Purpose. To compare.the.strate.y for mild stimulation IVF ve.sus conve.tional stimulation IVF. Me.hods: A de.ision tre. analytic mode. was cre.te. to compare.IVF with mild stimulation ve.sus conve.tion stimulation in infe.tile.wome. e.rs of age.to e.aluate.which strate.y is the.le.st costly pe. live.birth. Re.ults: Using base.case.e.timate. of costs and probabilitie. in wome. e.rs old with une.plaine. infe.tility, the.cumulative.live.birth rate.in the.mild ve.sus conve.tional stimulation group was 15.8% ve.sus 28.6%, re.pe.tive.y. The.ave.age.cost for mild and conve.tion IVF was $8789 and $14,364 pe. arm, re.pe.tive.y. In base.case.analysis, the.ICe. was $43,516 re.re.e.ting the.additional cost pe. live.birth with conve.tional IVF. One.way se.sitivity analysis e.aluate. the.robustne.s of the.data and re.e.le. a trade.ff in which mild IVF stimulation had lowe. total costs and also lowe. live.births ve.sus conve.tional stimulation. Conclusions: Mild stimulation IVF has a lowe. cost pe. cycle.but is also associate. with lowe. live.birth rate.. Patie.t care.de.isions should be.individualize. irre.pe.tive.of cost.

The.purpose.of this study was to e.amine.four urovirule.ce.factors (UVFs) and the.r phyloge.e.ic classe. and compare.the.r le.e.s of re.istance.to antimicrobial drug classe. in 159 canine.uropathoge.ic e. coli (UPe.) isolate. submitte. to the.Clinical bacte.iology and Mycology Laboratory of The.Unive.sity of Te.ne.se. Ve.e.inary Me.ical Ce.te. in 2007. Multiple. polyme.ase.chain re.ction was use. to de.e.t the.following UVFs: cytotoxic ne.rotizing factor (cnf), he.olysin (hly), S-fimbrial adhe.ion ge.e.(sfa), and Pilus associate. with pye.one.hritis ge.e.G alle.e.III (pap);polyme.ase.chain re.ction was use. to de.e.mine.phyloge.e.ic group. In vitro susce.tibility to antimicrobial classe. was e.aluate., and re.istance.was compare. to UVF pre.e.ce.as we.l as phyloge.e.ic class. UVFs we.e.pre.e.te. in 48% of UPe. and we.e.ne.ative.y corre.ate. with antimicrobial re.istance. Of the.159 sample., the.numbe. of UVFs e.pre.se. pe. isolate.was 0 = 82 (52%), 1 = 24 (15%), 2 = 2 (1%), 3 = 18 (11%), and 4 = 33 (21%). The.following UVFs we.e.e.pre.se.: sfa (33%), hly (24%), cnf (25%), and pap (18%). Horse., like.all animals, are.born without the.symbiotic microbe. that occupy the.gastrointe.tinal tracts of mature.animals. As grazing animals, horse. re.y on the.e.microbe. to fully utilize.the.grasse. and othe. ce.lulosic fe.ds that the. consume. Thus, colonization of the.foal's gastrointe.tinal tract must occur be.we.n birth and we.ning. The.fe.e. of nine.mare.and foal pairs we.e.sample. from the.day of parturition until 12 we.ks of age. and the.sample. we.e.analyze. by polyme.ase.chain re.ction amplification of the.bacte.ial 16S rRNA ge.e.and de.aturing gradie.t ge. e.e.trophore.is (PCR-DGGe.. The.ge.s from fe.e. of day (d) 0 foals had no or ve.y fe. ( x = 3, n = 6) bands, which indicate. that spe.ie. richne.s was low. The.numbe. of bands incre.se. during the.first 4 days of life. and by d 14 the.foals and mare. had similar numbe.s of bands ( x = 28, n = 23). Some.bands we.e.pre.e.t in young foals, but not in mare. or in foals on d 42 or d 84, which indicate. succe.sion of bacte.ial spe.ie.. Whe. the.PCR-DGGe.profile. we.e.compare. with Dice.s algorithm, all mare.foal pairwise.similaritie. on d 14 and late. we.e.as gre.t as the.pairwise.similaritie. be.we.n mare.. The.e.re.ults are.consiste.t with the.ide. that foals are.born with a ste.ile.gut, colonization proce.ds rapidly, and a mature.microbial community is pre.e.t in the.first fe. we.ks of life.

美国家庭医学小组于2000年10月在夏延山召开了Ke.stone.大会。与会者包括第一代(1970年之前进入,代表专业的创始人),第二代(1970—1990年进入,代表积极建立和推进这一领域的人员)及第三代家庭医学成员(1990年以后进入,代表当时最新的家庭医生)。会议期间,三代成员一起讨论思考过去、现在及家庭医学的未来,我们在其中的角色,以及如何理解现有的及发展中的家庭医生和家庭医学。历经10多年的发展后,由10名第三代成员撰写了本文,反映了他们目前的观点。本刊编辑部特翻译此篇文章,希望对中国的全科医生也有一定的启发和帮助。

The.Gre.t Plains re.ion of the.Unite. State. is susce.tible.to drought of all kinds including me.e.rological/climatological, agricultural, hydrological, and socioe.onomic. Drought conditions in the.re.ion span varying spatial and te.poral scale. and the.cause. include. 1) ce.tain synoptic conditions that favor drought such as mid-troposphe.ic ridging ove. the.drought-affe.te. are. and a we.k low-le.e. je.;2) se. surface.te.pe.ature.anomalie. and associate. te.e.onne.tions;3) land-atmosphe.e.coupling;and 4) anthropoge.ic e.fe.ts. While.drought can span as fe. as a couple.of months, the.most se.e.e.droughts can occur at the.de.adal scale.such as the.1930s Dust Bowl, the.worst drought in re.e.t history from a socie.al standpoint. Such droughts in the.Gre.t Plains have.wide.pre.d impacts on agriculture. wate. re.ource., human he.lth, and the.e.onomy.
