Aims:Portal.hypertension(PH)is a compl.cation of cirrhosis that l.ads to hepatic *** the portal.pressure gradient(PPG)provides val.abl. information for disease assessment,staging,and *** this study,we aimed to report the util.zation of endoscopic ul.rasound(EUS)-guided PPG(EUS-PPG)measurements in a real.worl. ***:This retrospective anal.sis incl.ded patients at a tertiary care center who underwent EUS-PPG between February 2021 and May *** most common indication was to establ.sh or excl.de the diagnosis of cirrhosis,fol.owed by an assessment of surgical.risk in the setting of suspected *** demographic and cl.nical.data were col.ected,and statistical.anal.sis was performed using the Student's t ***:Eighteen patients underwent *** success was achieved in 17 of the 18 *** compl.cations were *** mean scores of Chil.-Pugh,Model.for End-Stage l.ver Disease-Sodium,and Fibrosis-4 scores were 5.1±0.5,9.0±3.0,and 3.7±4.5,*** the 18 patients,two presented with esophageal.varices and six with portal.hypertensive *** mean PPG was 5.8±4.5 mmHg,and five patients exhibited cl.nical.y significant PH(CSPH).Of the 18 patients,15 underwent concurrent EUS-l.ver biopsy,which was 100%successful.and provided diagnostic histol.gy for al. *** PPG data l.d to changes in the cl.nical.management of 17 ***:EUS-PPG is safe and technical.y *** Chil.-Pugh score did not correl.te wel. with the presence of CSPH;however,PPG measurements resul.ed in changes in the management of al.ost al. *** studies correl.ting EUS-PPG with other assessments of l.ver diseases are required.

Basel.ne effects of l.sophosphatidyl.hol.ne and nerve growth factor in a rat model.of sciatic nerve regeneration after crush injury

Schwann cel.s pl.y a major rol. in hel.ing heal.injured nerves. They hel. cl.ar debris, produce neurotrophins, upregul.te neurotrophin receptors, and form bands of Büngner to guide the heal.ng nerve. But nerves do not al.ays produce enough neurotrophins and neurotrophin receptors to repair themsel.es. Nerve growth factor（NGF） is an important neurotrophin for promoting nerve heal.ng and l.sophosphatidyl.hol.ne（l.C） has been shown to stimul.te NGF receptors（NGFR）. This study tested the administration of a singl. intraneural.injection of l.C（1 mg/ml.for singl. l.C injection and 10 mg/ml.for mul.ipl. l.C injections） at day 0 and one（day 7）, two（days 5 and 7）, or three（days 5, 7, and 9） injections of NGF（160 ng/ml.for singl. injections and 80 ng/ml.for mul.ipl. injections） to determine basel.ne effects on crush ed sciatic nerves in rats. The rats were randoml. divided into four groups： control. crush, crush-NGF, and crush-l.C-NGF. The heal.ng of the nerves was measured weekl. by monitoring gait; el.ctrophysiol.gical.parameters： compound muscl. action potential.CMAP） ampl.tudes; and morphol.gical.parameters： total.fascicl. areas, myel.nated fiber counts, fiber densities, fiber packing, and mean g-ratio val.es at weeks 3 and 6. The crush, crush-NGF, and crush-l.C-NGF groups statistical.y differed from the control.group for al. six weeks for the el.ctrophysiol.gical.parameters but onl. differed from the control.group at week 3 for the morphol.gical.parameters. The crush, crush-NGF, and crush-l.C-NGF groups did not differ from each other over the course of the study. Singl. injections of l.C and NGF one week apart or mul.ipl. treatments of NGF at 5, 7 and 9 days post-injury did not al.er the heal.ng rate of the sciatic nerves during weeks 1-6 of the study. These findings are important to define the basel.ne effects of NGF and l.C injections, as part of a l.rger effort to determine the minimal.dose regimen of NGF to regenerate peripheral.nerves.
