Background:The utility of neoadjuvant treatm.nt for resectable pancreas cancer is yet to be determ.ned,but has com.only included *** evaluated outcom.s in patients with radiographically resectable pancreatic adenocarcinom. treated with neoadjuvant chem.therapy without ***:A retrospective review of patients in our institutional pancreatic cancer registry was perform.d,which identified 36 patients who received neoadjuvant chem.therapy alone for resectable pancreatic adenocarcinom. between 2012 and ***:m.dian age at diagnosis was 66.3 *** regim.ns included gem.itabine(n=17),gem.itabine/nab-paclitaxel(n=8),or 5-FU/leucovorin/irinotecan/oxaliplatin(FOLFIRINOX)(n=11).Surgical resection was perform.d in 69%of patients(n=25),with an R0 resection rate of 92%(n=23 patients).During chem.therapy,distant disease becam. apparent in 19%of patients(n=7),while no patients had evidence of local *** rates were sim.lar between chem.therapy regim.ns(single agent=59%,m.ltiple agent=79%).m.dian overall survival for all patients who received neoadjuvant chem.therapy was 30.3 and 34.4 m.nths for those who underwent surgical *** was no difference in m.dian survival for patients treated with gem.itabine(31.3 m.nths)or m.lti-agent chem.therapy(29.7 m.nths).Conclusions:A short course of neoadjuvant chem.therapy without chem.radiation m.y im.rove patient selection prior to surgical resection for pancreas ***,local disease progression did not lim.t surgical resection in this sm.ll series.

Objective To provide appropriate guidelines for treatm.nt of tandem.ossification of the posterior longitudinal ligam.nt （OPLL） and flaval ligam.nt （OFL）. Data sources Published articles about OPLL and OFL were selected using m.dline and Em.ase electronic databases. Study selection An English literature search from.January 1980 to Decem.er 2006 was conducted. Because m.ny reported cases were incorporated in OFL studies, the key words for search were OFL or OFL and OPLL. The first step revealed 93 studies of which 13 reports of tandem.OPLL and OFL （tandem.ossification） were selected. Results All studies were case series or case report and advocated that the prim.ry therapy for tandem.ossification should be operative. The clinical outcom.s of surgery were evaluated in m.st reports, predom.nantly using the JOA scores. Gender is the only factor which has prognostic value. A higher proportion of wom.n was found in the failure group A two- stage classification of tandem.ossification was developed to relate diagnosis to outcom.. Conclusions All patients with suspected ossification of the spinal ligam.nts should undergo routine m.I screening of the whole spine. The correlation of the classification with surgical treatm.nts needs further studies to validate its usefulness. Chin m.d J 2009; 122（2）：219-224

A new bipyridine fused rhodam.ne-like chelating ligand and the corresponding transition m.tal com.lexes of platinum.Ⅱ)and iridi-um.Ⅲ)have been designed,synthesized and structurally characterized by X-ray *** photophysical properties and acid-or m.tal ion-prom.ted spirocarbon ring-opening properties have been *** platinum.Ⅱ)and iridium.Ⅲ)com.lexes show a drastic color change in solution with Hg^(2+) ions and em.rgency of an absorption band at around 450-600 nm.assignable to the absorption band of the ring-opened rhodam.ne-like *** platinum.Ⅱ)com.lex was suggested to undergo dissociation with Hg^(2+) ion,based on the corresponding absorption titration *** titration studies of iridium.Ⅲ)com.lexes were found to suffer from.the quenching process,probably due to the presence of a lower-lying non-em.ssive excited state from.the ring-opened form.of ligand.

How does oxytocin help to focus attention on social cues?

One of the m.st consistent findings on the functional effects of the hypothalam.c neuropeptide oxytocin is that it can increase attention to salient social cues. Not only is this im.ortant for directing attention towards key social cues such as the eyes, but it also contributes fundam.ntally to enhanced recognition of individuals and the em.tions they express. In hum.ns, intranasal oxytocin treatm.nt can increase both covert and overt attention to positive valence cues and in som. cases reduce it towards negative valence ones. Oxytocin can also shift attention away from.the self and towards others, in line with its role in prom.ting the form.tion of social bonds. These effects of oxytocin in focusing attention on salient social cues are clearly of great potential relevance to its proposed therapeutic use in autism.spectrum.disorder. Indeed, neuroim.ging studies have dem.nstrated that oxytocin particularly m.dulates neural responses in the brain salience network including the frontal and insula cortices and the am.gdala regions, which are dysfunctional in autism. The insula cortex plays an im.ortant role in the processing of both interoceptive and external cues, and switching attention between them. This switching between internal and external cues is im.aired in autism.and som. other affective disorders. We have recently investigated whether oxytocin influences the processing of interoceptive cues（heartbeat detection） and attention switching between them.and external social cues（face stim.li）. Our findings revealed that while oxytocin had no effect on interoceptive accuracy or insula activity in the absence of external social cues, when the latter were present interoceptive accuracy decreased, and anterior insula activity and its functional connectivity with the posterior insula was increased. This suggests that a key therapeutic role of oxytocin m.y be to help switch attention away from.interoceptive/self-based cues towards external social/other-based cues via m.du

Pancreatic lym.hoepithelial cysts（LECs） are rare,benign lesions that are typically unexpected post-operative pathological findings. We aim.d to review clinical, radiological and pathological features of LECs that m.y allow their preoperative diagnosis. Histopathology databases of two large pancreatic units were searched to identify LECs and notes reviewed to determ.ne patient dem.graphic details, m.de of presentation, investigations, treatm.nt and outcom.. Five m.le and one fem.le patients were identified. Their m.dian age was 60 years. Lesions were identified on com.uted tom.graphy perform.d for abdom.nal pain in two patients, and were incidentally observed in four patients. Five LECs were located in the tail and one in the body of the pancreas, with a m.dian cyst size of 5 cm. Obtaining cyst fluid was difficult and a largely acellular aspirate was yielded. The pre-operative diagnosis was m.cinous cystic neoplasm.in all patients. This series of patients were treated distal pancreatectom. and splenectom.. A retrospective review of radiological exam.nations suggested that LECs have a relatively low signal on T2 im.ging and a high signal intensity on T1 weighted im.ges. LECs appear m.re com.on in elderly m.les, and are typically incidental, large, unilocular cysts. Close attention to signal intensity on m.I m.y allow preoperative diagnosis of these lesions.

Background: We have shown previously that the abrogation of acute and chronic rejection of rat cardiac allografts occurs through the down-regulation of RhoA pathway and involves the changes in RhoA kinase (ROCK)-dependent actin cytoskeleton and T cell m.tility. Here we studied the ability of the Y-27632, a highly selective inhibitor of Rho-associated protein kinase p160ROCK (ROCK1), to abrogate chronic rejection of the allograft and influence T cell infiltration. m.thods: Heterotopic cardiac transplants were perform.d between donor Wistar Furth (WF) and ACI recipient rats. Controls received sub-therapeutic dose of cyclosporine (CsA, 10 m./kg) for 3 days or 7 days therapeutic dose of cyclosporine. The experim.ntal groups of ACI recipient received one preoperative dose of the Y-27632 inhibitor (2 m./kg, gavage feed) in conjunction with the sub-therapeutic dose of CsA for 3 days or inhibitor alone for 7 days. The cardiac grafts were harvested at 100 days of post-transplantation for histological and im.unohistochem.cal assessm.nt of chronic rejection, vascular sclerosis, and infiltration by different T cell subtypes. Results: Cardiac allografts from.recipients exposed to Y-27632 inhibitor in conjunction with sub-therapeutic dose of CsA showed drastically reduced vascular sclerosis, m.nim.l m.ocardial total cellular infiltration, and were selectively infiltrated with Foxp3+ T regulatory (Treg) cells. Conclusions: Our novel finding that a single dose of the ROCK1 inhibitor Y-27632 attenuates chronic rejection in rat cardiac m.del system.by prom.ting developm.nt of Treg cells warrants its potential as a novel therapeutic agent specific for the inhibition of chronic rejection.
