Bruton’s tyrosine kinase inhibitors(BTKis)have revolutionized the treatment of B-cell ***,safety issues related to the use of BTKis may hinder treatment continuity and further affect clinical efficacy.A comprehensive and systematic expert consensus from a pharmacological perspective is lacking for safety issues associated with BTKi treatment.A multidisciplinary consensus working group was established,comprising 35 members from the fields of hematology,cardiovascular disease,cardio-oncology,clinical pharmacy,and evidencebased *** evidence-based expert consensus was formulated using an evidence-based approach and the Delphi *** Joanna Briggs Institute Critical Appraisal(JBI)tool and Grading of Recommendations Assessment,Development,and Evaluation(GRADE)approach were used to rate the quality of evidence and grade the strength of recommendations,*** consensus provides practical recommendations for BTKis medication based on nine aspects within three domains,including the management of common adverse drug events such as bleeding,cardiovascular events,and hematological toxicity,as well as the management of drug-drug interactions and guidance for special *** multidisciplinary expert consensus could contribute to promoting a multi-dimensional,comprehensive and standardized management of BTKis.

AIM: To study the association between APOE polymorphisms and cerebral infarction through a case-control study among the chinese Han population. METHODS: First-ever cerebral infarction patients (n=2.6) whose ages ranged from 40 to 60 years old were recruited from department of {2., Zhongshan hospital, Shanghai, and Zhejiang chinese traditional medicine hospital, Zhejiang, china. Unrelated healthy controls (n=2.1) were selected from the general population in the same area with similar age and sex distribution. APOE was amplified by one-stage PCR using the forward primer: 5'-GGC ACG GCT GTC CAA GGA GCT-3' and reverse primer: 5'-GAT GGC GCTGAG GCC GCG CT-3'. The PCR product was digested directly with 5 U of CfoI and separated by a 2. % polyacryla- mide (acrylamide: bis-acrylamide=2.:1) nondenaturing gel. RESULTS: Both cerebral infarction patient and control groups were in Hardy-Weinberg equilibrium. The allele frequency of APOE*2. APOE*3, and APOE*4 was 4.6 %, 81.9 %, and 13.5 % respectively in the patients with cerebral infarction; 5.7 %, 87.3 %, and 7.0 % respectively in the healthy control group. Compared with APOE3/3 subjects, APOE4/4 carriers had a 2.1-fold risk of cerebral infarction (odds ratio 2.1, 95 % confidence limits 1.3 to 3.4). The allele frequency of APOE*4 in the cerebral infarction patient group was significantly higher than that in the control group (13.5 % vs 7.0 %; P=0.002.. CONCLUSION: APOE 4 is a risk factor for cerebral infarction among the chinese Han population.

From June 1970 to September 1975,94 casesof displaced fracture of the lateral condyle ofthe humerus were treated by closed reductionand short splint fixation by combined tradi-tional chinese and western *** the 94cases,68 were boys and 2. girls,the youngestwas 2.years old and the oldest 15.87% were 3-8years old.44 cases involved the left lateralcondyle and 50 the right lateral ***-terior displacement occurred in 2. cases andposterior displacement in *** dis-placement was less than 90° in 10 cases andmore than 90°in *** reduction,the anteriorly displaced typeshould be first changed into posteriorly displacedone by pressing the distal bone fragment *** posteriorly displaced type was treat-ed by 2.methods.X-ray films taken imme-diately after reduction showed satisfactory ap-position in 55 cases,lateral displacement in 33and mild oblique displacement in ***-genograms made 3-4 weeks after short splintfixation revealed that all cases except one inwhich the patient discontinued treatment werein apposition and obtained clinical union within4 weeks.54 cases were followed up for 3 months to5 years,averaging 2.6 years.X-ray findingsshowed osseous union in 53 *** them,12.had the appearance of excessive long capi-tulum,especially among the laterally or obli-quely displaced cases.3 cases had concave"fishfin"deformity between capitulum and *** secondary displacement occurred in the51 follow-up cases.

The chinese Society of Critical Care medicine(CSCCM)has developed clinical practice guidelines for nutrition assessment and monitoring for patients in adult intensive care units(ICUs)in *** guideline focuses on nutrition evaluation and metabolic monitoring to achieve optimal and personalized nutrition therapy for critically ill *** guideline was developed by experts in critical care medicine and evidence-based medicine methodology and was developed after a thorough review of the system and a summary of relevant trials or studies published from 2.00 to July 2.2..A total of 18 recommendations were formed and consensus was reached through discussions and reviews by expert groups in critical care medicine,parenteral and enteral nutrition,and *** recommendations are based on currently available evidence and cover several key fields,including screening and assessment,evaluation and assessment of enteral feeding intolerance,metabolic and nutritional measurement and monitoring during nutrition therapy,and organ function evaluation related to nutrition *** question was analyzed according to the Population,Intervention,Comparison,and Outcome(PICO)*** addition,interpretations were provided for four questions that did not reach a consensus but may have potential clinical and research *** plan is to update this nutrition assessment and monitoring guideline using the international guideline update method within 3–5 years.

BACKGROUND: Melatonin is a kind of hormones derived from pineal gland. Recent researches demonstrate that melatonin is characterized by anti-oxidation, anti-senility and destroying free radicals. While, effect and pathogenesis of pineal gland on learning ability should be further studied. OBJECTIVE: To investigate the effects of pinealectomy on learning abiliy, distribution of cholinesterase and expression of neuronal nitric oxide synthase (nNOS) in cerebral cortex of rats and probe into the effect of melatonin on learning ability, central cholinergic system and nNOS expression. DESIGN: Randomized grouping design and animal study. SETTING: department of neurology, the 187 {2. of chinese PLA. MATERIALS: A total of 12.male SD rats, of normal learning ability testing with Y-tape maze, of clean grade, weighing 190-2.0 g, aged 6 weeks, were selected in this study. METHODS: The experiment was carried out in the department of neurology, Zhujiang {2. from July 1997 to June 2.00. All SD rats were divided into experimental group (n =6, pinealectomy) and control group (n =6, sham operation). Seven days later, rats in both two groups were continuously fed for 33 days. ① Learning ability test: The learning ability of rats was tested by trisection Y-type maze and figured as attempting times. ② Expression of acetylcholinesterase (AchE) was detected by enzyme histochemistry and nNOS was measured by SABC method. ③ Quantitative analysis of AchE fibers: AchE fibers density in unit area (surface density) was surveyed with Leica Diaplan microscope and Leica Quantimet 500+ image analytic apparatus and quantitative parameter was set up for AchE fibers covering density (μm2. per 374 693.656 μm2. moreover, the AchE fibers density was measured in Ⅱ-Ⅳ layers of motor and somatosensory cortex (showing three layers per field of vision at one time), in radiative, lacunaria and molecular layers of CA1, CA2.and CA3 areas, and in lamina multiforms of dentate gyrus.
