their thinking or feelings along with some prepared questions in two sessions. Patients’ narrative data were categorized by a qualitative analysis and 34 categories were chosen. They perceived good points in home hospice like “Being able to spend time freely”, “Close relationships with care staffs and strong confidence”, though they perceived trouble points like “Suffering from putting burden on the people around me” or “Worries about economic problems”. They perceived psychological changes through illness like “Having peace of mind and becoming kind” “Desire for a natural death”. Moreover they perceived their life like “Acceptance of one’s life including illness” “My life lived with satisfaction”. They regarded as important things such as “Spending life time usefully” “Feelings of my family members and of those around me”, and as hopes “Hope to leave my living proof” “Living left time to the fullest”. From these categories, we propose an acceptance model of patients’ life and a narrative program for nurses.

Air Trapping: A Cause of Heterogeneous Attenuation

Purpose: To investigate important factors affecting the frequency of air trapping through observation of normal lung attenuation, and propose a cause of heterogeneous attenuation. Materials and Methods: In this ethical committee-approved study, a total of 109 patients (30 males, 79 females;mean age, 58.2 years;range, 27 - 81 years) were included. All patients had undergone inspiratory and expiratory chest thin-section computed tomography (CT) examinations and pulmonary function tests. Air trapping on CT images was graded subjectively. Hounsfield units (HU) lung attenuation value and lung volume were measured on CT images. All variables (age, sex, indices of pulmonary function test results, air trapping score, HU value, and rate of change in lung volume) were compared by diagnoses and air trap- ping findings cohorts. The correlation between lung function test results and expiratory HU attenuation were analyzed. Results: Interstitial pneumonia showed higher and bronchiolitis obliterans showed lower HU attenuation at normal and air trapping regions. The variables affecting air trapping findings were age, a ratio of forced expiratory volume in 1 second to forced vital capacity (FEV1.0/FVC), maximal expiratory flow at 50% of forced vital capacity (MEF50), HU attenuation at normal regions, and rate of change in lung volume. Compared with expiratory HU attenuation, significant positive correlation was shown to FEV1.0/FVC and negative correlation to single-breath diffusion capacity for carbon monoxide, which was confirmed by a simple regression analysis. Conclusion: It can be suggested that lung attenuation can increase when fibrosis is advanced, and this is exaggerated when lungs are compressed.

Simplified Intraoperative Goal-Directed Therapy Using the FloTrac/Vigileo System: An Analysis of Its Usefulness and Safety

Purpose: We investigated whether the simplified intraoperative Goal-Directed Therapy (GDT) could improve the factors affecting medical costs, and contribute in standardizing intraoperative fluid management. Methods: The enrolled patients underwent esophagectomy, pancreatoduodenectomy, or aortic stent grafting in 2012, and between March 2013 and October 2014. We conducted a comparison study on the effects of GDT, between the before-GDT historical control group (n = 100) and GDT group (n = 100). The hemodynamic indices used for control group patients were conventional: Blood pressure, heart rate, and urine output. For GDT group, additionally, we used stroke volume variation (SVV) and stroke volume index (SVI). The primary outcomes were the length of intensive care unit (ICU) stay and hospital stay (LOS). Regression analysis was used to identify factors affecting LOS. The secondary outcomes were the albumin use, the fluid amount administered, and the variation in the fluid administration rate. Results: The control and GDT groups comprised 96 and 99 patients, respectively. The patient characteristics were similar. The length of ICU stay was significantly shorter (2.1 ± 2.1 days vs. 2.8 ± 1.9 days, P = 0.0009) and LOS was shorter but without statistical significance (24.5 ± 17.7 days vs. 27.7 ± 20.1 days, P = 0.21) in the GDT group than in the control group. The fluid amount administered and the presence/ absence of albumin use were factors affecting LOS. The variation of the fluid administration rate was significantly lower in the GDT group. Conclusion: The simplified GDT may contribute to the improvement of medical economics and standardize the fluid management.

Design and Sizing Electric Micro Generator Using Thermoelectric Modules

This paper presents the development of a methodology for calculating sizing electric micro sources of power generation using TEG （thermoelectric modules） to capture energy industrial process waste. Since the thermoelectric modules are able to convert a temperature gradient directly into electricity and still occupy a small space, and have no vibration or noise during operation. Furthermore, the cogeneration using thermoelectric modules is totally clean and reuses part of the residual thermal energy to generate power, or improve the overall yield of the process and avoid the emission of gases to the environment. Therefore, this research contributes to the development of a green energy to numerical modeling for the design and dimensioning of micro-sources of electric power generation from performance curves and predetermined temperature gradients industrial processes. The result is an effective methodology for the design and conditioning the voltage level and power of micro allowing the size of the electrical quickly and securely for many industrial applications, varying the types of modules used area, voltage and power generated.

Relationship between Death Conference Experiences and Nurses’ Terminal Care

Background: The experiences of death conference (DC) are regarded as improving the quality of nurses’ terminal care and developing nursing minds. Few studies in Japan clarify quantitatively how nurses’ participation in DC is related to nurses’ terminal care. Aim: This study aims to clarify the relationship between DC experiences and nurses’ terminal care using the prepared Terminal Care Nursing Self-Assessment Sheet. Method: The study is approved by the Ethics Review Board of St. Mary’s College, and the hospital where the survey was conducted. This study was an observational and cross-sectional study using an anonymous, self-completed questionnaire composed of the newly created Terminal Care Nursing Self-Assessment Sheet. Its evaluation items consist of six categories (Reflection, Cooperation, Response to patients, Response to patients’ family, Specific measures for nursing, and Feelings of the nurse), and 14 questions. The questionnaires were conducted for 144 nurses in the ER, ICU, and Palliative Care Ward (Hospice Ward) in a general hospital in Japan. For analysis, JMP ver.14.2 was used to perform a Chi-square test, Wilcoxon/Mann Whitney test, univariate, and multivariate logistic regression analysis. Results: One hundred thirty-nine participants (97%) were included in the final analysis. Forty-three nurses had DC experiences, and 96 had no experience. The median number (IQR;interquartile range) of DC experiences was 5.5 (2 - 22.5). There were significant differences in five of the above categories;however, there was no significant association in the five questions of the nurse’s stress on response to patients and patients’ family and specific measures for nursing regarding postmortem treatment for patients between DC experiences and Nurses’ terminal care. Conclusion: DC that looks back on terminal care has significance as an opportunity to consider the way of terminal concerns and direction of future care.
