Primary sclerosing cholangitis in Turkish patients:characteristic features and prognosis

BACKGROUND:Primary sclerosing cholangitis(PSC)is a chronic cholestatic liver disease characterized by destruction and fibrosis of the bile *** study aimed to demonstrate the hepatic and extrahepatic characteristic findings and prognostic outcomes of Turkish patients with PSC. METHODS:The medical records of 35 consecutive patients with PSC from January 1988 to June 2007 were recorded *** the time of diagnosis,clinical features and laboratory data were collected. RESULTS:The mean age of the 35 patients was 41.69 years (range 15-80 years)at the time of diagnosis;14(40%)were female,and 21(60%)were *** mean duration of follow-up was 58.86 months(1-180 months).Twenty(57.1%) of the patients with PSC were asymptomatic and 22(62.9%) had inflammatory bowel *** the time of diagnosis, 20(57.1%)of the patients had both intra-and extra-hepatic ***-one(60%)of the patients,who had undergone ERCP for stent placement,had dominant bile duct stenosis. Cholangiocarcinoma was found in 2(5.7%)of the patients and cirrhosis was detected in 7(20%);5(14.3%)underwent liver *** median follow-up time after liver transplantation was 23 months and all are still *** (17.1%)patients died. CONCLUSiONS:PSC has a clinical course varied from advanced liver disease requiring liver transplantation within a short time to being asymptomatic for decades. The prognosis of Turkish patients with PSC is also disappointing as described in other studies.

Comparison of the clinical efficacy of two different types of post systems which were restored with composite restorations

AiM To compare the efficacy of resin composite restorations, retained with either polyethylene or zirconia-rich glass fiber posts. METHODS Sixty-two single rooted maxillary and mandibularcentral incisor teeth in forty-four patients (15 males and 29 females; age range 15-32 years) were restored either with an ultrahigh molecular weight polyethylene(UHMWP) fiber post (Bondable Reinforcement Ribbon, DENSE, Ribbond, Seattle, WA, United States) or a zircon-rich glass fiber post (Snowpost, Lot H 040; Carbotech, Ganges, France). Then, direct resin composite restoration (Clearfil AP-X, Kuraray) was performed for both post systems in tooth color suitable. Patients were recalled for routine inspections at 6 mo, 1, 2 and 3 *** The restorations were assessed during each recall evaluation according to predetermined clinical and radiographic criteria (periapical lesion; marginal leakage and integrity; color stability; surface stain and loss of retention of the post or the composite buildup material). The follow-up data showed no significant difference in these criteria between polyethylene fibre posts and zirconia-rich glass fibre *** The efficacy of resin composite restorations, retained with either polyethylene or zirconia-rich glass fiber posts were similar, suggesting that both types of fiber post can be used successfully to help retain resin composite restorations.

Theoretical and Experimental Study of a Cylindro-Parabolic Solar Collector

This article outlines the theoretical and experimental performance studies of a cylindro-parabolic solar collector. The theoretical study consists on the establishment, through mass and energy balances, of a mathematical model to control the exiting temperature of the heating fluid as well as the temperatures of the absorber and the glass. The experimental level investigates the influence of the solar absorber tube diameter on the performances of the driving device. Several experiments were made in order to know the possibility to reach temperatures being able to ensure for example the ammonia vaporization in the generator of a solar absorption refrigeration system. These experiments were carried out under various operating and climatic conditions. The results are presented and discussed.

Background:Gastric cancer is the 2 nd most common cause of cancer-related deaths,and the morbidity rate after surgery is reported to be as high as 46%.The estimation of possible complications,morbidity,and mortality and the ability to specify patients at high risk have become substantial for an intimate follow-up and for proper management in the intensive care *** study aimed to determine the prognostic value of the preoperative platelet-tolymphocyte ratio(PLR)and neutrophil-to-lymphocyte ratio(NLR)and their relations with clinical outcomes and complications after gastrectomy for gastric ***:This single-center,retrospective cohort study evaluated the data of 292 patients who underwent gastrectomy with curative intent between January 2015 and June 2018 in a tertiary state hospital in Ankara,Turkey.A receiver operating characteristic curve was generated to evaluate the ability of laboratory values to predict clinically relevant postoperative *** area under the curve was computed to compare the predictive power of the NLR and ***,the cutoff points were selected as the stratifying values for the PLR and ***:The area under the curve values of the PLR(0.60,95%Ci 0.542–0.657)and NLR(0.556,95%Ci 0.497–0.614)were larger than those of the other preoperative laboratory *** the PLR,the diagnostic sensitivity and specificity were 50.00%and 72.22%,respectively,whereas for the NLR,the diagnostic sensitivity and specificity were 37.50%and 80.16%,*** PLR was related to morbidity,whereas the relation of the NLR with mortality was more *** study demonstrated that the PLR and NLR may predict mortality and morbidity via the ClavienDindo classification in gastric cancer *** variable was grade≥3 in the Clavien-Dindo classification,including complications requiring surgical or endoscopic interventions,life-threatening complications,and *** the PLR and NLR differed significantly according to Clavien-D

To the editor： Median arcuate ligament syndrome （MALS） or celiac axis compression syndrome is an uncommon causeof mesenteric ischemia. Median arcuate ligament usually passes superior to the origin of the celiac axis. Celiac trunk is compressed by the median arcuate ligament during expiration） The majority of patients are observed with symptoms weight loss and abdominal pain. Celiac artery compression with Doppler ultrasound, Computed tomography （CT） angiography, selective catheter angiography and magnetic resonance angiography can be ascertained. After the disease is diagnosed, surgery is performed to relieve the compression.2
