Background:Nosocomial infections are a major threat to patients in the intensive care unit(ICU).Limited data exist on the epidemiology of ICU-acquired infections in *** retrospective study was carried out to determine the current status of nosocomial infection in china. Methods:A retrospective review of nococomial infections in the ICU of a tertiary hospital in East china between 2003 and 2007 was performed. Nosocomial infections were defined according to the definitions of Centers for Disease Control and *** overall patient nosocomial infection rate,the incidence density rate of nosocomial infections,the excess length of stay,and distribution of nosocomial infection sites were ***,pathogen and antimicrobial susceptibility profiles were further investigated. Results:Among 1980 patients admitted over the period of time,the overall patient nosocomial infection rate was 26.8%or 51.0 per 1000 patient days.,Lower respiratory tract infections(LRTI) accounted for most of the infections(68.4%),followed by urinary tract infections(UTI, 15.9%),bloodstream(BSI,5.9%),and gastrointestinal tract(GI,2.5%) *** was no significant change in LRTI,UTI and BSI infection rates during the 5 ***,GI rate was significantly decreased from 5.5%in 2003 to 0.4%in *** addition,***,*** and *** were the most frequent pathogens isolated in patients with LRTIs,UTIs and BSIs,*** rates of isolates resistant to commonly used antibiotics ranged from 24.0%to 93.1%. Conclusion:There was a high and relatively stable rate of nosocomial infections in the ICU of a tertiary hospital in china through year 2003- 2007,with some differences in the distribution of the infection sites, and pathogen and antibiotic susceptibility profiles from those reported from the Western *** for surveillance and prevention of nosocomial infections must be implemented in order to reduce the rate.

Objective To explore the clinical value of strain imaging (SI) in quantitative detection of regional myocardial diastolic function in patients with coronary artery disease (CAD). Methods Fifty-three patients with CAD and forty-two normal control subjects were enrolled in the study. Longitudinal strain curves in the basal,mid,and apical segment of LV walls were obtained. Increased value of strain during rapid filling period and atrial systolic phase (△εE and△εA) were measured,and△εE/△εA were calculated. Also,radial strain curves in the basal,mid,and apical segment of anterior interventricular septum and posterior wall of left ventricle were obtained. Decreased value of strain during rapid filling period and atrial systolic phase (△εE and△εA) were measured,and△εE/△εA were calculated too. According to the results of coronary angiography,the data in segments supplied by narrowed coronary arteries of CAD (Narrowed segments) and in corresponding normal segments of controls (Normal segments) were compared. Results Increased value of longitudinal strain and decreased value of radial strain in Narrowed segments of CAD during rapid filling period were significantly smaller,disappeared,or even showed inversion.△εE in Narrowed segments were significantly lower than those in Normal segments. No significant differences were found in△εA,while statistical significance were found in△εE/△εA between Narrowed segments and Normal segments. Conclusions SI can be used as a new technique for detection of radial and longitudinal diastolic abnormality of regional myocardium in patients with CAD noninvasively and quantitatively.

Diagnosis and treatment of male infertility associated with immotile cilia syndrome:a report of 6 cases

Objective:To investigate the clinical characteristics,diagnosis and treatment of male infertility associated with immotile cilia syndrome(ICS). Methods:The clinical data of six cases of male infertility associated with ICS were reviewed retrospectively. Results:The clinical features in all cases included chronic or recurrent infections of the upper and lower airways and male ***,bronchitis and bronchiectasis were found in all cases and situs inversus totalis found in two *** hormone levels,chromosome karyotype and Y chromosome microdeletion(AZF) in all cases were *** cases were diagnosed as severe asthenospermia by routine semen *** cases had no motile spermatozoa in semen,while there were less than 0.2%of motile sperm in one *** total sperm count and sperm viability were normal in four cases and there were very few immotile sperm in two *** electron microscopic examination of sperm flagellum revealed disarrangement or a partial absence of 9+2 microtubules and/or an absence of the dynein arms in six *** bronchial cilia in one case showed to be devoid of inner dynein *** cases underwent six intracytoplasmic sperm injection(ICSI) cycles and the rates of fertilization,embryo cleavage and good quality embryos were 50.0%,69.2%and 55.6%,*** clinical pregnancies and one chemical pregnancy were achieved,with one birth of a healthy baby boy. Conclusions:The ultrastructural defect of cilia or flagellum is the most important diagnostic criteria of ICS. ICSI is an effective treatment for male infertility associated with ICS.

＜正＞Background:Many randomized controlled trials（RCT）have been completed to evaluate the efficacy of DanShen on ischemic vascular diseases（IVD）in recent years However,there are few systemic assessments of the quality of DanShen RCTs SO ***:To evaluate the quality of DanShen RCTs which have been published in Chinese mainland from 1998 to 2007 using the CONSORT for TCM fConsolidated Standards for Reporting of Trials for traditional Chinese medicine)and the JADAD scale respectively,and explore the factors that correlate with the ***:We identified 1 50 DanShen RCTs finally through searching major Chinese electronic databases and conference *** quality of each trial was assessed using the CONSORT for TCM statement,and the JADAD scale ***—way ANOVA and Pearson correlation analysis were applied to explore the association between the baseline features and the quality of these ***:The mean score（mean 4-SD）of 1 50 DanShen RCTs assessed by the CONSORT for TCM was 23.87～3.68,and 1.94～0.82 by the JADAD *** 6.7%（10/150）of RCTs was identified with high quality（Jadad score no less than 4）.The quality of DanShen RCTs correlates with the factors such as authors’affiliation;publication journal;the sample size;and the follow—up ***:The quality of DanShen RCTs in Chinese mainland has not been improved apparently in recent years,and the overall quality of DanShen RCTs is poor yet

Objective The present study was designed to evaluate the surgical procedure,taken according to intra-operative vEEG monitoring,of patients harboring one or more supratentorial cerebral cavernous malformations,with epileptic *** and Methods 25 cases,admitted to our department between January 1998 and June 2005,harboring one or more supratentorial cerebral cavernous malformations(CCMs), with epileptic seizures,were studied *** were divided into two groups according to vEEG examination and *** A included patients with epileptic focus and CCM located in or near area motoria,and group B included patients with epileptic focus and CCM away from area ***-operative vEEG monitoring had been used,and the disappearance of sharp wave,spike wave and spike-ware wave were taken as the key-sign for the end of microsurgical *** Lab neuronavigation has been applied in 12 patients with deep-located *** All the CCMs had been totally resected,without peri-operative death and permanent neurological *** them,5 patients received CCM resection only,and 7 patients had CCM and surrounding hemosiderin-stained brain tissue resected,and the other 13 patients had done partly cortex lesionectomy,resection of hippocampus and amygdaloid nuclear,cortex thermocoagulation,and multiple subpial transaction,besides CCM resection with or without resection of surrounding hemosiderin-stained brain tissue. After 10 to 54 months following-up,6 patients had EngelⅠand 2 patients had EngelⅡand 1 patient had EngelⅢoutcome in group A,and all 16 patients had EngelⅠoutcome in group B(IA 13,IB 3).Conclusion Different surgical procedures were used in different patients,and the intra-operative vEEG monitoring took an important role in procedure *** most cases,excision of the CCM and focal epilepto-genicity under intra-operative vEEG monitoring is safe and effective for seizure *** incomplete lesion excision,as it l
