AIM To investgate the feasibility and therapeutic effect of laparoscopic surgery for pyogenic liver abscess(PLA) with biliary pathology. Methods From January 2004 to October 2010,31 patients with PLA combined with biliary pathology meeting entry criteria received surgical management in our *** the 31 patients,13 underwent laparoscopic surgery(LS group) and 18 underwent open surgery(OS group).Clinical data including operation time,intraoperative blood loss,postoperative complication rate,length of postoperative hospital stay,and abscess recurrence rate were retrospectively analyzed and compared between the two groups. Results All patients received systemic antibiotic *** patients underwent ultrasound-guided percutaneous catheter drainage before *** complications occurred in 5 patients(16.1%, 5/31) including 2 in the LS group and 3 in the OS *** patient had retained calculus in the common bile duct and another had liver abscess recurrence in the OS *** retained calculus and liver abscess recurrence occurred in the LS *** the two groups,there was no mortality during the perioperative *** were no significant differences in operation time,intraoperative blood loss and transfusion,postoperative complication rate and abscess recurrence rate between the two *** intake was earlier(1.9 + 0.4 d vs 3.1 + 0.7 d,phospital stay was shorter(11.3 + 2.9 d vs 14.5 + 3.7 d, p<0.05 ) in the LS group than in the OS group. Conclusion Laparoscopic surgery for simultaneous treatment of PLA and biliary pathology is feasible in selected patients and the therapeutic effect is similar to that of open surgery.

目的:探讨胎盘11β-HSD2和母体皮质醇水平在不同的子痫前期病理状态下的表达及其可能机制。方法:收集15例早发型重度子痫前期患者(EOSPE组)、13例晚发型重度子痫前期患者(LOSPE组)和20例正常妊娠孕妇(NP组)的血清及胎盘组织,用酶化学发光分析仪检测血清中游离皮质醇的浓度,采用免疫组化超敏即用型二步法观察胎盘11β-HSD2在合体滋养层细胞的表达,采用实时荧光定量PCR法检测胎盘组织11β-HSD2的mRNA表达。结果:正常妊娠、早发型、晚发型重度子痫前期组的血清游离皮质醇水平分别为(728.05±155.91)nmol/L、(619.80±180.57)nmol/L、(750.53±153.80)nmol/L,组间比较差异无统计学意义(P>0.05)。免疫组化胎盘11β-HSD2测量平均光密度值正常妊娠、早发型、晚发型重度子痫前期组分别为0.12±0.06、0.07±0.02、0.08±0.04,组间比较差异有统计学意义(P2的mRNA表达水平在早发型、晚发型重度子痫前期组和正常妊娠组分别为:3.16±1.21、4.86±1.43、5.23±2.20,组间比较差异有统计学意义(P2可能灭活皮质醇从而保护胎儿,11β-HSD2表达减少与SPE发病有关。

Fulminant type 1 diabetes mellitus(T1DM) and drug hypersensitivity syndrome(DHS) are rare and life-threatening *** T1DM may develop after DHS with only few previous case reports and the possible role of human herpes virus 6(HHV-6) infection was indicated in the *** here presents three cases of fulminant T1DM developed shortly after the diagnosis of drug-induced hyper-sensitivity *** offending drugs were identified as Cefadroxil,cefuroxime and phenylbutazone *** of them were treated with high dose of glucocorticoids when hyper-glycemia symptoms occurred abruptly with rapid progression to *** terms of immunological parameters,one patient was positive for both GADA and ZnT8A,and had haplotypes of DQA1*0601-DQB1*0301/DQA1*0601-DQB1**** second one had positive GADA and DQA1*0102-DQB1*0601 /DQA1*05-DQB1*0201 *** third patient was negative for all autoantibodies tested and had haplotypes of DQA1*03-DQB1*0302/DQA1*03-DQB1**** HHV-6 virus antibodies were not detected in these patients,we found herpes simplex virus type 1 IgM antibodies(HSV1 IgM) positive in one and Coxsackie virus IgM antibodies(CVB IgM) positive in another *** understanding of the pathogenesis of fulminant T1DM will be improved if the relationship among DHS,glucocorticoids and virus infection can be clarified. More attention shoule be given to this novel and serious complication of patients with DHS.
