Objective: To assess the influence of preoperative transcatheter arterial chemoembolization (TACE) on liver resection in patients with resectable hepato- cellular carcinoma (HCC). Methods: Of 126 patients with resectable HCC, 62 received preoperative TACE (TACE group) and the remaining 64 patients were selected as controls (non- TACE group). Perioperative risk factors including liver function alteration, mean blood loss during op- eration, mean time of clamping the porta hepatis, length of operation, postoperative abdominal drain- age at day 1, 2 and 3, morbidity and mortality were compared between the two groups. Results: Neither significant difference in liver func- tion alteration nor mortality was observed between the two groups. More severe hepatic cirrhosis, lon- ger operation time, more blood loss and postopera- tive abdominal drainage were noted in the TACE group than in the non-TACE group. There was no significant difference in postoperative morbidity be- tween the two groups. Conclusions: Preoperative TACE for resectable HCC increases surgical difficulty and risk, and therefore should be considered prudently according to the indi- viduality of patients.

In an experimental rat’s renovascular hypertension model, we studied the genesis of anti- cardiac β1-adrenoceptor and M2-muscarinic receptor autoantibodies in relation to the changes in immunological function during the development of renal hypertension. The biological activities of these autoantibodies were also examined. It was shown that after two weeks of operation both the frequency of occurrence and the titre of autoantibodies to cardiac β1-adrenoceptor and M2-muscarinic receptor were significantly increased as compared with the control of pre-treatment. The increased autoantibodies lasted for several weeks and then automatically decreased gradually to the pre-clipping level at 10 weeks. Meanwhile the ratio of CD4 /CD8 + + was also undergone an initial increase followed by gradual recovery and correlated well with the changes in antibody titre. The biological effects of these autoantibodies displayed an “agonistic-like” activities on the beating frequency of cultured neonatal cardiomyocyte. It is suggested that autoimmune mechanisms are involved in the pathogenesis of renal hypertension and the cardiac receptor autoantibodies might be one of the mechanisms leading to cardiac dysfunction.

OBJECTIVES: To detect blood AFPmRNA in the nude mice bearing with human hepatocellular carcinoma (HCC) by using nested reverse transcriptase polymerase chain reaction (nested RT-PCR), and assess its significance in HCC distant metastasis. METHOD: We detected 20 blood samples from the nude mice bearing with human HCC by nested RT-PCR to find out AFPmRNA. RESULT: AFPmRNA was detected in 6 blood samples from the nude mice hearing with human HCC (30. 0%), in which 4 mice developed distant metastasis. CONCLUSION: AFPmRNA may be used as an efficient and sensitive marker to detect blood spread of HCC cells. It can predict the occurrence of HCC distant metastasis.

Objective: To study the indications for resection ofvery big primary liver cancer and the operative ***: From January 1985 to June 1996, 86 pa-tients with very big primary liver cancer (≥15cm indiameter) underwent hepatectomy in our *** volume of bleeding and blood transfusion wasrecorded during the operation. After the operation,the draining quantity from their abdominal cavities,and the days of transfusion and hospitalization wererecorded. The occurrence of complications and sur-vival time of the patient were followed ***: The postoperative mortality was 3.48% andthe occurrence rate of complications was 31.40%, whichwas significantly correlated with preoperative lowerlevel of serum albumin or the elevated γ-globulin lev-el and the amount of resected liver tissue. But their liverfunction before operation was fairly good, the 1-, 3-and 5-year survival rates after hepatectomy were58.2%, 35.7% and 17.64%.Conclusions: Patients with very big primary liver can-cer, should be subjected to hepatectomy if their liverfunction before operation are normal and the marginsare distinct between the tumor and liver tissues. Afterthe operation, other treatments are suitable for goodeffects.

BACKGROUND: hepatic ischemia-reperfusion injury is one of the major problems in liver transplantation. This study aimed to investigate the possible improvement of aerobic metabolism of ischemic donor livers through systemic persufflation with gaseous oxygen in rinse solution. METHODS: Thirty rats were randomly divided into 3 groups. In group A (control, n = 10), the livers were perfused with 4 ℃ lactic acid ringer's solution through the can-nulated portal vein for 25 minutes soon after warm ischemia for 30 minutes. In group B (n =10), the livers were treated the same way as in group A except for addition of gaseous oxygen in the rinse solution. In group C (n =10), the livers were treated similarly as in group B except for addition of superoxide dismutase (SOD) in the rinse solution. RESULTS: In group B, the malondialdehyde(MDA)levels in hepatic tissues after perfusion were significantly increased (Phepatic tissues after perfusion recovered significantly (P<0.01). CONCLUSION: Perfusion by gaseous oxygen persufQation in combination with antioxidative agents is promising for resuscitating the ischemically altered livers for successful transplantation.

Prevention and management of pleural effusion following hepatectomy in primary liver cancer

Postoperative pleural effusion occurs frequently after hepatectomy. The risk factors, prevention and management of postoperative pleural effusion in patients with primary liver cancer (PLC) who have undergone hepatectomy and the value of the argon beam coagulator (ABC) for the prevention of pleural effusion are studied. METHODS:A total of 523 patients with PLC at our institution who had had right hepatectomy from July 2000 to June 2004 were studied retrospectively. Comparative analysis was made to identify the factors contributing to postoperative pleural effusion and the efficacy of various managements. RESULTS:Of the 523 patients whose livers were dissociated using argon beam cutting and/or coagulation, 20(3.8%) developed pleural effusions;whereas in the other 467 patients underwent hepatectomy with suture ligation of the diaphragmatic secondary wound surface during the same period, 49(10.5%) had pleural effusion (Phepatic insufficiency with ascites, duration of hepatic occlusion and underlying cirrhosis. CONCLUSIONS: Dissociation of the liver by argon beam cutting and/or coagulation can save suture ligation of the diaphragmatic secondary wound surface and may also prevent postoperative pleural effusion. Pleural drainage using an indwelling central-venous-catheter (CVC) in the pleural cavity is safe and efficacious.

Aim:To determine whether glycine could downregulate interleukin 1 receptorassociated kinase-4(IRAK-4)expression to interfere with lipopolysaccharides(LPS)signal transduction and blunt transplantative liver ischemia-reperfusioninjury(I/RI).Methods:SD rats were randomly divided into two groups:donoranimals of the glycine group(n=40)were given glycine(1.5 mL;300 mmol/L,iv)1h before harvest,and the control group were treated with 1.5 mL physiologicalsaline(n=40).Orthotropic liver transplantation was then performed according tothe Kamada *** animals in each group were followed up for 7 d aftersurgery to assess *** remaining animals in each group were dividedinto 3 subgroups(n=10)at 1h,2h and 6 h after portal vein *** ofLPS,serum aspartate aminotransferase(AST),alanine aminotransferase(ALT)and total bilirubin in portal circulation,as well as IRAK-4 and TNF-α expression,NF-κB transcriptional activity and morphological study of liver tissues ***:Reperfusion resulted in a significant elevation of LPS concen-trations in each group persisting to the end of our ***,glycine,whichled to improved survival rate and liver function,significantly alleviated liver pa-renchyma cell damage by downregulating 1RAK-4,TNF-α expression and NF-κBtranscriptional activity compared with the control ***:Glycinecan attenuate hepatic I/RI by downregulating IRAK-4 to interfere with LPS signaltransduction.

BACKGROUND:Why 3.3% to 10% of all patients with hepatolithiasis develop intrahepatic cholangiocarcinoma (ICC) remains *** carried out a hospital-based case-control study to identify risk factors for the development of ICC in patients with hepatolithiasis in ***:Eighty-seven patients with pathologically diagnosed hepatolithiasis associated with ICC and 228 with hepatolithiasis alone matched by sex,age (±2 years),hospital admittance and place of residence were interviewed during the period of *** ratios (OR) and 95% confidence intervals (CI) were calculated for each risk ***:Among the patients with hepatolithiasis associated with ICC,the mean age was 57.7 years and 61.0% were female Univariate analysis showed that the significant risk factors for ICC development in hepatolithiasis were smoking,family history of cancer,appendectomy during childhood (under age 20),and duration of symptoms >10 *** multivariate stepwise logistic regression analysis,smoking (OR=1.931,95% CI:1.000-3.731),family history of cancer (OR=5.175,95% CI:1.216-22.022),and duration of symptoms >10 years (OR=2.348 95% CI:1.394-3.952) were independent ***:Smoking,family history of cancer and duration of symptoms >10 years may be risk factors for ICC in patients with hepatolithiasis.
