Hepatocellular carcinoma(HCC) is one of the most common human cancers worldwide, particularly in Southeast Asia and *** than 70-80%of HCC cases occur in high hepatitis B virus(HBV) endemic regions,and 50%of HCC cases worldwide are attributable to chronic infection with ***,the 5-year survival rate of HBV-related HCC patients remains poor,and approximately 600,000 HCC patients die each year despite recent advances in surgical techniques and medical *** previous studies identified many aberrantly expressed protein-coding genes in HCC,novel molecular markers that can help in early diagnosis and risk assessment are still urgently *** is of paramount importance to understand the relationships between clinical symptoms and molecular changes in HCC for developing new diagnosis and treatment strategies for HCC and improving the prognosis of diagnosed patients. The human transcriptome comprises not only large numbers of protein-coding messenger RNAs(mRNAs) but also a large set of non-protein coding transcripts that have structural,regulatory or unknown *** studies of small non-coding RNAs(18 to 200 nucleotides(nt)) have dominated the field of RNA biology in recent years,a surprisingly wide array of cellular functions has also been associated with long non-coding RNAs(IncRNAs).IncRNAs,tentatively defined as non-coding RNAs more than 200 nt in length,are characterized by the complexity and diversity of their sequences and mechanisms of action.A handful of studies have implicated IncRNAs in a variety of disease states,and altered IncRNA levels can result in aberrant expression of gene products that may contribute to cancer ***,there are only preliminary studies on the role of IncRNAs in HCC,and the overall pathophysiological contributions of IncRNAs to HCC remain largely unknown. Adenovirus-expressed preS2 antibody inhibits hepatitis B virus infection and hepatic carcinogenesis

Post-Surgical Recurrence of Hepatocellular Carcinoma

Objective To analyze post-surgical recurrence of hepatocellular carcinoma (HHC) according to pathologic findings, primary tumor and angiographic features of the recurrent tumor.Methods In this series, 100 cases of recurrent HCC were analyzed in following aspects: ( 1) size, tumor nodular numbers, gross and histologic findings of the primary tumor; (2) post-surgical recurrent time; (3) size, tumor nodular numbers, blood supply, staining property, and accumulation of lipiodol oil in the recurrent tumor. Following angiography, arterial chemoembolization was performed. Results In the primary tumor, single nodules were seen in 80 cases, multiple nodules in 16 cases and multiple fused nodules in 4 cases. All tumors were classified as: trabecular type, 65 cases; compact type, 12; sclerotic type, 2 cases; mixed type, 15 cases and cholangiocarcinoma type, 6 cases. 38 cases had incomplete or no capsule. Satellite tumor nodules were grossly identified during operation in 33 cases, but were proven microscopically in 66 cases. Tumor thrombi of portal vein was noted in 18 cases during oeration, but 85 cases in histopathological sections. The recurrent tumors were diagnosed post surgically within 6 months in 67 cases, 6-12 months in 15 cases and after 12 months in 18 cases respectively. On angiography, 67 % recurrent tumors was rich in blood supply and with abundant accumulation of lipiodol after embolization.Conclusion The post-surgical recurrence rate of the HCC patients with massive size, incomplete or no capsule, satellite tumor nodules and portal vein tumor thrombus was high. The patients shoud receive angiography in 1 ?2 months after surgery in order to detect early recurrence and, if confirmed, the patients may be treated by transcatheter arterial chemoembolization

Effect of Sirpα1 on the expression of nuclear factor-kappa B in hepatocellular carcinoma

BACKGROUND:Signal regulatory protein alpha1(Sirpα1) is a member of Sirps families containing four immunoreceptor tyrosine-based inhibitory motifs(ITIMs) domains in the cytoplasm of and an activated substrate of receptor tyrosine kinase(RTK),that negatively regulates the RTK-dependent cell proliferating signal transduction *** we found that Sirpα1 was closely associated with the occurrence and development of hepatocellular carcinoma(HCC)as well as liver *** it is unclear about the regulatory mechanisms,we established the cell line transfected Sirpα1 gene and preliminarily clarified the mechanisms by which Sirpα1 negatively regulates the carcinogenesis and development of HCC. METHODS:Liver cancer Sk-Hep1 cell was respectively transfected with plasmids of pLXSN,pLXSN-Sirpα1 and pLXSN-Sirpα1Δ4Y 2 ,screened with the drug of G418(1200 μg/ml),and various transfected Sk-Hep1 cell lines were *** protein expressions of P65,P50,IκBα,cyclin D1 and Fas in various Sk-Hep1 cell lines were determined by Western blotting,and P65 and P50 were localized by the immunofluorescence technique. RESULTS:Sirpα1 could significantly upregulate the protein expression of IκBα(*** cell lines,P2 ,but in nucleus of the Sk-Hep1 cell with wild Sirpα1.P50 protein expression was localized in the cytoplasm and nucleus of the pLXSN Sk-Hep1 cell,but in the nucleus of the Sk-Hep1 cell with wild Sirpα1 and mutant Sirpα1Δ4Y 2 plasmid. Resection of the Caudate Lobe Tumor of Liver

Objective To analyze the operative technique and results of the resection for caudate lobe carcinoma of the liver. Methods The liver was fully freed of the ligments, short hepatic veins were divided and sutured, and the tumor was then freed from the inferior vena cava(IVC) . The caudate lobe was resected alone or in combination with other segment.Results 28 patients underwent resection of caudate lobe tumor. The number of the short hepatic veins transected and tied was 2-5 ( mean 3) . An intermittent Pringle' s manoeuver was used in 26 patients with a median occlusion time of 21.7 ( range 10-32) min. The median blood loss was 574 (range 100? 300) ml. No major complications such as massive bleeding and biliary fistula occurred. Intraop-erative total vascular occlusion was perfored on only 5 cases. All patients were discharged from the hospital. Outpatient periodic TAE plus chemotherapy was performed. Postoperative recurrence and metastasis was found in 13 cases,with 5 deaths.Conclusion The use of third porta hepatis dissection for resection of the caudate lobe tumor can reduce the risk of massive bleeding during the operation and can raise the rate of resection of caudate lobe tumor.

BACKGROUND:Soft pancreatic texture and a small main pancreatic duct are thought to be the most significant risk factors for the occurrence of pancreatic fistula (PF),a common and serious complication after pancreaticoduodenectomy (PD).This is in part due to the technical difficulties of pancreaticojejunostomy (PJ) posed by a soft gland with a normal-sized *** deal with this problem,we developed a new anastomotic technique which combines the two most widely used techniques,namely,the invagination technique and the duct-to-mucosa technique,with a modification of the suture route and insertion of a temporary stent ***:Between January 2003 and December 2009,ninetytwo consecutive patients underwent PD in which the new PJ technique was *** and follow-up data of these patients were reviewed for operative details,early postoperative events,and outcomes at 6 months after the *** was defined by the International Study Group on Pancreatic Fistula (ISGPF) guidelines and graded (A,B or C) according to the clinical procedures and ***:In this group of 92 patients,there was only 1 early death from acute renal *** was observed in 11 patients (12.0%),8 in grade A,1 in grade B,and 2 in grade *** the 2 patients in grade C,PF was surgically *** were no early or late deaths attributable to *** months after the operation,all of the patients were free of PJ-related symptoms except for 2,who were found to have ***:Our modified technique is simple and safe in *** data suggest that this technique produces excellent early and medium-term results.

Primary liver cancer (PLC) is one of the most common neoplasms and the second cancer-related cause of death in china, respresenting a major health problem. In china, PLC resulted in 20.40 deaths per lO0 000 per year,with 19.98 per 100 000 in cities and 23.59 per 100 000 inrural areas. Of all the newly enrolled PLC cases in the world each year, about 43% are found in the mainland of china.

CYP2E1 Pst Ⅰ/Rsa Ⅰ polymorphism and colorectal cancer risk:A meta-analysis

AIM:To clarify the association between CYP2E1 PstⅠ/RsaⅠ polymorphism and susceptibility to colorectal ***:A meta-analysis based on 10 eligible casecontrol studies involving 4979 cases and 6012 controls was carried out to summarize the data on the association between CYP2E1 RsaⅠ/PstⅠ polymorphism and colorectal cancer ***:In comparison of the homozygote c2c2 and c2 carriers(c1c2 + c2c2) and the homozygous wild-type genotype(c1c1),no association was found between CYP2E1 RsaⅠ/PstⅠ polymorphism and colorectal cancer risk [odds ratio(OR)=1.24(95% CI:0.93-1.66) for c2c2;OR = 1.02(95% CI:0.88-1.19) for c2 carriers].In stratif ied analysis,Caucasians with c2c2 homozygote appeared to have an increased risk of colorectal cancer(OR=2.67,95% CI:1.03-6.89,P=0.043),no signif icant associations were found in other ***:c2c2 homozygote of CYP2E1 PstⅠ/RsaⅠ polymorphism may be associated with the increased risk of colorectal cancer in Caucasians,which needs further investigations.

Percutaneous cryoablation after chemoembolization of liver carcinoma: report of 34 cases

OBJECTIVE: To assess the indications, efficacy and clinical significance of percutaneous cryoablation for liver carcinoma after transcatheter liver artery chemoembolization (TACE). METHODS: Thirty-four patients with histologically or clinically confirmed primary or metastatic carcinomas were treated with TACE. One week to 1 month later, they were treated percutaneously under ultrasound guidance using cryosurgical system in the period of July 2001 -June 2002. All patients were followed up to determine serum tumor marker, CT scans, MRI images or ultrasound images. RESULTS: This therapy was performed in 34 patients including 32 patients with Child A liver reserve, 2 patients with Child B and no patient with Child C. There were 28 patients with primary liver cancer and 6 patients with metastatic liver cancer. During the follow-up period (3 to 15 months), 41.1% patients were recognized clinically cured because the serum tumor markers became normal, or CT scans and MRI images revealed that the lesion became completely necrotic. 44.1% patients were recognized effectively treated. CONCLUSIONS: Percutaneous cryoablation combined with TACE is a choice of treatment for liver carcinoma. It is minimally invasive, safe and effective for those patients with liver cancer unsuitable for surgery.
