BACKGROUND: The mortality rate of heavy type hepatitisis high. No special treatment is available except generaltreatment. This multicenter clinical study was designed toobserve the safety and efficacy of promoting hepatic growthfactor (PHGF) in the treatment of heavy type hepatitis andsevere chronic ***: 347 patients with heavy type hepatitis and 324with severe chronic hepatitis were subjected to administra-tion of 120 μg of PHGF per day for 4 weeks on the basis ofgeneral treatment. Those who were being effectively treat-ed would last additional 2 to 4 weeks. Blood routine, urineroutine, blood urea nitrogen (BUN), blood creatinine(Cr), blood ammonia, alpha fetoprotein (AFP), electro-lyte, alanine transaminase (ALT), aspartate transaminase(AST), serum total bilirubin (TBIL), serum direct biliru-bin (DBIL), prothrombin time activity (PTA), total pro-tein (TP) and albumin (ALB) were detected in the pa-tients before treatment, 2 weeks after treatment, and at theend of the treatment. Any side-effect would be ***: In the patients with severe chronic hepatitis, thetotal effective rate of the treatment was 88. 9% The levelsof ALT, AST and TBIL decreased significantly (P<0.001),whereas those of PTA and ALB increased significantly (P <0.001), and the level of AFP increased slightly. In patientswith heavy type hepatitis, the total effective rate of thistreatment was 78.4%, and patients at different stage showeddifferent results. The total effective rates of patients withearly, medium and terminal stage heavy type hepatitis were89.9%, 84.8% and 27.5%, respectively. No severe side-effect was ***: PHGF is effective and safe in the treat-ment of patients with heavy type hepatitis and severe chro-nic hepatitis. But it should be administered early in patientswith heavy type hepatitis so as to get better curative effects.

Melatonin abates liver ischemia/reperfusion injury by improving the balance between nitric oxide and endothelin

BACKGROUND: Melatonin exerts complex physiological and pharmacological effects on multiple systems and organs. We hypothesized that melatonin might abate ischemia/ reperfusion (I/R) injury in the liver by inhibiting excessive oxidative stress and keeping nitric oxide (NO) from being scavenged by free radicals. The aim of the present study was to investigate whether melatonin protects the liver from I/R injury and, if so, by what underlying mechanism. METHODS: Under anesthesia, Wistar rats were intraperi- toneally injected with 20 mg/kg melatonin (dissolved in physiological saline containing 4% ethanol, Mel group), 4% alcohol (Alc group), or physiological saline (NS group). The artery, portal vein and bile duct of the left lobe of the liver were clamped for 60 minutes and then released. At different time points after I/R, the rats were sacrificed and blood samples were collected to measure the levels of serum alanine aminotransferase (ALT), lactic dehydrogenase (LDH), and NO. Hepatic tissue samples were collected for measuring endothelin expression by immunohistochemical staining and for routine morphological and histological examination. RESULTS: The levels of both ALT and LDH in the Mel group were significantly reduced for up to 24 hours after I/R compared with the Alc and NS groups (P<0.05). The levels of NO in the Mel group were significantly elevated for up to 12 hours after I/R relative to the NS group (P<0.05). The NO levels were also elevated at 0.5 and 6 hours after I/R in the Alc group (P<0.05). The immunohistochemical staining of hepatic tissue showedthat endothelin-positive cells were significantly fewer in the Mel group than in the Alc and NS groups at 6 hours after I/R (P<0.01). The necrosis of hepatocytes and the destruction of hepatic cords in the Alc and NS groups were greatly improved in Mel-treated rats, which is in concert with our functional data. Abnormal immunity and gene mutation in patients with severe hepatitis-B

AIM: To evaluate the abnormal immunity and gene mutation at precore 1896 site in patients with severe ***: This study included 23 patients with severe hepatitis-B, 22 patients with acute hepatitis-B and 20 *** at precore 1896 site of HBV gene was confirmed with restriction fragment length polymorphism (RFLP) *** including TNF-α, IFN-y, IL-6, and IL-8 were measured with ELISA, and T subgroups were detected with alkaline phosphatase anti alkaline phosphatase (APAAP) ***: In patients with severe hepatitis-B, the infective rate of HBV mutant strain was 52.5% (12/23), and only one patient with acute hepatitis-B was infected with the mutant strain. The percentage of CD8+ T lymphocyte was obviously lower (0.16±0.02%) and the ratio of CD4+/CD8+ was obviously higher (2.35±0.89) in mutant group than in wildtype group (0.28±0.05% and 1.31±0.18%, respectively,P＜0.01 or P＜0.05). The levels of cytokines in patients with severe hepatitis-B were higher (TNF-α 359.0±17.2 ng/L, IFNγ 234.7±16.5 ng/L, IL-6 347.5±31.3 ng/L, IL-8 181.1±19.6ng/L) than those in acute hepatitis-B (TNF-α 220.6±8.9ng/L, IFN-γ 174.9±12.0 ng/L, IL-6 285.8±16.5 ng/L, IL-8118.4±5.1 ng/L, P＜0.01 or 0.05). In patients with severe hepatitis-B, the levels of IFN-γ and IL-6 were higher in mutant group (273.4±26.6 ng/L, 387.7±32.5 ng/L) than in wild-type group (207.8±12.8 ng/L, 300.9±16.3 ng/L). The mortality of patients infected with HBV mutant strain was higher (100%)than that with wild-type (0.9%).CONCLUSION: In severe hepatitis-B, the infective rate of HBV mutant strain was high. The mutant strain induces more severe immune disorders in host, resulting in the activation of lymphocyte and release of cytokines. HBV DNA mutates easily in response to the altered immunity. Ultimately liver damage is more prominent.

Effect of morphine and M-cholinoceptor blocking drugs on human sphincter of Oddi during choledochofiberscopy manometry

OBJECTIVE: To evaluate the effects of morphine on the human sphincter of Oddi pressure and the antagonism of anticholinergic agents against morphine. METHODS: The action of these drugs on the sphincter of Oddi (SO) was evaluated by means of choledochofiberscopy manometry in 40 operated patients with T-tube. The patients were divided randomly into 4 groups: anisodamine, atropine, buscopan, and control. The following data were recorded: duodenal pressure (DP), basal pressure of the sphincter of Oddi (BPSO), contractive amplitude of the sphincter of Oddi (CASO), contractive frequency of the sphincter of Oddi (CFSO), contractive duration of the sphincter of Oddi (CDSO), and pressure of the common bile duct (PCBD). Both morphine and anticholinergic agents were given intramuscularly. RESULTS: After injection of 10 mg morphine, BPSO, CASO, CFSO, and PCBD increased significantly. After injection of 15 mg anisodamine or 0.75 mg atropine, CASO, BPSO declined obviously, and after injection of 20 mg buscopan, CASO, BPSO, CFSO declined obviously, but in anisodamine, atropine and buscopan groups, they differed insignificantly. CONCLUSIONS: The results illustrate that SO manometry via choledochofiberscopy is a new method for SO dynamic study. Morphine can increase DP, BPSO, CASO, PCBD, but anisodamine atropine and buscopan can antagonize the effect of morphine.

BACKGROUND: Primary liver cancer (PLC) is one of the most frequently seen tumors in china. Thirty years ago, patients with PLC were often detected at relatively late stage, with a palpable mass or marked clinical symptoms and poor prognosis. In the past 30 years, the diagnosis and treatment of PLC have been greatly improved with better prognosis. METHODS: In order to study the changes of PLC during the 30 years, the clinical data of 3250 patients with PLC from 10 medical institutions of china were collected, ana- lyzed , and compared with those of 3254 PLC patients be- fore the 30 years. RESULTS: In the 3250 patients aged 1-80 years, with an average age of 49.1 years, the male to female ratio (2.3:1) was lower than that before the 30 years. 73.5% of the 3250 patients sought medical advice within 3 months after the onset of the disease in contrast to 63.8% before the 30 years. Compared with those patients before the 30 years the symptoms and signs were alleviated generally. The HBsAg positive rate was 81.0%, but the HCV-Ab positive rate was 13.2%. The AFP level in 75% of patients was elevated, but in the remaining 25% was normal. 1912 patients (58.8%) were confirmed pathologically. Among them 1755 patients (91.8%) had hepatocellular carcinoma. The overall resec- tion rate was 46.3%. Those who had early, middle, late stage carcinoma accounted for 29.9%, 51.5%, and 18.6%respectively in contrast to 0.4%, 47.0%, and 52.6% repor- ted before the 30 years. The 1-, 3-, 5-year survival rates of the patients were 66.1%, 39.7%, and 32.5% respectively, whereas 93.5%, 70.1%, and 59.1% for the early stage pa- tients, and 65.3%, 30.5%, and 23.5% for the middle stage patients. The half and 1-year survival rates of the late stage patients were 52.5%, and 14.7%, respectively. CONCLUSION: Comparison with the clinical data before and after the 30 years show that PLC can be diagnosed ear- ly. 目的观察冠脉搭桥术麻醉前应用戊乙奎醚对麻醉诱导前后患者血流动力学和脑电双频指数(BIS)的影响。方法择期全麻下行冠脉搭桥术患者40例,随机分为2组,每组20例。戊乙奎醚组麻醉诱导前肌内注射(肌注)戊乙奎醚0.01 ***-1,东莨菪碱组肌注东莨菪碱0.006 ***-1。观察比较2组麻醉诱导前后心率、血压、心率收缩压乘积(RPP)和BIS值。结果东莨菪碱组用药后收缩压和RPP无明显变化,用药后20 min患者心率有所上升,用药后10、20 min BIS值有所下降,但高于戊乙奎醚组(P<0.05)。戊乙奎醚组用药后10、20 min,患者心率、血压、RPP和BIS值均降低(P<0.05)。气管插管前后戊乙奎醚组心率、收缩压、RPP和BIS值多低于东莨菪碱组。2组均未见明显不良反应。结论应用戊乙奎醚作为冠脉搭桥术前用药,可降低心肌耗氧量,有利于患者血流动力学平稳和镇静。

Gadolinium chloride and salvia miltiorrhiza compound ameliorate reperfusion injury in hepatocellular mitochondria

AIM: To investigate the effect of gadolinium chloride (GaCl3)and salvia miltiorrhiza compound (SMCo) on ischemia and reperfusion (I/R) injury in hepatocellular ***: Wistar rats were randomly to divided into control group, GaCl3 group, SMCo group and GaCl3 + SMCo group (n=15 each). GaCl3 (7 rmg@kg-1) was injected into tail vein on d 1 and d 2 in contrast group. SMCo ( 2 ml@kg-1) was injected into muscle on d 1 and d 2 in SMCo group. GaCl3+SMCo group received both GaCl3 (iv) and SMCo (im) injection. Control group received saline injection only. On d 3, all the rats were subjected to 2 h ischemia in the middle and left lobes of the liver, followed by reperfusion for 2 h, 6 h and 18 h respectively. The level of serum alanine aminotransferase (ALT) and malondialdehyde (MDA) in hepatocellular mitochondria was measured. Pathological changes in hepatic tissue and in hepatocellular mitochondria were determined with optical microscope and electronic microscope, ***: Remarkablly pathohistological and biochemical changes were detected after 6 h of I/R. Compared with control, the level of ALT was decreased in GaCl3, SMCo and GaCl3+SMCo treated groups (1 314.0±278.7 vs809.4±196.1,716.6±242.8 and 837.2±190.6 IU@L-1, respectively. P＜0.05).Similarly, the level of MDA was decreased in GaCl3, SMCo and GaCl3+SMCo treated groups (293.1±±51.1 vs 190.8±55.5,214.3±32.9 and 221.0±47.3 nmol@g-1, respectively, P＜0.05).Accordingly, in control group, swelling, degeneration, focal necrosis, infilt-ation of leucocyte were found in reperfused tissue under an optical microscope, and mitochondda swelling, rupture and even breakdown were seen under an electronic *** pathohistological and ultrastructural damages caused by I/R were greatly attenuated in GaCl3, SMCo and GaCl3+SMCo treated groups.
