Combined measurement of thyroid and plasma homocysteic acid for detecting the severity of vascular dementia

BACKGROUND: Recent researches demonstrate that onset of cerebral infarction always accompanies with obvious changes of function of thyroid axis; while, high-homocysteic acidemia has been proved as one of risk factors of vascular dementia and Alzheimer disease. Meanwhile, it is found that level of plasma homocysteic acid is positive correlation with injured degrees of cognitive function and brain damage. OBJECTIVE: To observe the changes of function of thyroid and level of homocysteic acid among patients with vascular dementia and compare with those patients without dementia cerebral infarction. DEsIGN: Randomized grouping and contrast observation. sETTING: department of neurology, people’s hospital affiliated to yunyang medical college, south china hospital of Wuhan University. PARTICIPANTs: A total of 38 patients with vascular dementia were hospitalized in the department of neurology, people’s hospital affiliated to yunyang medical college from February 2004 to December 2005. All patients met the diagnostic criteria of the Fourth Edition of Diagnostic and statistical Manual of Mental Disorder (DsM-IV) established by American Psychiatric Association. Based on educational degrees, Mini-mental status Examination (MMsE) was classified into illiteracy (≤ 17 points), education of primary school (educational duration ≤ 6 years, ≤ 24 points) and education of middle school or above (educational duration > 6 years, ≤ 24 points). Forty patients with non-dementia cerebral infarction were regarded as the control group and checked with CT examination. There were no significant differences of sex and age between the two groups. All patients and relatives were provided the consent. METHODs: Within 24 hours after hospitalization, patients with vascular dementia received MMsE scores, and the degrees were classified based on the scoring results: mild (20-24 points), moderate (10-19 points) and severe (below 10 points). Influence of mild hypothermia on vascular endothelial growth factor and infarct volume in brain tissues after cerebral ischemia in rats

BACKGROUND: It has been demonstrated that mild hypothermia has obvious protective effect on both whole and local cerebral ischemia. However, the definite mechanism is still unclear for the brain protection of mild hypothermia on cerebral edema, inhibiting inflammatory reaction, stabilizing blood brain barrier, etc. OBJECTIVE: To investigate the effect of mild hypothermia on the expression of vascular endothelial growth factor and the infarct volume after cerebral ischemia in rats, and analyze the brain protective mechanism of mild hypothermia. DEsIGN: A randomized grouping and controlled animal trial. sETTING: department of neurology, people’s hospital of yunyang medical college. MATERIALs: Twenty adult male sD rats of clean degree, weighing (250±30) g, were provided by the animal experimental center, school of Medicine, Wuhan University. The kits for sP immunohistochemistry were purchased from Beijing Zhongshan Golden Bridge Biotechnology Co., Ltd. METHODs: The experiments were carried out in the laboratory of department of neurology, Renmen hospital of Wuhan University from May to July 2005. ① The 20 rats were divided randomly into normal temperature group (n =10) and mild hypothermia group (n =10). Models of permanent middle cerebral artery occlusion were established with modified nylon suture embolization. The rats were assessed with the Longa standards: 0 point for without nerve dysfunction; 1 for mild neurological deficit (fore claws could no extend completely); 2 for moderate neurological deficit (circling towards the affected side); 3 for severe neurological deficit (tilting towards the affected side); 4 for coma and unconscious; 1-3 points represented that models were successfully established. The rats of the normal temperature group were fed at room temperature, and those in the mild hypothermia group were induced by hypothermia from 2 hours postoperatively, and the rectal temperature was kept at 34-35 ℃ for 72 hours. ② Measurement of infarct volume: All t

BACKGROUND: It has been demonstrated that adenosine can induce glial cell to release cytochrome C, enhance expression of apoptotic gene bax, inhibit anti-apoptotic gene bcl-2, and activate caspase-3 to apoptosis; Whereas inosine can inhibit neuronal apoptosis which is similar to bcl-2. OBJECTIVE: To observe the effects of inosine on neuronal apoptosis and expression of cytochrome C mRNA in rats after focal cerebral ischemia/reperfusion, and analyze the pathway of its neuroprotective effect. DEsIGN: A randomised controlled animal trial. sETTINGs: department of neurology, Rongcheng second people's hospital; department of neurology, affiliated Union hospital, Tongji medical college, Huazhong University of science and Technology. MATERIALs: sixty-eight rats, weighing 230-280 g and clean grade, were used. TdT-mediated dUTP-biotin nick end labeling (TUNEL) and cytochrome C mRNA in situ hybridization kits and DAB staining kit were purchased from Wuhan Boster Biological Co., Ltd.; Inosine injection [200 mg (2 mL) each] from Qingdao First Pharmaceutical Factory. METHODs: The experiment was accomplished in the animal experimental center in Tongji medical college of Huazhong University of science and Technology from December 2003 to June 2005. ① sixty-four rats were made into focal ischemia by middle cerebral artery occlusion (MCAO) with a nylon monofilament suture. The successfully induced rats were assigned to inosine group (n =32) and model group (n =32) at random. Rats in the inosine group were intraperitoneally administrated with inosine in dose of 100 mg/kg preoperatively, twice a day, 7 days in all. The rats in the control group were injected with the same dose of saline solution by the similar way preoperatively. Each group was randomized into ischemia /reperfusion 2, 6, 12, 24 hours, 2, 3, 7 and 14 days subgroups consisted of 4 rats. BACKGROUND: Myasthenia gravis (MG) is considered as a peripheral neuromuscular disorder. some investigations demonstrate that MG affects central nervous system (CNs), and there are disorders in cognitive function, affection, sleep, mental status and others; however, there are also some different standpoints. some questions are still disputed, such as whether or not MG exists in the CNs and cognitive function is impaired. DATA sOURCEs: Using the terms of 'myasthenia gravis, neuropsychological test, memory, central, brain, cognitive dysfunction and so on', we searched the Medline database for the articles about CNs involved in MG, nerve and neuropsychology as well as cognitive function, which were published in the English language between January 1985 and December 2004. Meanwhile, we also searched CNKI database by inputting 'myasthenia gravis, neuropsychology and cognitive function' in Chinese for articles about involved neuropsychology and cognitive function which were published between October 1995 and June 2006 in the Chinese language. sTUDY sELECTION: Data were checked in the first trial, and articles about the influences of MG on CNs, neuropsychology, cognitive function and central cholinergic function were selected, then those were obviously unrelated with above criteria were excluded. Articles which expound the influences of MG on neuropsychology and cognitive were included, but those with repeated contents were also excluded. DATA EXTRACTION: A total of 89 related English articles and 34 Chinese articles were collected, including 43 studies on involved CNs and 32 studies on involved central cholinergic function and cholinergic receptor. sixty-three literatures were about neuropsychology and cognitive function, which were closely related with this article. seventy-two articles met inclusive criteria and 66 were excluded. BACKGROUND: At present, researches on differentiating from human adipose-derived adult stromal cells (hADAsC) to neuron-like cells are focus on inducing by artificial-synthetic compound solution; however, hippocampal astrocyte conditioned medium (HCAM) can induce in vitro differentiation from hADAsC to neuron-like cells is still unclear. OBJECTIVE: To observe whether HCAM can induce in vitro differentiation from hADAsC to neuron-like cells. MATERIALs: Donor of adipose tissue was donated by female volunteers suffering from caesarean section in the department of obstetrics & gynecology in our hospital and aged 20-35 years. Adipose tissue was collected from subcutaneous tissue of abdomen during the operation. In addition, 8 male newborn Wistar rats within 24 hours with average body mass of 20 g were provided by Animal Institute of Chinese Academy of medical sciences. Rabbit-anti-human Nestin polyclonal antibody, rabbit-anti-human glial fibriliary acidic protein (GFAP) polyclonal antibody, rabbit-anti-human neuro-specific enolase polyclonal antibody and mouse-anti-human microtubal associated protein 2 (MAP-2) polyclonal antibody were provided by Wuhan Boster Company. METHODs: The experiment was carried out in the Central Laboratory of North china Coal medical college from October 2004 to June 2005. hADAsC was cultured with HCAM and its growth and morphological changes were observed under inverted phase contrast microscope. BACKGROUND: Cognitive impairment after stroke associates with various factors, such as age, educational years, etc. Besides concerning about the recovery of limb function after stroke, we should also focus on the rehabilitation of cognition. Moreover, we’d better pay attention to the control of all the risk factors of stroke, and improve the quality of life in stroke patients. OBJECTIVE: To analyze the factors that affect cognitive impairment after stroke. DEsIGN: A related factors analysis. sETTINGs: department of neurology of Dalian Port hospital and Dalian second people’s hospital. PARTICIPANTs: Totally 148 stroke inpatients were selected from department of neurology, Dalian Port hospital and Dalian second people’s hospital from April 2004 to December 2005, including 100 males and 48 females, aging 45-75 years with an average age of (67±8) years; Their educational years ranged 2-10 years with an average of (6.1±3.7) years; The disease course ranged 15-30 days; All were right-handed. Inclusive criteria: All were accorded with the diagnostic standard set by the Fourth National Academic Meeting for Cerebrovascular Disease; Confirmed by CT or MRI; Informed consents were obtained from all the subjects. METHODs: After the disease conditions were stable, the patients were assessed with Wechsler memory scale (WMs) and Wisconsin card sorting test (WCsT). WMs included forward and backward recitation of numbers and short-term memory (verbal memory, visual recognition). The number of times for correct and wrong classifications in WCsT and the time to complete the trail making tests A and B were recorded. The focal volume, area and layer were recorded at 24 hours after admission. The general data of the patients were recorded, including name, sex, age, educational years, history of hypertension, history of diabetes mellitus. study on HBV Vertical Transmission via the in vitro Fertilization (IVF) Technique

Objective To study the hepatitis B virus （HBV） vertical transmission via infected spermatozoa. Methods Eighteen male patients with HBV infection who underwent in vitro fertilization （IVF） were studied, 5 HBV negative patients were selected as the control. Fluorescence in situ hybridization （FIsH） analysis using the partial-length HBV DNA as the hybridization probe was performed to explore the existence of HBV DNA in the sperm and in the host embryonic genome. Results FIsH showed that 5 of 18 patients＇ sperm presented positive signals and 2 of 18 embryos presented positive signals, while no positive signals were found in control group. Conclusion The HBV DNA was found in human sperm and embryos of HBV patients. These results provide direct evidence that HBV DNA could transmit to foetus via human infected spermatozoa.

Effects of topiramate and carbamazepine on thyroid hormone level in adults with epilepsy

BACKGROUND: It has been demonstrated that traditional antiepileptics, such as phenytoin, carbamazepine (CBZ), phenobarbital, etc., can result in the decrease of thyroid hormone of epileptic patients. However, there is still no sufficient evidence for the studies about the effect of new-type antiepileptics, such as topiramate (TPM), on thyroid hormones. OBJECTIVE: To observe the effects of TPM and CBZ on the level of thyroid hormones in serum of adults with epilepsy. DEsIGN: A comparative observation. sETTING: department of neurology, sichuan Provincial people's hospital. PARTICIPANTs: Totally 100 outpatients or inpatients newly diagnosed to have epilepsy were selected from the department of neurology, sichuan Provincial people's hospital from July 2003 to August 2005, including 60 males and 40 females, aged 18-70 years. All the patients were accorded with the standard for the classification of epilepsy set by International League Against Epilepsy (ILAE) in 1981; Had been Informed and agreed with the detection; Had no history of thyroid gland disease; Had not taken any drugs could affect the thyroid function. Meanwhile, 40 adult healthy examinees were selected from our hospital as the control group, including 24 males and 16 females, aged 18-65 years. METHODs: ① The 100 epileptic patients were randomly divided into TPM group (n =50) and CBZ group (n =50), and they were treated with TPM (Xian-Janssen Pharmaceutical, Ltd.; Batch number: 03As032, Norm: 25 mg/tablet) and CBZ (shanghai sunve Pharmaceutical Co., Ltd.; Batch number: 030201, Norm: 100 mg/tablet) respectively. The initial dosage of TPM was 25 mg per day, increased by 25 mg every week, the objective dosage of 100-200 mg per day was maintained when the symptoms were satisfactorily controlled. The dosage of CBZ was 6-8 mg/kg per day. Effect of Transcranial Magnetic stimulation on the Expression of c-Fos and Brain-derived Neurotrophic Factor of the Cerebral Cortex in Rats with Cerebral Infarct

The effect of transcranial magnetic stimulation （TMs） on the neurological functional recovery and expression of c-Fos and brain-derived neurotrophic factor （BDNF） of the cerebral cortex in rats with cerebral infarction was investigated. Cerebral infarction models were established by using left middle cerebral artery occlusion （MCAO） and were randomly divided into a model group （n=40） and a TMs group （n=40）. TMs treatment （2 times per day, 30 pulses per time） with a frequency of 0.5 Hz and magnetic field intensity of 1.33 Tesla was carried out in TMs group after MCAO. Modified neurological severity score （Nss） were recorded before and 1, 7, 14, 21, and 28 day（s） after MCAO. The expression of c-Fos and BDNF was immunohistochemically detected 1, 7, 14, 21, and 28 day（s） after infarction respectively. Our results showed that a significant recovery of Nss （P〈0.05） was found in animals treated by TMs on day 7, 14, 21, and 28 as compared with the animals in the model group. The positive expression of c-Fos and BDNF was detected in the cortex surrounding the infarction areas, while the expression of c-Fos and BDNF increased significantly in TMs treatment group in comparison with those in model group 7, 14, 21, and 28 days （P〈0.05） and 7 14, 21 days （P〈0.01） after infarction, respectively. It is concluded that TMs has therapeutic effect on cerebral infarction and this may have something to do with TMs＇s ability to promote the expression of c-Fos and BDNF of the cerebral cortex in rats with cerebral infarction.
