Nursing Diagnostics of Nutrition Domain of NANDA International in Hemodialysis Patients

Introduction: The dialysis causes the loss of the nutrients and the elevation of catabolism, thus, patients in dialysis have at high risk for the loss of body reserves of protein and energy. Objective: To identify nursing diagnoses of nutrition domain of NANDA International in hemodialysis patients and to correlate them to the socioeconomic and clinical data. Design: Cross-sectional study. Setting: Performed in a large university hospital in northeastern Brazil. Participants: The first stage was carried out with 50 patients, selected with the following inclusion criteria: Being hospitalized and undergoing hemodialysis in that hospital;aged 18 years old or over;being conscious and oriented. Exclusion criteria were patients undergoing hemodialysis in that hospital with external service, pregnant women and patients being treated by plasmapheresis. In the second stage, participants were three nursing specialists in nephrology and nursing diagnoses. Methods: The defining characteristics of the nursing diagnoses inserted in the nutrition domain were observed in patients and then these data were judged by nurses, in order to identify diagnoses present in each patient. The Chi-square, Fisher’s exact and Mann-Whitney tests were used in the analysis of the relationship of socioeconomic and clinical data, with an adopted p value of p = 0.001), origin (p = 0.014), religion (p = 0.046) and income (p = 0.039), respectively. Conclusion: The variables that showed significant association were risk of impaired liver function and gender;imbalanced nutrition: More than body requirements and origin requirements;risk for deficient fluid volume and religion;and deficient fluid volume and income.

Causes and consequences of microsatellite instability in gastric carcinogenesis

Loss of DNA mismatch repair(MMR) function, due to somatic or germline epi/genetic alterations of MMR genes leads to the accumulation of numerous mutations across the genome, creating a molecular phenotype known as microsatellite instability(MSI). In gastric cancer(g C), MSI occurs in about 15% to 30% of the cases. This review summarizes the current knowledge on the molecular mechanisms underlying the acquisition of MSI in g C as well as on the clinic, pathologic and molecular consequences of the MSI phenotype. Additionally, current therapeutic strategies for g C and their applicability in the MSI subset are also discussed.

Assessing the estrogenic potency in a Portuguese wastewater treatment plant using an integrated approach

The estrogenic potency of a wastewater treatment plant （WWTP） was evaluated using chemical and biological analyses, which showed that after the station treatment processes some of the selected endocrine disruptor compounds （EDCs） were still present in the treated effluent （e.g., bisphenol A, alkylphenols, estrone）. Thus, the most common endocrine EDCs were identified and quantified and the overall estrogenicity of the treated effluent assessed by integrating the results. Male goldfish （Carassius auratus） were used as biological indicators in a 28-day experiment. Vitellogenin （Vtg）, gonadosomatic and hepatosomatic indices, steroids （17β-estradiol and 11-ketotestosterone） and histopathology were biomarkers used in fish to evaluate WWTP treated effluent estrogenicity, in combination with instrumental analyses. The results showed a significant increase （P 〈 0.01） in plasma and liver Vtg, which were significantly correlated （r = 0.66; P 〈 0.01）. The gonadosmatic index was significantly （P 〈 0.01） reduced in exposed fish. The steroid analyses revealed significant elevations in 17β-estradiol and depressed 11-ketotestosterone concentrations. The histological examinations show changes in exposed fish gonads, such as regressed testes and in some cases （43% to 75%） the development of ovo-testis in fish exposed to 50% and 100% treated effluent.

Leukocyte-technetium-99m uptake in Crohn's disease:Does it show subclinical disease?

AIM:To evaluate inflammatory activity in patients with Crohn's disease (CD) using technetium-99m-hexamethylpropyleneamine oxime (99mTc-HMPAO) granulocyte ***: Twenty patients (7 male and 13 female) with CD and five healthy volunteers were selected for 99mTc-HMPAO granulocyte scintigraphy. The Crohn's Disease Activity Index (CDAI), blood tests and C-reactive protein (CRP) of each patient were performed 7 d before the scintigraphic images. The leukocytes were labeled according to the International Society of Radiolabeled Blood Elements (ISORBE) consensus protocol and the scintigraphic images, including single photon emission computed tomography, were obtained 30 min and 2 h after injection of the radiolabeled ***:The labeling yield of the leukocytes with the lipophilic complex 99mTc-HMPAO was 55.0%±10%. Six of the 20 patients (30%) presented congruent results for the three parameters investigated (CDAI, Scintigraphic Index and CRP). On the other hand, 14 patients (70%) did not show congruent results. There was no significant correlation between the indices analyzed according to the Spearman test (P>0.05,n=20).CONCLUSION: The results suggest that 99mTc-HMPAO-labeled leukocyte scintigraphy could be important for determining inflammatory activity in CD even in the absence of clinical symptoms.

Fluid-dynamics evaluation in a conical spouted bed and characterization of foxtail millet seeds

Foxtail millet （Setaria italica） is one of the most valuable species in economic terms in the genus Setaria and plays an important role in human nutrition, animal feed, and agriculture. The present study described chemical, physical, and quality aspects of seeds of foxtail millet. Furthermore, the fluid-dynamic behavior of the seeds was evaluated in a conical spouted bed, which has advantages in terms of promoting the cyclic and regular movement of the seed particles. Dynamic parameters of spouting （minimum spouting veloc- ity, stable and peak pressure drop） were determined and compared with those obtained from empirical correlations available in the literature. The results obtained from physical characterization showed that the seeds can be classified as belonging to Group D of Geldart, having a non-rough surface, mean diameter of 1.75 mm, and sphericity of 0.74. Fluid-dynamics analysis showed that the seeds are suitable for processing in a spouted bed, which is in agreement with the results of particle physical characterization.

A long-term annual water balance analysis of the Aracuai River Basin, Brazil

Semi-arid and arid areas exhibit great temporal variability in water availability. In some of these regions, a one- or two-day rainfall is followed by intervening dry periods of variable length. In recent decades, many rivers of the semi-arid portion of the Jequitinhonha Basin have been undergoing a progressive discharge reduction, mainly of their base flow, the Aracuai River is one of them. In order to understand this transformation, a long-term analysis of the annual water balance of the Aracuai River Basin was performed, the results of which are presented herein. Satellite images, hydrometeorological and river discharge data were analyzed; mean and variance tests were conducted to determine temporal homogeneity. Historical pluviometric data analysis shows no corresponding precipitation reduction and temperature undergoes only a slight increase in the same period. On the other hand, evaporation is extremely high, higher than precipitation during most of the year, leaving almost no water for infiltration （aquifer recharge） and runoff. Furthermore, the Aracuai head- waters of its tributaries are now occupied by a monoculture, Eucaliptus sp., used for paper production. Because of the decreased fluvial discharges, its lowlands, usually used for agriculture and pasture, are abandoned and partially eroded.

Best management practices to reduce soil erosion and change water balance components in watersheds under grain and dairy production

Soil erosion and sedimentation are among the most serious global environmental *** and water conservation measures have been proven effective ways to reduce soil *** objective of this study was to evaluate the impact of three approaches of soil and water conservation measures(soil management,vegetative measures,and structural practices)on soil erosion and water balance of two paired agricultural watersheds located in Southern *** and sediment monitoring was carried out from 2016 to 2019 in the two small paired agricultural watersheds;called North watershed(NW)and South watershed(SW).Modeling using Soil&Water Assessment Tool(SWAT)was performed to simulate individual(nine scenarios)and combined(four scenarios)best management practices(BMPs),by including the three *** the nine individual BMP scenarios,the most effective in reducing soil erosion was crop rotation and cover crop(sediment yield,SY,reduction of 38.4 for NW,and 28.8%for SW).Among the four combined scenarios,the association of all conservation approaches was the most effective in reducing soil erosion(SY reduction of 46 for NW,and 41.5%for SW),followed by the vegetative measures scenario(SY reduction of 43.5 and 34.1%for NW and SW).All combined scenarios increased infiltration and subsurface water components,and decreased surface *** findings of this study can help farmers and policymakers choosing appropriate BMPs to reduce current soil erosion problems and promote water and soil conservation.

AIM:To investigate the effects of titanium dioxide (TiO2) nanoparticles (NPTiO 2 ) and microparticles (MPTiO 2 ) on the inflammatory response in the small intestine of mice. METHODS: Bl 57/6 male mice received distilled water suspensions containing TiO 2 (100 mg/kg body weight) as NPTiO 2 (66 nm), or MPTiO 2 (260 nm) by gavage for 10 d, once a day; the control group received only distilled water. At the end of the treatment the duodenum, jejunum and ileum were extracted for assessment of cytokines, inflammatory cells and titanium content. The cytokines interleukin (IL)-1b, IL-4, IL-6, IL-8, IL-10, IL-12, IL-13, IL-17, IL-23, tumor necrosis factor-α (TNF-α), intracellular interferon-γ (IFN-γ) and transforming growth factor-β (TGF-β) were evaluated by enzyme-linked immunosorbent assay in segments of jejunum and ileum (mucosa and underlying muscular tissue). CD4 + and CD8 + T cells, natural killer cells, and dendritic cells were evaluated in duodenum, jejunum and ileum samples fixed in 10% formalin by immuno-histochemistry. The titanium content was determined by inductively coupled plasma atomic emission spectrometry. RESULTS: We found increased levels of T CD4 + cells (cells/mm 2 ) in duodenum:NP 1240 ± 139.4, MP 1070 ± 154.7 vs 458 ± 50.39 (P < 0.01); jejunum:NP 908.4 ± 130.3, MP 813.8 ± 103.8 vs 526.6 ± 61.43 (P < 0.05); and ileum:NP 818.60 ± 123.0, MP 640.1 ± 32.75 vs 466.9 ± 22.4 (P < 0.05). In comparison to the control group, the groups receiving TiO 2 showed a statistically significant increase in the levels of the inflammatory cytokines IL-12, IL-4, IL-23, TNF-α, IFN-γ and TGF-β. Maytenus erythroxylon Reissek(Celastraceae) ethanol extract presents antidiarrheal activity via antimotility and antisecretory mechanisms

To investigate the acute toxicity, phytochemical profile, antidiarrheal activity and mechanisms of action of Maytenus erythroxylon (M. erythroxylon) ethanol extract. METHODSA castor oil-induced diarrhea model was used to evaluate antidiarrheal activity. Intestinal transit and gastric emptying protocols were used to evaluate a possible antimotility effect. KATP channels, nitric oxide, presynaptic α2-adrenergic and tissue adrenergic receptors were investigated to uncover antimotility mechanisms of action and castor oil-induced enteropooling to elucidate antisecretory mechanisms. RESULTSAll tested doses of the extract (62.5, 125, 250 and 500 mg/kg) possessed antidiarrheal activity, with a significant decrease of the evacuation index. This activity is possibly related to a reduced gastric emptying (125, 250 and 500 mg/kg) and to a decreased percentage of intestinal transit for all tested doses. That last effect seems to be modulated by nitric oxide, KATP channels and tissue adrenergic receptors. Besides, the extract also presented antisecretory effect due to a decrease of intestinal fluid accumulation. CONCLUSIONThe antidiarrheal effect of M. erythroxylon found in this study involves antimotility and antisecretory mechanisms that may be attributed to the chemical compounds found in this species: saponins, flavonoids, tannins, triterpenes and steroids.
