A new family of windows——convolution windows and their applications

A new family of windows is constructed by convolutions via a few rectangular windows with same time width and is thus referred to as convolution windows. The expressions of the second-order up to the eighth-order convolution windows in both the time and frequency domains are derived. Their applications in high accuracy harmonic analysis of periodic signals are investigated. Comparisons between the proposed windows and some known windows with the same width shows that, when the synchronous deviation of data sampling is slight, the proposed ones have the least effect of spectral leakage. Therefore, the new windows are well suited for high accuracy harmonic analysis and parameter estimation for periodic signals. The error analysis and computer simulations show that the estimation errors, corresponding to frequency, amplitude and phase of every harmonic component of a signal, are proportional to the pth power of the relative frequency deviation in case of the pth-order convolution window is applied to windowing signal of approximately p cycles. By introducing real time adjustment in sampling interval, the proposed algorithm can adaptively trace signal frequency and lead to less sampling synchronous deviation. The proposed approach has the advantages of easy implementation and high measure precision and can be used in harmonic analysis of quasi-periodic signals whose fundamental frequency drifts slowly with time.

精准外科理念的提出与现代医学技术进步和学科交叉融合密切相关, 并深刻影响着外科决策方向和治疗范式。其核心要义是利用可视化、可量化和可控化等关键外科技术, 在病灶清除、脏器保护、损伤控制三者之间达到平衡, 从而实现患者受益最大化。在肝癌诊断与治疗领域, 精准外科理念应贯穿于疾病诊断与治疗全过程, 包括术前精准评估、术中精细操作和术后精心管理等。这些先进理念的实现, 依赖于技术不断进步和流程不断优化。然而, 当前肝癌的诊断与治疗依然面临诸多挑战, 如术前肿瘤生物学行为的精确评估、术中精准切除技术的优化, 以及术后复发监测与干预措施的完善等。应用分子影像、循环源标志物及人工智能等技术手段, 持续完善精准外科理念并拓展其实践范围, 并将其贯穿于肝癌诊断与治疗的各个环节, 才能全面提升手术安全性与疗效, 为患者提供更科学、更高效的治疗选择。笔者将精准外科理念应用于肝癌的诊断与治疗实践和对未来发展方向的思考进行系统阐述。

AIM:To investigate the endoscopy and histology of short-segment Barrett's esophagus (SSBE) and cardia intestinal metaplasia (CIM),and their correlation with Helicobacter pylori (H. pylori) gastritis and gastroesophageal reflux disease (GERD). METHODS:Biopsy specimens were taken from 32.SSBE patients and 41 CIM patients with normal appearance of the esophagogastric junction. Eight biopsy specimens from the lower esophagus,cardia,and gastric antrum were stained with hematoxylin/eosin,Alcian blue/periodic acid-Schiff,Alcian blue/high iron diamine and Gimenez dye. Results were graded independently by one pathologist. RESULTS:The SSBE patients were younger than the CIM patients (P < 0.01). The incidence of dysplasia and incomplete intestinal metaplasia subtype was higher in SSBE patients than in CIM patients (P < 0.01). H. pylori infection was correlated with antral intestinal metaplasia (P < 0.05),but not with reflux symptomatic,endoscopic,or histological markers of GERD in CIM patients. SSBE was correlated with reflux symptomatic and endoscopic esophagitis (P < 0.01),but not with H. pylori infection and antral intestinal metaplasia. CONCLUSION:Dysplasia risk is significantly greater in SSBE patients than in CIM patients. CIM is a manifestation of H. pylori-associated and multifocal atrophic gastritis,whereas SSBE may result from GERD.

酪氨酸激酶抑制剂(TKI)治疗显著改善了慢性粒细胞白血病(CML)患者的预后,停止TKI治疗后实现无治疗缓解(TFR)是CML患者追求的新目标。多种停药方法正在不断探索中,成功停药与TKI的治疗时间、深度分子学反应(DMR)持续时间、转录本类型、免疫学因素等影响因素有关。早期转换TKI、联合其他疗法以及靶向白血病干细胞治疗可能有助于提高TFR率。本文就TKI停药的探索、影响TFR的因素、停药患者的选择、提高TFR的策略、停药后的监测等方面的最新研究进展作一综述,以期为CML患者实现TFR提供借鉴和参考。
