BACKGROUND: Transplanted mononuclear cell (MNC) of umbilical blood can survive in central nervous system (CNS) of host through blood brain barrier, differentiate into nerve cells, migrate to damaged site and integrate morphological structure and function with nerve cells of host so as to improve deficiencies of sensatory function, motor function and cognitive function and influence on stroke sequela. OBJECTIVE: To observe the vein transplantation of human umbilical cord blood stem cells (HUCBSC) for improving neurological function, limb function and activity of daily living of patients with stroke and evaluate the reliability. DESIGN: Self-controlled study. SETTING: department of neurosurgery, the Second People's hospital of zhengzhou City; Red-crossed Blood Center of henan province; department of neurosurgery, the Fist affiliated hospital of zhengzhou university. PARTICIPANTS: A total of 10 patients with stroke sequela were selected from department of Cerebral Surgery, the Second People's hospital of zhengzhou City from April to December 2005. There were 9 males and 1 female aged from 35 to 75 years with the mean age of 56 years. All of them were diagnosed with CT and MRI examination and coincidence with diagnostic criteria of stroke established by the Fourth National Academic Meeting for Cerebrovascular Disease. All patients provided informed consent. METHODS: 80-140 mL umbilical blood of term birth of newborn was selected hermetically and maintained in sterile plastic bag. And then, the blood was centrifugated at the speed of 1 500 r/min for 30 minutes at 22 ℃ in order to separate MNC, i.e., HUCBSC. In addition, after final diagnosis during hospitalization, stroke patients were perfused with HUCBSC through superficial vein of back of the hand. Each patient was averagely perfused with 6 portions of HUCBSC (cellular numbers ≥ 1×108/portion) and the interval between each portion was 1-7 days with the mean interval of 4 days. AIM:To investigate the relationship between human papillomavirus (HPV) infection and concurrent esophagus and gastric cardia cancer from the same patient (CC) and examine the significance of P16 INK4A protein ***:Polymerase chain reaction was used to detect the presence of HPV type16 (HPV16).The expression of P16 INK4A protein was detected using ***:Among the CC specimens,HPV16-DNA was found in eight cases of esophageal squamous cell carcinoma (ESCC) and five cases of gastric cardia adenocarcinoma (GCA),respectively (47% vs 29%),and two of both ESCC and GCA.P16 INK4A was highly expressed in both ESCC and *** the HPV-associated positive CC,higher P16 INK4A expression was observed in the GCA than in the ESCC (75% vs 25%,P < 0.05).CONCLUSION:HPV16 as a correlated risk factor may play an important role in the development of ESCC and GCA.P16 INK4A may be a screening index in the HPVassociated carcinoma of gastric cardia.

Management of Otitis Media with Effusion

Otitis media with effusion is one of the most common diseases in children. Its treatment remains controversial. Clinical practice guidelines of OME allow watchful waiting for 3 months before treatment if the child with OME is not at risk for speech/language/or learning problems. Tympanostomy tube insertion is the preferred initial procedure when a child becomes a surgical candidate. Complementary or alternative medicine is not recommended as a treatment for OME. This paper provides a systematic review of management of OME, which we hope will be helpful for clinicians.

In order to investigate the levels of stem cell factor (SCF) and its receptor c-kit protein and mRNA in pediatric aplastic anemia (AA) and their relevance to the pathogenesis, immunocytochemical and in situ hybridization were utilized to detect the expression of SCF and its receptor c-kit gene protein and mRNA, respectively in 59 children with AA and 51 normal controls. The relationship between SCF and c-kit and the pathogenesis of AA was analyzed subsequently. The results showed that the positive rate of SCF protein and mRNA expression in children with AA was significantly lower than that in healthy controls (P0.05). It was concluded that the expression of SCF is significantly decreased in children with AA, which may be closely associated with the pathogenesis of the AA. c-kit may be unrelated to the development of pediatric AA. Therefore, AA in children may have abnormalities at SCF/c-kit signal transduction levels.

In vitro Differentiation of Adipose-Derived Stem Cells into Hair Cell-Like Cells in Guinea Pigs

Objective Adipose-derived stem cells(ADSCs) are suggested to possess a highly plastic ability to differentiate into several specific cell types in addition to adipocyte lineages, including germ layer tissue-specific cell lineages such as chondrocyte, myocyte, neuronal, and osteoblast lineages. The aim of this study is to establish an in vitro culture technique for ADSCs in an adult guinea pig model that facilitate their differentiation into hair cell-like cells. Materials and Methods Cells from inguinal fat pads in adult guinea pigs were cultured with β-mercaptoethanol, RA, Forskorin, Heregulin, bFGF, BDNF and EGF. Cellular differentiation was examined using immunocytochemistry techniques. Results The ADSCs demonstrated hair cell immunophenotypes with expression of epitopes of the hair cell marker protein myosin Ⅶa. Conclusion ADSCs from adult guinea pig adipose tissue can differentiate into hair cell-like cells when cultured in vitro. ADSCs may serve as seed cells for tissue engineering.

Correlation of matrix metalloproteinase suppressor genes RECK,VEGF,and CD105 with angiogenesis and biological behavior in esophageal squamous cell carcinoma

AIM: To explore the expression of reversion inducing cysteine-rich protein with Kazal motifs (RECK), vascular endothelial growth factor (VEGF) and endoglin (CD105) protein and its correlation with occurrence, development, invasion and metastasis in esophageal squamous cell carcinoma (ESCC). METHODS: Streptavidin-peroxidase (SP) immunohisto- chemistry was used to detect expression of RECK and VEGF in 62 cases of ESCC, 31 cases of adjacent atypical hyperplastic epithelium and 62 cases of normal esophageal epithelium. CD105 Mb was used to assess microvessel density (MVD). RESULTS: The expression of RECK was closely correlated with histological grade, infiltrative depth and lymphatic metastasis in ESCC (P 3/62), adjacent atypical hyperplastic epithelium (71.0%, 22/31), and carcinoma (59.7%, 37/62). There was a significant difference among the groups (P 31), and carcinoma (67.7%, 42/62). There was a significant difference among the groups (P 36.92 ± 10.85; grade Ⅱ, 37.65 ± 9.50; and grade Ⅲ, 38.06 ± 12.19). The MVDCD105 was closely correlated with infiltration and lymphatic metastasis in ESCC (P < 0.05). The expression of RECK was inversely correlated with the expression of VEGF and CD105. CONCLUSION: RECK, VEGF and CD105 play important roles in the infiltration, metastasis and carcinogenesis in esophageal carcinoma. Angiogenesis in ESCC may be promoted by over-expression of CD105.

Different ischemic preconditioning for rat liver graft: protection and mechanism

OBJECTIVE: To investigate the protective mechanism of different ischemic preconditioning (IPC) to ischemia/reperfusion (I/R) injury of rat liver graft. METHODS: 192 Wistar rats were randomly divided into 4 groups (48 rats in each group): control group (group C), experimental group 1 (group E_1), experimental group 2 (group E_2), and experimental group 3 (group E_3). IPC was not carried out in group C. In the experimental groups, IPC was carried out by blocking blood flow of the portal vein and hepatic artery and then reperfusion by removal of the clamp before donor liver was resected. Group E_1: 5-minute ischemia and 10-minute reperfusion; Group E_2: 5-minute ischemia and 5-minute reperfusion and one more the same procedure; Group E_3: 10-minute ischemia and 15-minute reperfusion. Four hours after IPC, liver transplantations were performed. Recipient blood and graft samples were obtained to determine the levels of ALT, AST, TNF-α and apoptosis index at 0.5, 2, 6, 24 hours after portal vein reperfusion. RESULTS: At 0.5, 2 hours after portal vein reperfusion, the levels of TNF-α in the experimental groups E_1, E_2, and E_3 were significantly lower than in the control group (P3 (P3 (P3 was significantly less than in the control group (P3(P3 was significantly less than in the control group (P<0.05). CONCLUSIONS: Ischemic preconditioning could attenuate liver graft injury by decreasing apoptosis of hepatocytes and production of TNF-α. Treatment of membranous Budd-Chiari syndrome: analysis of 480 cases

BACKGROUND: Budd-Chiari syndrome (BCS) presents a kind of disease resulted from the occlusion of hepatic vein and/or the intrahepatic inferior vena cava. Its different pathological types were proposed. According to our expe- rience , the membranous type takes a large part of it, and we tried to explore the best treatment of membranous BCS through the analysis of 480 cases retrospectively. METHOD: The operative results of 480 patients with mem- branous BCS were analysed retrospectively. RESULTS: Patients after Kimura's finger rupture, inter- ventional treatment and membrane resection were followed up with rates of 84.62%, 86.55%, and 87.37%, respective- ly. The effective rates of the three methods were 61.4%, 91.7%, and 90.4%, respectively, and the recurrence rates of the disease after the 3 procedures were 38.6%, 8.3% and 9.6%, respectively. The long-term effects of interventional treatment and resection were significantly better than those of Kimura' s finger rupture (P <0.05). CONCLUSION: Balloon dilatation is the choice for mem- branous BCS. Patients with extensive lesion, thick mem- brane or recurrence after percutaneous transhepatic angio- graphy should undergo membrane resection.

Protective effects of glycine pretreatment on brain-death donor liver

BACKGROUND: Morphological and functional changes commonly occur in livers of brain-death donors. Preven- tion of liver injury from brain-death will benefit the results of transplantation. This study was conducted to evaluate the protection effects of glycine on the liver of brain-death donor. METHODS: Fourty-two male Wistar rats were randomly divided into brain-death donor (BDD) group (B), glycine pretreatment group with BDD (G), and strychnine pre- treatment group with BDD(S). For these groups, brain death model was established in donor rats and liver trans- plantation was performed subsequently utilizing microsur- gical techniques. After the establishment of the model and during cold rinsing of liver donors or liver reperfusion of recipients, glycine was given at a dose of 0. 6 mmol, 25 μmol and 25 μmol in the group G, and a same dose of gly- cine and strychnine (1000 :1) was prescribed for the group S, but nothing for the group B. Before cold rinsing at 2 and 6 hours after portal vein(PV) reperfusion, blood sam- ples were taken from infrahepatic vena cava (IHVC) to de- termine the levels of alanine aminotransferase (ALT), as- partate aminotransferase (AST), tumor necrosis factor al- pha (TNF-α) and hyaluronic acid (HA). At 6 hours after PV reperfusion, graft samples were fixed for morphological observation and apoptosis of hepatocytes was detected using the TUNEL method. RESULTS: Before liver cold rinsing and at 2 and 6 hours af- ter PV reperfusion, the serum levels of ALT, AST, TNF-α, HA and apoptosis index (AI) in the groups B and S were significantly higher than those in the group G (P 0.05). Electron microscopy showed that Kupffer cells were activated and hepatic cells injured more obvious- ly in the groups B and S than in the group G. CONCLUSION: Glycine pretreatment can improve the via-bility of the liver of the brain-death donor rat.
