Iodine Malnutrition and Associated Factors in Schoolchildren Aged 6 to 14 Years in a Municipality Situated in the Semi-Arid Region of the State of Minas Gerais, Brazil, 2008

The objective of this study was to assess the prevalence of iodine deficiency and possible correlated factors among schoolchildren in the municipality of Novo Cruzeiro-MG by means of an evaluation of urinary iodine excretion. Five hundred and forty schoolchildren aged 6 to 14 years selected by stratified sampling, were evaluated. Urine samples were analyzed using the Sandell-Kolthoff method preconized by the WHO and iodine concentrations in the salt were assessed by means of volumetric analysis, in accordance with recommendations from the Ministry of Health. Iodine deficiency was observed in 38.9% of the schoolchildren. Of these, 28.7% of them presented slight deficiency, 6.2% of them were moderately deficient and 4% of them were seriously deficient. Median urinary iodine levels in the urban and rural population were 150.8 and 119.2 μg/L, respectively, with a clear distribution of iodine deficiency among the populations (p < 0.001) being registered. It was also observed that there was a high prevalence of deficient urinary iodine excretion among schoolchildren that consumed salt with an insufficient iodine content. In relation to the quality of the salt consumed by families, in 12.2% of the residences studied the iodine content in salt was below the recommended level (20 mg/kg), while the iodine content was below 15 mg/kg in only 5.3% of them. The results indicated that, although not characterized as a public health issue, according to WHO criteria, iodine deficiency showed very high prevalence among schoolchildren in this region. Regarding the iodine content of salt found in household consumption, our findings indicated the National Program of Salt Iodination was not efficient in the city, since it did not hit the target proposed by the Ministry of Health. Furthermore, low urinary iodine excretion associated with the consumption of salt with low iodine content suggests the need for periodical evaluations in order to provide effective control of the endemic disease.

Compositional Features of Fine Sediments Involved in the Montescaglioso Landslide(Southern Italy)

This paper presents the multidisciplinary study of the southern Italy Plio-Pleistocene sediments involved in the large Montescaglioso *** principal aim of the work is to assess the compositional characters(i.e.,grain-size,mineralogy,petrography and geochemistry)and some rheological features(Atterberg’s limits,plasticity index and activity)of these sediments to enrich our knowledge about the Montescaglioso fine sediments and correlation among the lithological properties *** types of sediments,from a deep geognostic borehole and from the surrounding landslide area,were collected and *** significant compositional differences have been found between the core and landslide area ***,some changes have been detected in sediments along the ***,the-15 to-20 m lithostratigraphic level hosts the highest percentages of phyllosilicates and clay fraction(CF),commonly considered as possible hazard factors for the landslide *** more,in the studied core sediments,the CF contents correlate with the Atterberg’s liquid and plastic limits and a CF value of about 38%was suggested as threshold value for the changing of index properties of *** factors,such as the weathering degree and clay mineral type,do not show significant correlations with the rheological properties of sediments here studied.

Phrenic nerve displacement by intrapericardial balloon inflation during epicardial ablation of ventricular tachycardia: Four case reports

AIM:To investigate feasibility,morbidity and surgical mortality of a docetaxel-based chemotherapy regimen randomly administered before or after gastrectomy in patients suffering from locally-advanced resectable gastric ***:Patients suffering from locally-advanced(T3-4 any N M0 or any T N1-3 M0)gastric carcinoma,staged with endoscopic ultrasound,bone scan,computed tomography,and laparoscopy,were assigned to receive four 21 d/cycles of TCF(docetaxel 75 mg/m 2 day 1,cisplatin 75 mg/m 2 day 1,and fluorouracil 300 mg/m 2 per day for days 1-14),either before(Arm A)or after(Arm B)*** morbidity,overall mortality,and severe adverse events were compared by intention-to-treat ***:From November 1999 to November 2005,70 patients were *** preoperative TCF(Arm A),thirty-two(94%)resections were performed,85% of which were *** response was complete in 4 patients(11.7%),and partial in 18(55%).No surgical mortality and 28.5%morbidity rate were observed,similar to those of immediate surgery arm(P= 0.86).Serious chemotherapy adverse events tended to be more frequent in arm B(23%vs 11%,P=0.07),with a single death per ***:Surgery following docetaxel-based chemotherapy was safe and with similar morbidity to immediate surgery in patients with locally-advanced resectable gastric carcinoma.

Acute colonic obstruction due to malignancies is an emergency that requires surgical treatment. Elderly patients or inoperable tumors require intestinal decompression that is a simple colostomy in almost all cases. This “manoeuvre” leads the patient to a percentage of mortality/morbidity and to a bad quality of life due to acceptance of stoma. The introduction of enteral metal stent inserted endoscopically has, in our opinion, provided a new way to obtaining the definitive palliation of inoperable colo-rectal cancer with a simple method. We reported our case-series and we analyzed the current literature and costs of treatments.

CRT-D Upgrading in a Patient with Persistent Left Superior Vena Cava and Right Superior Vena Cava Atresia Using the Novel Active-Fixation Quadripolar Left Ventricular Lead

The persistence of left superior vena cava (PLSVC) is the most common congenital anomaly of the venous return system to the heart. Because of the increasing number of patients referred for cardiac resynchronization therapy (CRT) devices implantations, it is expected to encounter this venous anatomic variation. Left ventricular lead placement at an appropriate site is an integral and technically challenging part of successful CRT. In case of cardiac abnormalities could be difficult to achieve an optimal cardiac rhythm management devices implantation. Previous reports in patients with PLSVC highlighted the challenges to achieve an optimal cardiac rhythm device implantation. Recently, a new quadripolar active fixation left ventricular lead is available for CRT device implantation. Hereby we report a case of a device upgrading from dual-chamber pacemaker to CRT with defibrillator backup using the active fixation left ventricular quadripolar lead in a patient with PLSVC and right superior vena cava atresia.

Combined locoregional treatment of patients withhepatocellular carcinoma： state of the art

In recent years, a combination of intervention therapies has been widely applied in the treatment of hepatocellular carcinoma(HCC). One such combined strategy is based on the combination of the percutaneous approach, such as radiofrequency ablation(RFA), and the intra-arterial locoregional approach, such as trans-arterial chemoembolization(TACE). Several types of evidence have supported the feasibility and benefit of combined therapy, despite some studies reporting conflicting results and outcomes. The aim of this review was to explain the technical aspects of different combined treatments and to comprehensively analyze and compare the clinical efficacy and safety of this combined treatment option and monotherapy, either as TACE or RFA alone, in order to provide clinicians with an unbiased opinion and valuable information. Based on a literature review and our experience, combined treatment seems to be a safe and effective option in the treatment of patients with early/intermediate HCC when surgical resection is not feasible; furthermore, this approach provides better results than RFA and TACE alone for the treatment of large HCC, defined as those exceeding 3 cm in size. It can also expand the indication for RFA to previously contraindicated "complex cases", with increased risk of thermal ablation related complications due to tumor location, or to "complex patients" with high bleeding risk.

A Novel Classification of Brain Invasion in Malignant Ethmoidal Tumors and Management Proposal

Object: Brain invasion is unusual in patients with paranasal sinuses cancer. Staging systems do not specify brain invasion extension who has an impact in therapeutic strategy and prognosis. Surgical series of this patient treated by endonasal endoscopic approach are scarce. In this paper, a novel classification and surgical strategy is proposed. Methods: A retrospective review of patients with ethmoidal malignant tumors with brain invasion treated at the National Institute of Oncology and Radiobiology from January 2015 to October 2017 by means endonasal endoscopic surgery was performed. All medical records were reviewed for demographics, diagnoses, presentation, endoscopic approach, reconstructive techniques and clinical outcomes. Pattern of brain invasion was classified in five grades. Chi-squared test, ANOVA test and Kaplan-Meier curves were used. Results: Forty-seven patients (26 males and 21 females) were registered. Mean age was 52.3 years ± 27.0. Only 3 patients presented with neurologic symptoms. Histologic subtypes were squamous cell carcinoma (25 patients), adenocarcinoma (16 patients), and esthesioneuroblastoma (6 patients). Five patients corresponded to Grade I, 39 Grade II and 3 Grade III. The mean surgical time was 351.9 min (±45). The mean blood loss was 704.5 ml (±206). The mean ICU stay was 1.1 (ranging from 1 to 2 ) days and the hospital stay was 5.3 (ranging from 5 to 7) days. Conclusions: Brain invasion represents a particular therapeutic challenge in patients with malignant nasosinusal tumors. Surgical treatment is possible in some cases, including endonasal endoscopic approach. A stage classification of brain invasion helps to establish an optimal surgical strategy and a prognosis.

Minimally Invasive Pericranial Flap for Reconstruction after Endonasal Endoscopic Surgery of 30 Consecutive Patients

Objectives/Hypothesis: The introduction of intranasal pedicled flaps has reduced the incidence of postoperative cerebrospinal fluid (CSF) leaks to less than 5%. Nevertheless, in malignant tumors those flaps are not always available because of nasal septum invasion. Minimally invasive pericranial flaps (PCF) are associated with minimal adverse effects and good cosmetic appearance. In spite of that, there are only a few reports of this reconstructive technic limited to short surgical series and radio-anatomical analysis. Clinical results of a surgical cohort are presented. Study Design: Cohort prospective study. Methods: Clinical data, including age, gender, stage, histopathological findings, rate of complications and appearance of PCF at fifth day and two months postoperative were recorded. Postoperative morbidities were recorded as wound abnormalities, nasosinusal, orbital and central nervous system complications. Chi-squared test was used to correlate qualitative variables and Student-t-test to correlated qualitative and quantitative variables. Items were considered statistically significant with a p value of less than 0.05 (confidence Interval of 95%). Results: Thirty patients (18 males and 12 females) were registered. Mean age was 51.5 years ± 23.0 and range between 20 and 71 years. Most common histologic subtypes were adenocarcinoma, epidermoid carcinoma and squamous cell carcinoma. Complete resection of the tumor was achieved in all patients including surgical margins. Length of the PCF varies between 9.9 cm and 13.9 cm with a mean of 11.8 cm. There was an association between length of the flaps and the covering structure with the nose apex relation. None patient experienced postoperative cerebrospinal fluid (CSF) leak, frontal sinusitis or other complications. Conclusions: Minimally invasive PCF constitute a good and inexpensive reconstructive option in patients with malignant anterior cranial base tumors in whose nasoseptal flap was not a feasible option.
