目的:探讨开角型青光眼患者局部应用拉坦前列素滴眼液6mo后(0.005%,一日一次),眼压(IOP)、角膜中央厚度(CCT)和前房深度(ACD)的变化。方法:本研究包含初诊为原发性开角型青光眼(POAG)或剥脱性青光眼(PXG)患者24例37眼。采用Goldmann压平眼压计测量IOP,超声测厚仪测量CCT,超声生物测量仪测量ACD和ACD/轴长(AL)。比较治疗前,记录治疗后3mo和6mo的IOP,CCT,ACD和ACD/AL测量值。结果:IOP于治疗前,治疗后3mo、6mo平均值分别为25.0±4.2、17.5±2.0、16.9±1.7,可见治疗后显著降低。CCT于治疗前,治疗后3mo、6mo平均值分别为546.6±31.5,541.0±29.4、542.2±29.3,可见治疗后显著降低。ACD于治疗前,治疗后3mo、6mo平均值分别为3.00±0.43,2.95±0.42、2.97±0.41,可见治疗后3mo显著降低,6mo无显著改变。ACD/AL治疗后改变与ACD情况相似。CCT和ACD的测量值在POAG中有显著变化,而在PXG中则没有。结论:开角型青光眼患者应用拉坦前列素滴眼液治疗后,除IOP大幅降低外,角膜厚度及ACD亦出现短期内降低。

Diaphragmatic Plication: Retrospective Study with 54 Patients

Objective: This study seeks to evaluate the results of surgery performed according to the etiological causes of diaphragmatic eventration or paralysis. Method: Files of 54 patients (35 males, 19 females;mean age: 39.1 ± 3.01 years) who had undergone diaphragm plication between January 2005 and June 2011 in two clinics located in Istanbul were studied in terms of pulmonary spirometry, applied surgical procedure, duration of hospitalization, morbidity, and mortality. Results: In diaphragm plications, observed etiologies were isolated diaphragmatic eventration without phrenic nerve injury (Group 1) in 20 (37%) patients due to blunt thoracic trauma;Group 2 (thymoma in 19 (35.2%) patients;lung cancer in 7 (12.9%) patients;mediastinal tumor in 3 (5.6%) patients) and congenital heart surgery (Group 3) in 5 (9.3%) patients. The mean time to plication was 7.4 months in Group 1. The mean duration of hospitalization was 4 days in Group 1, 6.2 days in Group 2, 11.8 days in Group 3. Mean forced vital capacity, forced expiratory volume at 1 second improved by 17%, 20.3%, respectively (p In Group 1, mean dyspnea-related hospital admission before plication was 2.4 times. At the end of 6 months after plication mean dyspnea-related hospital admission was 0 time. All of them returned to daily activities or their job within 6 months in Group 1. Postoperative mortality was observed in 2 (3.7%) patients in Group 3 while the overall complication rates were 24%. The morbidity rates were 8%, 27.5%, 60%, respectively;and the mortality rates were 0%, 0%, 40%, respectively. Conclusion: As a result, it could be suggested that early and timely performed plications for diaphragmatic eventration improves functional status and assesses a shorter lenght of stay. On the other hand,simultaneously performed plications in intraoperative phrenic nerve resections can be performed with acceptable morbidity and no mortality. Collection of Dialysis Treatment Data and Patient-Related Information in a Single Dialysis UnitState-of-the-art dialysis machines are capable of automatically collecting huge volumes of dialysis treatment data (blood and dialysate flow rates, venous and arterial pressures, ultrafiltration rate, ultrafiltration volume, transmembrane pressure, conductivity, etc.) and displaying the information on the monitor screen. Commercially available software allows this information to be exported to a clinical database on a server through a local network in the clinic, enabling healthcare professionals to further process, display, and evaluate the data. Moreover, on-line sensors can be used to monitor patient-related information, *** ionic dialysance as a marker of urea clearance[1-3], urea removal and additional parameters(e.g. whole-body Kt/V) based on dialysate urea measurements[4,5], and blood volume changes[6]. Although many of these sensors are available in dialysis machines, their clinical value in routine dialysis is still subject to debate[7].

Melatonin reduces bacterial translocation and apoptosis in trinitrobenzene sulphonic acid-induced colitis of rats

AIM: To investigate the effects of exogenous melatonin on bacterial translocation and apoptosis in a rat ulcerati-ve colitis model. METHODS: Rats were randomly assigned to three groups: groupⅠ: control, group Ⅱ: experimental colitis, group Ⅲ: colitis plus melatonin treatment. On d 11 after colitis, plasma tumor necrosis factor-α, portal blood endotoxin levels, colon tissue myeloperoxidase and caspase-3 activity were measured. Bacterial translocation was quantified by blood, lymph node, liver and spleen culture. RESULTS: We observed a significantly reduced inciden-ce of bacterial translocation to the liver, spleen, mesen-teric lymph nodes, portal and systemic blood in animals treated with melatonin. Treatment with melatonin signifi-cantly decreased the caspase-3 activity in colonic tissues compared to that in trinitrobenzene sulphonic acid-trea-ted rats (16.11 ± 2.46 vs 32.97 ± 3.91, P < 0.01). CONCLUSION: Melatonin has a protective effect on ba-cterial translocation and apoptosis.

Esthetic and Functional Rehabilitation of Amelogenesis Imperfecta: Report of Four Cases with a One-Year Follow-Up

In this report, we describe the performance of a conservative and minimally invasive dental approach in four patients exhibiting Amelogenesis Imperfecta (AI), a structural anomaly of the enamel. In each patient, approximately 0.5 mm of the most external, porous, and colored enamel layer was removed, and the teeth were restored using two different nanocomposites. Posterior restorations were completed with the same approach. As a result, this contemporary restorative system is a conservative and successful treatment option to restore the loss of oral esthetics and function due to AI. Rehabilitation with direct resin restorations is not only an inexpensive treatment choice, but also a more conservative technique that reduces the amount of preparation required for teeth that are already compromised.

目的:探讨玻璃体内注射贝伐单抗对伴有黄斑水肿的糖尿病患者眼血流参数的影响。方法:患者21例21眼,伴有弥漫性糖尿病性黄斑水肿(diabeticmacular edema,DME),黄斑中心厚度(central macularthickness,CMT)>320μm,无缺血性黄斑水肿或其他血管性视网膜疾病。患者术前以及注射1.25mg/0.05mL贝伐单抗1d,4wk后接受三种眼科检查:视力(visual acuity,VA)检测、光学相干断层扫描(optical coherence tomography,OCT)测量CMT以及采用超声成像测量眼动脉(ophthalmicartery,OA)、视网膜中央动脉(central retinal artery,CRA)、鼻侧睫状后动脉(nasal posterior ciliary artery,NPCA)以及颞侧睫状后动脉(temporal posterior ciliary artery,TPCA)中的收缩期峰值血流速度(peak systolic velocities,PSV)和舒张末期血流速度(end-diastolic velocities,EDV)。阻力指数(resistive indices,RI)由软件自动计算,注射前与注射后1d的血流速度进行了比较。患者术前和注射后4wk的VA和CMT值进行了比较。结果:注射前平均最佳矫正视力(best-corrected visual acuity,BCVA)为0.88±0.21logMAR,注射后为0.54±0.19logMAR(P<0.01)。平均CMT由注射前440.57±54.58μm下降至250.33±31.12μm(-190.24±36.00μm)。PSV、EDV和RI的变化并不显著。结论:贝伐单抗注射后视力显着改善,CMT降低,而PSV、EDV和RI在OA、CRA、TPCA和NPCA中没有显著变化。玻璃体内注射贝伐单抗注射液能改善VA和CMT,但对于糖尿病患者的OA、CRA、TPCA和NPCA的血流速度没有影响。

An interesting cause of esophageal ulcer etiology: Multiple myeloma of IgG kappa subtype

Multiple myeloma is a neoplasm of mature and immature plasma cells. A 50-year-old woman with lumbago, dysphagia, and left arm pain was presented. Upper endoscopical examination was performed. There was an exudate-covered ulcer in the distal esophagus, located at 30-32 cm from the incisors, covering the whole mucosa. Histopathological examination of the specimens obtained from the lesion showed the involvement of plasma cells consistent with multiple myeloma of IgG kappa subtype. Esophageal involvement of multiple myeloma should be kept in mind in patients presenting with dysphagia.
