Surgical treatment for hepatocellular carcinoma and secondary hypersplenism

BACKGROUND: Hepatocellular carcinoma (HCC) is a common disease with high mortality and serious effect on the life quality of patients. Operation is still the most effective treatment. Currently, in china, patients with HCC are often complicated by hepatitis B related liver cirrhosis and secondary hypersplenism. This study was undertaken to evaluate the effect and indications of synchronous hepatectomy and splenectomy for HCC patients with hypersplenism. METHODS: The clinical records and treating processes of 24 patients with HCC and hypersplenism during the period of January 1991 to July 2004 were analyzed retrospectively. RESULTS: Sixteen patients underwent hepatectomy and splenectomy, including extensive devascularizasion around the cardia (9 patients). Seven patients were treated with microwave ablation and splenectomy plus extensive esophagogastric devascularization. One patient underwent hepatectomy combined with microwave ablation and splenectomy plus extensive esophagogastric devascularization. There were no deaths during the operation. During the first week after operation, the symptoms of hypersplenism disappeared and the platelet (Plt) and white blood cell (WBC) counts were significantly elevated (Plt: 247×109/L vs. 45.9×109/L, WBC: 13.0×109/L vs.3.3×109/L,P<0.01). CONCLUSIONS: Synchronous splenectomy can increase the safety of hepatectomy in selected patients with HCC and secondary hypersplenism by reducing bleeding complications. Splenectomy enhances patients' immunity against tumor in a long period as well.

A comparative molecular field analysis of cytotoxic beta-carboline analogs

AIM: To derive a model that could be used in drug design. METHODS: Beta-carbolines are reported to have antitumor activities on cultured cancer cell lines. A comparative molecular field analysis (CoMFA) was undertaken to elucidate the correlation of cytotoxities and structural parameters of 16 beta-carboline analogs (1-16). The compound 12 was finally used as a template for the other compounds in the dataset because of its highest biological activity. RESULTS: The CoMFA applied to the final alignment resulted in a qcv of 0.656 and it showed that the 2 steric fields contributed 43.3 % of the model information while the electrostatic fields represented the other 56.7 %. CONCLUSION: Three designed compounds, which were predicted to have high, moderate and low activities respectively, were synthesized. The IC50 values of these compounds indicated the significance of the analysis in this study. The model derived from the current study could be further used in design for more active compounds.

Background and Objective: Little is known about the incidence of hepatitis B virus (HBV) infection in Hodgkin's lymphoma patients. This study was to evaluate the impact of HBV infection on the survival of Hodgkin's lymphoma patient. Methods: Clinical data of 120 Hodgkin's lymphoma patients treated at the sun yat-sen university Cancer Center between January 2004 and October 2007 were collected. The impact of prognostic factors including HBV infection on survival was examined by univariate and multivariate analyses. A log-rank test was used for univariate analysis and the Cox proportional hazards regression model was used for multivariate analysis. Results: Of the 120 patients, 18 (15.0%) were hepatitis B virus surface antigen (HBsAg)-positive. The HBsAg-positive patients had lower 5-year survival rate than did the HBsAg-negative ones (66.9% vs. 91.3%, P = 0.006). When the patients were divided into early-stage (Ⅰ+Ⅱ) and advanced-stage (Ⅲ+Ⅳ) groups, the 5-year survival rate was significantly different between the HBsAg-positive and -negative patients in early-stage group (64.8% vs. 96.0%, P < 0.001), while not significantly different in advanced-stage group (75.0% vs. 84.8%, P=0.667). Both univariate and multivariate analyses showed that radiotherapy and HBV infection were independent prognosis factors for the patients with early-stage Hodgkin's lymphoma (P=0.006 and 0.014, respectively). Conclusions: The incidence of HBV infection is similar between Hodgkin's lymphoma patients and normal population. HBV infection is an independent prognosis factor for survival in the patients with early-stage Hodgkin's lymphoma.

In this paper,we address the problem of unsuperised social network embedding,which aims to embed network nodes,including node attributes,into a latent low dimensional *** recent methods,the fusion mechanism of node attributes and network structure has been proposed for the problem and achieved impressive prediction ***,the non-linear property of node attributes and network structure is not efficiently fused in existing methods,which is potentially helpful in learning a better network *** this end,in this paper,we propose a novel model called ASM(Adaptive Specific Mapping)based on encoder-decoder *** encoder,we use the kernel mapping to capture the non-linear property of both node attributes and network *** particular,we adopt two feature mapping functions,namely an untrainable function for node attributes and a trainable function for network *** the mapping functions,we obtain the low dimensional feature vectors for node attributes and network structure,***,we design an attention layer to combine the learning of both feature vectors and adaptively learn the node *** encoder,we adopt the component of reconstruction for the training process of learning node attributes and network *** conducted a set of experiments on seven real-world social network *** experimental results verify the effectiveness and efficiency of our method in comparison with state-of-the-art baselines.

AIM: To investigate the correlation between expression of calreticulin and infiltration of lymphocytes in stage ⅢB colon cancer. METHODS: Sixty-eight pathologically-confirmed speci-mens were obtained from stage ⅢB (T3N1M0) colon cancer patients who underwent radical resection between January 1999 and May 2002 at the Cancer Center of sun yat-sen university, guangzhou, china. Immuno-histochemical analysis was performed to show infiltration of lymphocytes and expression of calreticulin in colon cancer. Association between calreticulin expression, infiltration of lymphocytes, and 5-year survival rate of patients was assessed. RESULTS: The expression level of calreticulin was lower in cancer nest than in its adjacent normal epithelium since 61.8% (42/68) of the samples were stained with calreticulin in colon cancer. The expression of calreticulin in colon cancer was associated with the infi ltration of CD45RO+ cells rather than with that of CD3+ cells. In addition, the stronger expression of calreticulin and the higher infiltration of CD3+ and CD45RO+ cells in colon cancer were associated with the higher 5-year survival rate of patients. CONCLUSION: Expression of calreticulin is associated with infiltration of T-cells, which implies that a low expression level of molecular marker may represent a new mechanism underlying immune escape in colon cancer.

Canal Wall Reconstruction Mastoidectomy

Objective To investigate the advantages of canal wall reconstruction(CWR) mastoidectomy, a single-stage technique for cholesteatoma removal and posterior external canal wall reconstruction, over the open and closed procedures in terms of cholesteatoma recurrence. Methods: Between June 2002 and December 2005, 38 patients (40 ears) with cholesteatoma were admited to sun yat-sen Memorial Hospital and received surgical treatments. Of these patients, 25 were male with ages ranging between 11 and 60 years(mean = 31.6 years) and 13 were female with ages ranging between 20 and 65 years (mean = 38.8 years). Canal wall reconstruction(CWR)mastoidectomy was performed in 31 ears and canal wall down(CWD) mastoidectomy in 9 ears. Concha cartilage was used for ear canal wall reconstruction in 22 of the 31 CWR procedures and cortical mastoid bone was used in the remaining 9 cases. Results At 0.5 to 4 years follow up, all but one patients remained free of signs of cholesteatoma recurrence, i.e., no retraction pocket or cholesteatoma matrix. One patient, a smoker, needed revision surgery due to cholesteatoma recurrence 1.5 year after the initial operation. The recurrence rate was therefore 3.2% (1/31). Cholesteatoma recurrence was monitored using postoperative CT scans whenever possible. In the case that needed a revision procedure, a retraction pocket was identified by otoendoscopy in the pars flacida area that eventually evolved into a cholesteatoma. A pocket extending to the epitympanum filled with cholesteatoma matrix was confirmed during the revision operation, A decision to perform a modified mastoidectomy was made as the patient refused to quit smoking. The mean air-bone gap in pure tone threshold was 45 dB before surgery and 25 dB after(p < 0.05). There was no difference between using concha cartilage and cortical mastoid bone for the reconstruction regarding air-bone gap improvement, CT findings and otoendoscopic results. Metallomics is an emerging scientific area integrating the research fields related to the understanding of the molecular mechanisms of metal-associated life processes and the entirety of metal and metalloid species within a cell or tissue type. In metallomics,metalloproteins,metalloenzymes and other metal-containing biomolecules in a biological system are referred to as metallomes,similar to genomes and proteomes in genomics and proteomics,respectively. This review discusses the concept of metallomics with a focus on analytical techniques and methods,particularly the so-called hyphenated techniques which combine a high-resolution separation technique (gel electrophoresis/laser ablation,chromatography or capillary electrophoresis) with a highly sensitive detection method such as elemental (inductively coupled plasma,ICP) or molecular (electron spray ionization (ESI) or matrix-assisted laser desorption/ionization (MALDI)) mass spectrometry,or nuclear X-ray fluorescence/absorption spectrometry. The applications of these advanced analytical methods in the identification of metallo-/phospho-/seleno-proteins,probing of relationships between structure and function of metal-loproteins,and study of clinically used metallodrugs will be selectively outlined,along with their advantages and limitations.

Investigation of thermoluminescence in Li_2B_4O_7 phosphors doped with Cu, Ag and Mg

Li2B4O7 （LBO）:Cu,Ag,Mg phosphors have been prepared by the sintering technique. The roles of the Ag and Mg dopants in the phosphors have been studied using the methods of thermoluminescence （TL） glow curves and TL 3D spectra. The results indicated that proper concentrations of Ag and Mg can enhance the TL of LBO:Cu. It was also indicated that the intensity of TL peak at 130℃ is reduced with the in- creasing Ag concentration, and enhanced with the increasing Mg concentration. From the TL 3D spectra, three emission bands （λ1 = 421 nm, λ2 = 380 nm, λ3 = 350 nm） were observed: the intensity of low energy emission band is reduced and that of the high energy is enhanced with the increasing dopant Ag; on the contrary, the intensity of low energy emission band is enhanced and that of the high energy one is reduced with the increasing dopant Mg.

INHIBITION OF RAT LENS ALDOSE REDUCTASE BY QUERCETAGETIN AND PATULETIN
