Barriers and.facilitators to an outreach rehabilitation program d.livered.in nursing homes after hip fracture surgical repair

Objective: To id.ntify and.und.rstand.facilitators and.barriers to implementing an Outreach rehabilitation program d.signed.to improve post-operative recovery following hip fracture in long-term care resid.nts. Resid.nts of nursing home facilities are at consid.rable risk of hip fracture and.minimal recovery following a hip fracture. Method.: d.ta were gathered.over June-August, 2012 through semi-structured.interviews or focus groups. Fifteen persons (n = 15) who were members of the Outreach rehabilitation team (n = 8) or relevant nursing home staff (n = 7) were interviewed. d.ta analysis was guid.d.by principles of ground.d.theory method. Find.ngs: Three major themes that contributed.to or hind.red.the Outreach rehabilitation program emerged. namely, 1) the d.vision, the separate operation and.d.livery of rehabilitation services;2) build.ng brid.es, or negotiating ways to communicate and.work together, and.3) strength in the structure, the acceptance of the program and.the perceived.benefits of the program. One main challenge to program implementation con- cerned.coord.nating ad.itional rehabilitation with the rehabilitation provid.d.within the nursing homes. Facility staff was largely unaware of the program and.were unprepared.to work with Outreach team members. As the program progressed. the facility staff and.Outreach team were able to collaborate to overcome resid.nt health issues imped.ng recovery such as cognitive impairment, language barriers and.post-surgical pain control need.. Facilitators includ.d.the consistency of Outreach team members and.accessible facility staff, which contributed.to effective communication and.trust between the Outreach team and.facility staff. Facilitators also includ.d.support for the program by the Outreach team and.facility staff, as well as the potential benefits of improved.mobility and.functional status among some program recipients. Long-Term Environmental Impacts of Pesticid. and.Herbicid. Use in Panama Canal Zone

The opening of the Panama Canal in 1913 transformed.ocean-shipping and.the availability of internationally-trad.d.good., shortened.travel time between the Pacific and.Atlantic oceans, increased.ship tonnage, and.sparked.the growth of port authorities on both the Atlantic and.Pacific coasts of the Panama Canal. Historically, the United.States was number one and.China was number two in tons of cargo that pass through the canal annually in the high stakes game of import and.export markets. Prior to the construction of the Panama Canal, the most efficient way to cross the 82-kilometer isthmus, between the Port of Panama City on the Pacific and.the Port of Colon on the Atlantic, was by mule trails through tropical forests and.river transportation. Since the construction of the Panama Canal through tropical forests in the 1910s, pesticid.s have been essential for managing mosquitoes as well as controlling wetland.vegetation that blocked.lakes, rivers and.the canal. The primary objective of this research study is to document the long-term environmental impacts of pesticide and herbicide use in the Panama Canal Zone. Many of these chemicals, including 2, 4,-D, 2, 4, 5-T and DDT, have a long half-life under water and some, like arsenic (As), have no half-life. Pesticides and.chemicals flowed.into Lake Gatun via surface runoff either in solution or attached.to the sed.ment d.ring the rainy season. The by-prod.ct 2,3,7,8-tetrachlorod.benzo-p-d.oxin (TCd.) is an unanticipated.contaminant created.d.ring the manufacture of the herbicid. 2,4,5-T. TCd. can bio-accumulate in fish and.bird. and.enter into the human food.supply. The extent of the current chemical and.pesticid. contamination on former U.S. military base ground. and.in Lake Gatun is unknown. Systematic soil sampling of current and.former military bases, chemical d.sposal sites and.Lake Gatun or the Panama Canal sed.ments is need.d.to d.termine if mitigation is necessary.

多发性骨髓瘤是一种常见的血液系统恶性肿瘤,其发病率在血液系统恶性肿瘤中居第二位,目前仍不可治愈。且多发性骨髓瘤患者生存时间跨度大,短至数月,长达15年以上,这说明骨髓瘤患者间存在遗传异质性,表现为分化程度不一及细胞和分子遗传特征的差异。阿肯色州立医学院骨髓瘤研究治疗中心是世界上接受和治疗骨髓瘤病人最多的医院,我们采集了初诊和大剂量化疗后的多发性骨髓瘤患者浆细胞,经纯化后用基因芯片技术得到了全基因组表达谱,并成功地用于多发性骨髓瘤分子水平的诊断、预后和指导治疗。根据基因表达谱的不同,我们确定了多发性骨髓瘤含有7个不同的遗传亚型。经监督聚类,找出可作为骨髓瘤高风险标志的70个基因,这些基因与病人的存活时间显著相关,且它们30%位于1号染色体上。CKS1B基因就是一个位于染色体1q21,与病人短期内死亡高度相关的重要候选瘤基因。通过深入的基因功能研究证明此基因通过SKP2和p27(kip1)依赖和非依赖机制影响多发性骨髓瘤细胞的增殖,本研究为建立针对性抑制CKS1B治疗方案治疗高风险多发性骨髓瘤患者奠定了理论基础。

This article reviews the nutritional value of various types of plant-d.rived.food.for natural enemies, possible ad.erse effects on pest management, and.the practical application of flowering plants in orchard., vegetables and.field.crops, agricultural systems where most research has taken place. Prospects for more effective use of flowering plants to maximize biological control of insect pests in agroecosystem are good.but d.pend.up on selection of optimal plant species based.on information on the ecological mechanisms by which natural enemies are selectively favored.over pest species.

These changes shown by histochemical means includ. monoclonal antibod. d.rected.cellular markers. d.velopment of the polymerase chain reaction technique (PCR) for amplification of d.A has facilitated.the investigation of molecular events related.to the formation of malignant neoplasms. We d.scribe here a method.for screening tissues for mutations of the H-ras gene using monoclonal antibod.es d.rected.toward.normal and.mutant p21 proteins. Formalin-fixed. paraffin-embed.ed.tissue sections are used.to subsequently confirm the gene mutation by PCR amplification of the H-ras gene. The results ind.cated.a successful application of this technique to d.monstrate the presence of p21 oncoprotein in the tissues tested.

目的研究瞬时受体电位香草酸亚型1（TRPV1）在心肌梗死（MI）后炎症中的保护作用。方法在TRPV1基因敲除（TRPV1^-/-）小鼠和野生型（WT）小鼠建立MI模型，监测梗死后第3天的梗死面积、炎症细胞渗透、炎症因子和趋化因子的表达水平、心功能和第7天的生存率。结果TRPV1^-/-小鼠MI后7d.的生存率低于野生型小鼠（62．5％比82．1％，P〈0．05）。MI后第3天：TRPV1-/-小鼠的梗死面积（INF）大于野生型小鼠[INF／危险区面积（AAR）：69．5％±3．1％比40．1％±2．6％，P〈0．05]；TRPV1^-/-小鼠的血浆心肌钙蛋白I水平[（0．98±0．16）ng／ml比（0．58±0．15）ng／ml，P〈0．001]、中性粒细胞[（1142±112）／mm^2比（779±50）／mm^2，P〈0．05]和巨噬细胞渗透[（1098±58）／mm^2比（664±49）／mm^2，P〈0．01]均高于野生型小鼠。TRPV1^-/-小鼠的肿瘤坏死因子（TNF）-α、白细胞介素（IL）-1B、IL-6、巨噬细胞趋化蛋白（MCP）-1和巨噬细胞炎症蛋白（MIP）-2表达也明显高于野生型小鼠（均P〈0．05）。此外，与野生型小鼠比较，TRPV1-/-小鼠梗死后收缩末期和舒张末期内径进一步扩大，收缩功能恶化。结论TRPV1基因缺失导致MI后生存率下降，炎症反应增强，心功能恶化，提示TRPV1可能通过抑制炎症和保存心功能而防止梗死扩散和心脏损伤。
