Determination of gemcitabine and its metabolite in human plasma using high-pressure liquid chromatography coupled with a diode array detector

AIM: To establish a high pressure liquid chromatography (HPLC) method for determination of the concentration of gemcitabine (dFdC) and its metabolite(dFdU) in human plasma. METHODS: Plasma 1.0 mL spikedwith floxuridine as an internal standard was extracted with 3.0 mL of methanol-acetonitrile (v/v, 1:9). The supernatant was evapo- rated at 60 oC and the residue was reconstituted with 0.5 mL of the solution used as the mobile phase. After centrifugation, 50 μL of the supernatant was injected into the HPLC system. Separation was achieved on a C18 (4.6 mm×250 mm, 5 μm) column at 25 oC with the flow rate of the mobile phase set to 0.8 mL/min. The compounds were detected at 268 nm. The mobile phase consisted of 40.0 mmol/L acetate ammonium buffer solution (pH 5.5) and acetonitrile (v/v, 97.5:2.5). RESULTS: The linear range was 0.20-10.0 mg/L(r=0.9999) for dFdC and 0.50-50.0 mg/L(r=0.9999) for dFdU. The limit of detection (LOD) was 0.10 mg/L for dFdC and 0.25 mg/L for dFdU, while the limit of quantification (LOQ) was 0.20 mg/L (RSD3 %) for dFdU. The average recovery of dFdC and dFdU by this method were 103.3 % and 98.7 %, respectively. For intra-day and inter-day, the corresponding standard deviations of the measurements of dFdC and dFdU were both less than 5.5 %. CONCLUSION: An analytical method was established to measure the concentrations of dFdC and dFdU in human plasma and was effectively applied to the dFdC and dFdU pharmacokinetic studies of 8 Chinese patients with malignant tumors.

What we know about ST13, a co-factor of heat shock protein, or a tumor suppressor?

This article is to summarize the molecular and functional analysis of the gene “suppression of tumorigenicity 13” (ST13). ST13 is in fact the gene encoding Hsp70 interacting protein (Hip), a co-factor (co-chaperone) of the 70-kDa heat shock proteins (Hsc/Hsp70). By collaborating with other positive co-factors such as Hsp40 and the Hsp70-Hsp90 organizing protein (Hop), or competing with negative co-factors such as Bcl2-associated athanogen 1 (Bag1), Hip facilitates may facilitate the chaperone function of Hsc/Hsp70 in protein folding and repair, and in controlling the activity of regulatory proteins such as steroid receptors and regulators of proliferation or apoptosis. Although the nomenclature of ST13 implies a role in the suppression of tumorigenicity (ST), to date available experimental data are not sufficient to support its role in cancer development, except for the possible down-regulation of ST13 in gastric and colorectal cancers. Further investigation of this gene at the physiological level would benefit our understanding of diseases such as endocrinological disorders, cancer, and neurodegeneration commonly associated with protein misfolding.

Bruton’s tyrosine kinase inhibitors(BTKis)have revolutionized the treatment of B-cell ***,safety issues related to the use of BTKis may hinder treatment continuity and further affect clinical efficacy.A comprehensive and systematic expert consensus from a pharmacological perspective is lacking for safety issues associated with BTKi treatment.A multidisciplinary consensus working group was established,comprising 35 members from the fields of hematology,cardiovascular disease,cardio-oncology,clinical pharmacy,and evidencebased *** evidence-based expert consensus was formulated using an evidence-based approach and the Delphi *** Joanna Briggs Institute Critical Appraisal(JBI)tool and Grading of Recommendations Assessment,Development,and Evaluation(GRADE)approach were used to rate the quality of evidence and grade the strength of recommendations,*** consensus provides practical recommendations for BTKis medication based on nine aspects within three domains,including the management of common adverse drug events such as bleeding,cardiovascular events,and hematological toxicity,as well as the management of drug-drug interactions and guidance for special *** multidisciplinary expert consensus could contribute to promoting a multi-dimensional,comprehensive and standardized management of BTKis.

Induced pluripotent stem cell-related genes influence biological behavior and 5-fluorouracil sensitivity of colorectal cancer cells

Clinical significance of surgical resection for hepatocellular carcinoma with portal vein invasion:a nationwide cohort study

Background:Hepatocellular carcinoma(HCC)with portal vein invasion(PVI)is considered an advanced stage with a poor *** current guidelines recommend systemic treatment for HCC with PVI,surgical resection could produce acceptable outcomes in selected *** study aimed to identify the clinical significance of surgical resection for HCC with PVI patients using a large-scale nationwide ***:This retrospective,multicenter,observational cohort analyzed data from the Korean Primary Liver cancer Registry.A total of 16,781 patients who were newly diagnosed with HCC between 2008 and 2018 were enrolled in this *** with worse Child-Turcotte-Pugh scores(≥7)or performance status(≥2)were *** them,998 patients who received treatment for HCC with PVI were included in the analysis and were divided into two groups:resection group of 151(15.1%)and palliative group of 847(84.9%)who received transarterial and systemic therapy according to the treatment *** matching the number and size of the tumors and model for end-stage liver disease(MELD)score between the groups,the final study cohort for analysis comprised 151(26.6%)patients in the resection group and 417(73.4%)in the palliative *** primary endpoints were overall survival(OS)and cancer-specific survival(CSS).Results:The number and maximum size of HCC did not differ between the resection and palliative groups after matching[1(range,1-5)vs.1(range,1-6),P=0.11 and 5.5(range,1.2-20.6)vs.6.0(range,1.0-20.5)cm,P=0.24,respectively].Tumor markers,including alpha-fetoprotein(AFP)and protein induced by vitamin K absence or antagonist-II(PIVKA-II),also did not differ between the groups(P=0.29 and P=0.36,respectively).The 5-year OS and CSS rates of the resection and palliative groups were 44.8%and 17.4%(P<0.001)and 47.7%and 18.6%(P<0.001),*** analysis showed that palliative treatment intent was the most significant risk factor for OS and CSS[odds ratio(OR

Analysis for phenotype of HNPCC in china

AIM: The aims of this study were to identify theclinicopathological features of Chinese HNPCC families andto evaluate the value of criteria for suspected HNPCC(sHNPCC) in clinical ***: According to the follow-up records, 54 HNPCCfamilies (including 12 TCG-HNPCC families and 42 sHNPCCfamilies) were screened out from patients with colorectal cancers(CRCs), operated upon in 2nd Affiliated hospital of zhejiangUniversity from 1984 to 2001. Clinical data of probands andtumor spectrum in these families were listed and analyzed,RESULTS: (1) Mean age, proportion of colonic cancer, poorlydifferentiated cancer, multiple CRCs and Dukes′ A+B of theprobands in ICG-HNPCC and sHNPCC kindred were 39ys and47.5ys, 75 % and 62 %, 0 and 12.8 %, 16.7 % and 14.3 %,58.3 % and 81%,respectively. Compared with sporadiccolorectal cancers, probands from ICG-HNPCC and sHNPCCfamilies were obviously different at age of onset(P＝0.025and 0.031), tumor location (P＝0.001 and 0.000),differentiation(P＝0.002 and 0.011) and development ofmultiple tumors (P＝0.014 and 0.002). (2) A total of 178malignant neoplasms were found in 54 HNPCC families,including 139 colorectal cancers. Besides of colorectal cancer,extracolonic tumors occurred in stomach, endometrium,hepatobiliary system, and so on (8 gastric cancers, 6 endometrialcancers, 6 hepatobiliary system cancers and 19 others) canalso be seen in Chinese ICG-HNPCC and sHNPCC ***: (1) Chinese HNPCC families have specificclinicopathological features, such as early onset, predilectionfor the involvement of colon, tendency of multiple CRCs,development of extracolonic tumors and well differentiation.(2) The criteria for suspected HNPCC is useful in clinicaldiagnosis and management of HNPCC.

乳腺癌是全球女性最常见的恶性肿瘤,2020年GLOBOCAN流行病学数据显示,乳腺癌首次超过肺癌成为全球第一高发的恶性肿瘤,严重威胁女性健康。相较于欧美人群,中国女性患者的乳腺癌发病率相对较低,但是随着近年来生活方式、饮食习惯的改变等,中国人群乳腺癌发病逐年增加。中国2020年年龄标化的乳腺癌发病率达到39.1/10万,新发乳腺癌达41.6万例,占全球每年新发乳腺癌总人数的18.4%。乳腺癌防治工作也日益严峻,其中,三阴性乳腺癌作为一种恶性程度高、预后较差的乳腺癌亚型,由于以往缺乏明确的治疗靶点,近年来治疗进展缓慢。尤其是晚期三阴性乳腺癌的治疗,由于治疗手段相对有限,有待进一步优化治疗建议。因此,在中国抗癌协会乳腺癌专业委员会、中国抗癌协会国际医疗交流分会和中国医师协会肿瘤医师分会乳腺癌学组的倡导下,中国乳腺癌各学科诊疗专家组结合国内外最新研究证据、治疗理念和经验,制定了中国晚期三阴性乳腺癌临床诊疗指南(2024版),旨在优化中国晚期三阴性乳腺癌精准治疗决策。
