Objective To compare and evaluate the efficacy of diagnosis and excision for appropriately selected breast multi-focal lesions and solitary lesion by ultrasound-guided vacuum-assisted biopsy(UGVAB).Methods Among 392 appropriately selected patients,187 patients with multi-focal lesions and 205 patients with solitary lesion were treated by the 8-gauge UGVAB from May 2007 to June *** lesions were removed as completely as *** patients with benign pathology underwent physical and ultrasound examinations at one week and 6 months after *** During the procedure,only three patients had vasovagal syncope and twenty others complained of other intraoperative *** accurate pathological diagnosis was obtained in all *** was no apparent false-negative result among the 696 lesions with benign pathology at a follow-up of 6 months after *** rates of malignant or premalignant pathology,postoperative complications and residual lesions in patients with multi-focal lesions were higher than those in patients with solitary *** each lesion was considered as a subject of study,there was no significant difference between the two *** UGVAB is an effective method for diagnosis and excision of appropriately selected breast multi-focal lesions and can be used routinely.

OBJECTIVE: To assess the role of early peritoneal lavage and drainage in the management of severe acute pancreatitis. DATA sOURCEs: Early peritoneal lavage and drainage was defined as that lavage and drainage should be done in the peritoneal cavity without interference of the pancreas within 72 hours after onset of acute pancreatitis. Biomedical literature database (Medline) from 1981 through 2003 was retrieved and papers about this treatment were analyzed. REsULTs: Nine papers retrieved included 179 patients with severe acute pancreatitis. In 108 patients undergoing closed peritoneal lavage and drainage, 7 died, in 15 patients having laparoscopic procedure, 1 died, and in 56 patients having open procedure 2 died. The total survival rate was 94.4%. CONCLUsIONs: Even if extensive pancreatic necrosis occurs, early peritoneal lavage and drainage is feasible to keep the inflammatory pancreas intact and drain peri-pancreatic region in an attempt to improve the survival rate.

In order to improve the electrochemical kinetic performances of La-Mg-Ni-based alloy,complex surface modification of Ni with excellent catalytic activity and conducting polymer polypyrrole(PPy)was performed via electroless plating *** images revealed that the complex Ni-PPy treatment resulted in more micropores at the alloy surface,with Ni particles and cotton fiber-shape PPy microspheres *** the larger surface area induced by the micropore and the higher catalytic activity and conductivity on account of the dispersed Ni particles/PPy microspheres promoted the electrode reaction,thereby increasing the discharge capacity of the modified alloy *** impedance spectroscopy(EIs)and linear polarization results showed that the Ni-PPy treatment decreased the charge-transfer resistance and increased the exchange current density greatly,far more than the single-component Ni or PPy ***,a notable improvement in high rate dischargeability(HRD)was observed,and at a high discharge current density of 1800 mA/g,the HRD of the modified electrode increased by 10.4%compared with that of the bare electrode.

Myoblast transplantation (MT) is a cell-based gene therapy treatment, representing a potential treat-ment for Duchenne muscular dystrophy (DMD), cardiac failure and muscle trauma. The rapid and mas-sive death of transplanted cells after MT is considered as a major hurdle which limits the efficacy of MT treatment. Heat shock proteins (HsPs) are overexpressed when cells undergo various insults. HsPs have been described to protect cells in vivo and in vitro against diverse insults. The aim of our study is to investigate whether HsP overexpression could increase myoblast survival after autotransplantation in pig intact skeletal muscle. HsP expression was induced by warming the cells at 42℃ for 1 h. HsP70 expression was quantified by Western blot and flow cytometry 24 h after the treatment. To investigate the myogenic characteristics of myoblasts, desmin and CD56 were analysed by Western blot and flow cytometry; and the fusion index was measured. We also quantified cell survival after autologous transplantation in pig intact skeletal muscle and followed cell integration. Results showed that heat shock treatment of myoblasts induced a significative overexpression of the HsP70 (P ss of their myogenic characteristics as assessed by FACs and fusion index. In vivo (n=7), the myoblast survival rate was not significantly different at 24 h between heat shock treated and non- treated cells (67.69% ± 8.35% versus 58.79% ± 8.35%, P > 0.05). However, the myoblast survival rate in the heat shocked cells increased by twofold at 48 h (53.32% ± 8.22% versus 28.27% ± 6.32%, P 20 h (26.33% ± 5.54% versus 8.79% ± 2.51%, P stological analy-sis showed the presence of non-heat shocked and heat shocked donor myoblasts fused with host myoblasts. AIM: To investigate the influence of L-methionine-deprived total parenteral nutrition with 5-FU on gastric cancer and host metabolism. METHODs: N-methyl-N'-nitro-nitrosoguanidine (MNNG) induced gastric cancer rats were randomly divided into four groups: Met-containing TPN group (n=11), Met-deprived TPN group (n =12), Met-containing TPN+5-FU group (n=11) and Met-deprived TPN+5-FU group (n=12). Five rats in each group were sacrificed after 7 days of treatment and the samples were taken for examination. The remaining rats in each group were then fed separately with normal diet after the treatment until death, the life span was noted. REsULTs: The tumors were enlarged in Met-containing group and shrank in Met-deprived group markedly after the treatment. The DNA index (DI) of tumor cells and the body weight (BW) of rats had no significant change in the two groups, however, the ratio of tumor cells's phase was increased. The ratio of G2M phase went up in Met-containing group, but down in Met-deprived group. In the other two groups that 5-FU was added, the BW of rats, and the diameter of tumors, the DI of tumor cells, the s and G2M phase ratio of tumor cells were all decreased, particularly in Met-deprived plus 5-FU group. Pathological examination revealed that the necrotic foci of the tumor tissue increased after Met-deprived TPN treatment, and the nucleoli of tumor cells enlarged. In MetTPN+5-FU group, severe nuclear damage was also found by karyopyknosis and karyorrhexis, meanwhile there was slight degeneration in some liver and kidney cells. The serum free Met and Cysteine decreased markedly (Ps, such as serum free serine and glutamine increased significantly (Ps died of multiple organ failure caused by cancer metastasis. 目的 :观察甲氧氯普胺与多潘立酮 2种胃动力药联合应用对剖宫产术后预防和减轻术后腹胀的效果。方法 :选择剖宫产产妇 30 0例 ,按手术顺序分成A ,B 2组。A为奇数 ,B为偶数各 15 0人。A为对照组 ,术后立即肌内注射维生素B110 0mg ,6h后给复合维生素B 2片 ,po ,tid ,餐前 30min ,至肛门排气停药 ;B组为试验组 ,剖宫产术后立即肌内注射甲氧氯普胺 10mg ,6h后给多潘立酮 10mg ,po ,tid ,餐前 30min ,至肛门排气停药。通过肛门排气时间观察疗效。结果 :B组显效 4 2 % ,有效 5 6% ,总有效率 98%。A组显效 8% ,有效 2 6% ,总有效率 34% ,2组比较差异有非常显著意义 (P 2例出现轻微腹痛。结论

Ischemic preconditioning (IPC) protects liver graft function following ischemia in liver transplantation and liver resection. The aim of this study was to assess the advantages and any potential disadvantages of liver IPC prior to isolation for rat hepatocytes during isolation and cryopreservation. After isolating and thawing the cryopreserved hepatocytes after 14 and 28 d,cell viability,efficiency,and lactate dehydrogenase (LDH) levels in preserve solution were examined for every group. Groups treated with IPC had better cell viability determination,assessment of plating efficiency and lactate dehydrogenase (LDH) assay than Group without IPC,suggesting that IPC prior to isolation may have a significant protective effect on hepatocytes subjected to isolation and short period cryopreservation.

AIM:To investigate the endoscopy and histology of short-segment Barrett's esophagus (ssBE) and cardia intestinal metaplasia (CIM),and their correlation with Helicobacter pylori (H. pylori) gastritis and gastroesophageal reflux disease (GERD). METHODs:Biopsy specimens were taken from 32 ssBE patients and 41 CIM patients with normal appearance of the esophagogastric junction. Eight biopsy specimens from the lower esophagus,cardia,and gastric antrum were stained with hematoxylin/eosin,Alcian blue/periodic acid-schiff,Alcian blue/high iron diamine and Gimenez dye. Results were graded independently by one pathologist. REsULTs:The ssBE patients were younger than the CIM patients (P splasia and incomplete intestinal metaplasia subtype was higher in ssBE patients than in CIM patients (P s correlated with antral intestinal metaplasia (P symptomatic,endoscopic,or histological markers of GERD in CIM patients. ssBE was correlated with reflux symptomatic and endoscopic esophagitis (P stinal metaplasia. CONCLUsION:Dysplasia risk is significantly greater in ssBE patients than in CIM patients. CIM is a manifestation of H. pylori-associated and multifocal atrophic gastritis,whereas ssBE may result from GERD.

BACKGROUND: Hepatic artery thrombosis is one of the serious complications after liver transplantation. It wil mostly cause a failure of the transplantation. This case o hepatic artery thrombosis showed a stable clinical course and minimal histological change, and now has been survi ving for 4 years with normal liver function. We investigated the possible causes for asymptomatic hepatic artery throm bosis in one patient after orthotopic liver transplantation ( OLT) and discussed the diagnosis of ischemia of OLT pathologically and clinically. METHODs: Liver function test, color Doppler ultrasono graphy, and hepatic arteriography were performed during the development of hepatic arteriothrombosis. Possible fac- tors for the asymptomatic process of the thrombosis were analyzed. REsULTs: On the 4th postoperative day, thrombosis form ed at the anastomotic stoma of the hepatic artery, and on the 11th postoperative day, the artery was completely oc- cluded. serial liver biopsies revealed intrahepatic cholesta- sis, hydropic degeneration of hepatocytes, atrophy of the biliary epithelium, and fibrosis in the portal area. Monitor- ing of liver function showed nothing abnormal except ele- vation of γ-GT and ALP levels. On the 71st day after OLT, arteriography demonstrated that the hepatic artery re- mained completely occluded in addition to the establish- ment of collateral circulation and compensation of the por- tal vein. The patient didn' t show any symptoms of arterial thrombosis. CONCLUsION: Collateral circulation and compensation o the portal vein are beneficial to allograft survival and avoidence of retransplantation after thrombosis of the he- patic artery. Color Doppler ultrasonography within 2 weeks after OLT is helpful to the early diagnosis of hepatic arterio- thrombosis.
