Effect of topical 0.05% cyclosporine A on corneal endothelium in patients with dry eye disease

· AIM: To determine the effect of topical 0.05% cyclosporine A (CsA) on corneal endothelium in patients with dry eye disease. · METHODS: Observational, prospective, case series study. Fifty-five eyes of 29 consecutive patients (9 males and 20 females; median age: 66.8 years, interquartile range: 61 -73.2 years) with moderate -severe dry eye disease were evaluated. All patients were treated with topical 0.05% CsA ophthalmic emulsion twice a day in addition to lubricant eyedrops 5 times a day. The follow- up period was 12 months. Before treatment and at 3 and 12 months post -treatment central corneal specular microscopy was performed. The endothelial cell density (ECD), coefficient of variation of cell size (CoV), and percentage of hexagonal cells (Hex %) were analyzed. ·RESULTS: The median ECDs pre-treatment and at 3 and 12 months post-treatment were 2 352.5/mm 2 (inter- quartile range, 2 178 -2548.5), 2 364/mm 2 (interquartile range, 2 174.25 -2 657.5), and 2 366 cells/mm 2 (inter - quartile range, 2 174.75-2 539.75), respectively (P=0.927, one way ANOVA). The median CoVs pre-treatment and at 3 and 12 months post -treatment were 34.5 (interquartile range, 30 -37), 35 (interquartile range, 30 -38), and 34 (interquartile range, 30.75-38.25), respectively (P=0.7193, one way ANOVA). The median Hex % values pre - treatment and at 3 and 12 months post -treatment were 53 (interquartile range, 47 -58), 54 (interquartile range, 45.75 -59), and 50.5 (interquartile range, 45.75 -58), respectively (P=0.824, one way ANOVA). · CONCLUSION: Treatment of patients with dry eye disease for 12 months with topical 0.05% CsA does not seem to cause substantial changes on corneal endothelium.

The high activity of metallic magnesium and alloys limits its potential in biomedical applications;in recent years,extensive efforts have been devoted to modulating this *** this work,we present Mg(OH)_(2) and TiO_(2)barrier coatings to reduce the degradation of magnesium alloy(Mg-Ca-Zn)*** coatings were deposited by the anodization method and the spin-coating technique,*** anodized layer was coated with TiO_(2)generated from the hydrolysis of 3%weight of TTIP(Ti[OCH(CH_(3))_(2)]_(4),Titanium(IV)isopropoxide)in 2-Propanol deposited by the spin-coating *** the degradation in Ringer’s solution by electrochemical impedance spectroscopy and OCP revealed a 98%reduction in pittings in uncoated samples after 14 days of *** p H measurements revealed that the TiO_(2)coating reduced the alkalization of the physiological environment,keeping the pH at 6.0 *** vitro studies of two types of bacteria(*** and ***)exhibited zones of inhibition in the agar and activity bactericidal(kill time test).The mechanisms behind the improved degradation resistance and enhanced antibacterial activity are presented and discussed *** modification with Mg(OH)_(2)/TiO_(2)coatings is a promising strategy to control the biodegradation of magnesium implants for bone regeneration.

K-1 Yongquan complementary resuscitation maneuver, systematized since 1987, has been consistently performed in sudden death and cardiac arrest conditions as a final resource in both basic and advanced CPR failure. Experimental analytical studies identify the prevention, control and assessment of treatments set up as well as the determination of their efficiency offering;in that way, there are more possibilities than risks during the trial period. That type of study refers to the random criterion in order to measure differences and, in that way, it establishes causal associations which may better determine the intervention mechanism. As inferred by this criterion, control groups would not profit from the benefit of providing a second chance by means of the proposed maneuver during rescue. Such determination leaves those included in the control group deserted to their own ill-fate, adding—consequently—a certain lethal risk, which should basic and advanced CPR fail. In view of this panorama, we tried to find a methodology that should ensure the validation process according to the model presented;to that end, we propose the Cohort Retrospective Study. All of the above has given evidence that there is a difference if one takes into consideration as control the group of “deceased patients” instead of considering among them “patients that may be deceased”. Such apparently simple consideration—thanks to the Cohort Retrospective model—manages to efface the high possibility of a “fatal damage”, as proposed by the randomness principle upon a prospective non-intervention group. Beyond the scientific methodology that supports it the efficiency of the maneuver derives mainly from the sustained increase in survival rates presented in the successive statistics published since its application.

The large amount of classifications about the concept of death from a medical-legal perspective leads us to think that there are still many aspects to accurately define that precise moment in which the end of our earthly existence is considered as a final and unalterable fact. An answer to such a question may come from a retrospective analysis of those victims of impending-death situations that have been rescued after both basic and advanced cardiac pulmonary resuscitation failure and their consequent medical-legal death declaration. The aim of the following work is to introduce a new phase within forensic thanatology, supported by a complementary resuscitation maneuver based upon millennial traditional Chinese medicine principles together with a detailed analysis of current global agreements on organ transplantation and an avant-garde perspective on actual knowledge about cell death. Those terms will then allow us to achieve a holistic view of said concept, still loosely defined at present. Such an innovative diagnostic-therapeutic resource can in turn enable us to evaluate and face the irreversibility of such extreme situation, analyzing the statistical feasibility of its promising results.

The Theory of Morphic Fields explains how co-evolution through collective information fields acquires its own organization pattern, nurtured by habits or thoughts that “in-form” the memory of species. The influences of electromagnetic fields (EMFs) on bio-energy transport and its mechanism of changes has been investigated through analytic and numerical simulation as well as experimentation. As an answer to that need Therapeutic Acupunctural Resonance is born. This innovative therapeutic is rooted in concepts from classic Physics as well as Field Quantic Theory—highlighting the practical application of the criterion defined by the Soliton theory and the current, extraordinary discoveries at Cellular Quantic Kinetics level. The development of Therapeutic Acupuntural Resonance has taken into account to a certain extent these concepts: both the compatibility between the Five Element Theory and the principles of Euclidean Geometry, as well as the new forms of interpretation of Quantum mechanisms involved in Cellular Kinetics and its relationship with Chinese Ancient wisdom.

Long before the western concept of 'circadian cycle' appeared, the Chinese already knew where and at what time our vital energy ran along our bodies, having in this way a surprising diagnostic as well as therapeutic certainty in their medical treatments. As a kind of 'Retro-Medicine' or 'Inverse Medicine', the author of this work has been researching over decades, under an exhaustive scientific and methodological analysis, the usefulness of certain therapeutics and parameters of Chinese medicine, as a way of clarifying its treatments so as to have such techniques available for the present medicine. Far from being a mere coincidence, the effects of Yin/Yang through the solar light cycles over the meridians of acupuncture, seem to correspond with the subsequent western research, such as those referred to the circadian cycle and their pathophysiological relationship with the myocardial infarction and other severe cardiovascular conditions.

The foundational chronology of Torsadogenic Index is introduced to explain the relationship between impending death situations and drug prescription, combination, self-indication, or abuse of torsadogenic pharmacological products. The pathophysiological basis of Torsade de Pointes is presented with the most frequent causes of Long QT syndrome. Traditional Chinese Medical principles are exposed in order to help people to understand its para-logical sequence, providing another view of medical explanations upon scientific evidence. Development of Torsadogenic Risk Management Project and Torsadogenic Traceability concepts derived from these Chinese Medical perpetual axioms are presented in this paper.
