Objective. Acid secretion produced by a heterotopic gastric mucosal patch (HGMP) in the proximal esophagus, instead of gastric acid, may be responsible for laryngopharyngeal reflux (LPR), passing the upper esophageal sphincter. The aim of this study was to investigate the prevalence of HGMP in the proximal esophagus in patients with posterior laryngitis indicating the presenc e of LPR in comparison with a control group and to elucidate the possible role o f this lesion in the pathogenesis of LPR. Material and methods. A total of 36 co nsecutive patients with posterior laryngitis diagnosed on laryngoscopic examinat ion were enrolled in the study. Esophagoscopy and ambulatory 24 h intra-esophag eal dual-probe pH monitoring were performed in all patients. During endoscopy, special attention was paid to the proximal part of the esophagus, and the proxim al electrode for pH monitoring was placed in this region under endoscopic view. The control group comprised 660 consecutive patients who had undergone upper gas trointestinal endoscopy for the usual indications. When HGMP was found, biopsies were taken for histological confirmation. Results. HGMP was detected in 5 out o f 36 patients. One out of five patients with patches was excluded from the study because the histopathology of this patient‘s patch revealed antral-type mucos a, which is not capable of acid secretion. Thus a total of 35 patients were incl uded in the study, yielding a HGMP prevalence of 11.4%(4/35). Measurement of circulating levels of VEGF-A,-C,and -D and their receptors,VEGFR-1 and -2 in gastric adenocarcinoma

AIM: To analyze the serum levels and prognostic significance of vascular endothelial growth factor (VEGF) -A,-C,and -D,and their receptors,VEGFR-1 and -2 in gastric adenocarcinomas. METHODS: The serum levels of VEGF family members were measured in 76 control subjects and 76 patients with gastric adenocarcinoma using an enzyme-linked immunosorbent assay (ELISA). These measurements were correlated with clinco-pathological features and survival rates. RESULTS: The serum levels of VEGF-A and its receptor,VEGFR-1,were signifi cantly higher in patients with gastric cancer than in healthy donors (t = 2.3,P = 0.02 and t = 4.2,P bjects than in patients (t = 2.9,P = 0.004). There was no significant difference in serum levels of VEGF-C and VEGFR-2 between patients and controls. VEGF-C was associated with advanced tumor stage and presence of metastasis. VEGFR-1 was associated with metastasis,advanced overall stage,tumor differentiation and survival. VEGFR-2 levels were associated with poor tumor differentiation. There was no significant prognostic value for any of the VEGF family members or their receptors except for VEGFR-1 where high levels were associated with a poor overall survival. CONCLUSION: Serum VEGF levels vary significantly in the same cohort of patients with variable clinico-pathological features and prognostic values. The simultaneous measurement of VEGF receptors levels in sera may overcome the limitations of a single biomarker assay.

Targeting hepatic ceruloplasmin mitigates nonalcoholic steatohepatitis by modulating bile acid metabolism

Nonalcoholic steatohepatitis(NASH)is a condition that progresses from nonalcoholic fatty liver disease(NAFLD)and is characterized by hepatic fat accumulation,inflammation,and *** has the potential to develop into cirrhosis and liver cancer,and currently no effective pharmacological treatment is *** this study,we investigate the therapeutic potential of targeting ceruloplasmin(Cp),a copper-containing protein predominantly secreted by hepatocytes,for treating *** result show that hepatic Cp is remarkedly upregulated in individuals with NASH and the mouse NASH ***-specific Cp ablation effectively attenuates the onset of dietary-induced NASH by decreasing lipid accumulation,curbing inflammation,mitigating fibrosis,and ameliorating liver *** employing transcriptomics and metabolomics approaches,we have discovered that hepatic deletion of Cp brings about remarkable restoration of bile acid(bA)metabolism during *** deletion of Cp effectively remodels bA metabolism by upregulating Cyp7a1 and Cyp8b1,which subsequently leads to enhanced bA synthesis and notable alterations in bA *** conclusion,our studies elucidate the crucial involvement of Cp in NASH,highlighting its significance as a promising therapeutic target for the treatment of this disease.

AIM:To isolate,characterize and investigate the antifungal activity of the root bark of Psorospermum corymbiferum(Clusiaceae).METHODS:The finely ground root bark of Psorospermum corymbiferum was cold-extracted by percolation with 98% ethanol(5.0 L) for two *** ethanol extract was macerated with n-hexane,chloroform,ethylacetate and methanol successively to give the respective solvent *** of the extract was done using column chromatography on silica gel with cyclohexene:ethylacetate(1:1) as eluents,and the subfractions were purified by TLC using cyclohexane:ethylacetate(7:3).The structures were elucidated based on spectral analysis(IR,1H NMR and 13C NMR) in comparison with the data with those reported in the *** minimum inhibitory concentration(MIC) was determined on Candida alblican and Candida ***:Three known compounds,acetylvismione F,prenylated bianthrone and 1,8-dihydroxyanthraquinone and a steroid were isolated from the *** determined on Candida alblican and *** were 20 and 15 μ***-1,*** steroidal compound(diosgenin) was characterized for the first time from Psorospermum corymbiferum(Clusiaceae).CONCLUSION:The results suggested that the isolation has anticandidal properties,supporting the use of this plant in folk *** current drug of choice for disseminated Candidiasis is amphotericin b with MIC of 0.39 μ***-1,which could be a promising lead for a new antifungal drug especially if subjected to SAR in antifungal drug design.

Children Safety Devices in brazil—Why Do People Don’t Use Them after the Law?

Introduction: Children safety car devices decrease injuries and death in children. A survey conducted in brazil in 2009 revealed that only 36.1% of children safely transported. In 2010, a brazilian law was implemented, obligating the use of safety devices. The aim of this study is to evaluate the use of such devices after the law and the eventual reasons for non-use. Methods: A significant sample of brazilian population aged 18 years or more, which normally carry children until 10 years old answered a survey between August 16 and 25, 2012. The study was conducted in two phases. The first one aimed to raise the proportion and profile of the target population, while the second investigated the children car safety device use (or not). Results: 622 interviews. Children’s transporters are young males (57%), living in a metropolitan area in the South region, concerned about safety and law supervision, with greater education level and income than non-transporters (31%) who are male with lack of information, living in a non-metropolitan area of the Northeast region that would be motivated to use the device by effective law fiscalization or threat of an accident. Conclusion: Two years after the law it is observed that education, income, age, gender and region of origin influence the use/non-use of the safety seats in brazil. The accident threat and fiscalization are the main reasons for using the device, showing that the lack of information impairs the democratization of its use and the prevention culture has to be enhanced in the country.

Antifungal activity of natural compounds against Candida species isolated from HIV-positive patients

Objective: To evaluate the antifungal effect of Cinnamomum zeylanicum(C. zeylanicum) and Melaleuca alternifolia essential oils and honey against strains of Candida sp. from HIV-positive patients in order to subsidize new therapeutic strategies for ***: The study evaluated the antifungal effect of natural antimicrobials against 30 strains of Candida sp. isolated from oral cavities in HIV-infected patients. Then, they were compared to the action of fl uconazole and amphotericin b. Antifungal susceptibility was evaluated by the broth macrodilution technique and the minimum inhibitory concentration and the minimum fungicidal concentration were ***: Among all antifungals evaluated in this study, amphotericin b was the one showing the best results; however, all compounds studied here showed inhibitory activities against isolates of Candida sp. Honey(0.031 3 to 64 μg/m L) demonstrated fungistatic activity inhibiting 70% of the isolates. C. zeylanicum essential oil(0.031 3 to 64 μg/m L) inhibited 93.3% of the Candida strains and Melaleuca alternifolia essential oil(0.031 3 to 64 μg/m L) was able to inhibit 73.3% of ***: Therefore, all natural compounds evaluated in this study, especially C. zeylanicum essential oil, may become promising agents for oral candidiasis therapy including in HIV-positive patients.

Nonalcoholic fatty liver disease in asymptomatic brazilian adolescents

AIM： To evaluate the prevalence and clinical characteristics of Nonalcoholic fatty liver disease （NAFLD） among asymptomatic brazilian adolescents. METHODS： Transversal observational study included asymptomatic adolescents with central obesity from private and public schools in Salvador-bahia, northeastern brazil. The children answered a questionnaire that in- cluded age, gender, race, and medical history, and were submitted to a complete physical exam and abdominal ultrasound. biochemical exams included： alT, AST, GGT, C reactive protein （CRP）, fasting glucose, insulin, cholesterol and triglycerides. Criteria for NAFLD included： the presence of steatosis in ultrasound and/or high level of alT, negative or occasional historic of intake of alcohol （4 140 g/wk）, negative investigation for hepatitis A, b, C, auto-immune hepatitis, Wilson disease and ***： From October, 2005 to October, 2006, the study included 1801 subjects between 11 and 18 years of age and a mean age of 13.7± 2.0 years. One hun- dred ninety-nine had central obesity. The prevalence of NAFLD was 2.3%, most of whom were male and white. Insulin resistance （IR） was observed in 22.9% of them and had positive correlations with alT and GGT （P 〈 0.05）. Elevated CRP was observed in 6.9% of the cases; however, it was not associated with WC, IR or liver enzymes. CONCLUSION： The prevalence of NAFLD in brazilian adolescents was low. The ethnicity may have influence this frequency in the population studied, which had a large proportion of African descendents.
