Esophagogastric Reconstruction in Cobra-Head Shape with Toupet-Like Partial Anti-Reflux Technique for Resection of Proximal gastric Tumors. Experience with Three Cases from a Non-Asian Population

Background: Proximal gastrectomy is a rarely performed procedure but a feasible option in benign tumors and malignant neoplasms in the proximal third of the stomach since novel options of reconstructions are available nowadays with fewer long-term sequels. Methods: Report of three cases of proximal gastric gastrointestinal stromal tumors (gIST), with a description of its presentation, histological characteristics, and follow-up after being treated with proximal gastrectomy with cobra head reconstruction. Results: Case 1: A 62-year-old woman with epigastric pain of four months with endoscopic evidence of a cardia gIST. The surgery was performed without complications. The histopathological report confirmed a fusiform gIST of 3.2 × 3 × 2.5 cm, stage IA. No adjuvant treatment was considered. An esophagogram showed no evidence of reflux and no stenosis, and no disease recurrence after 40-months follow-up. Case 2: A 66-year-old woman with dyspepsia and a palpable tumor in the epigastrium. The surgery was performed without complications. The pathology report confirmed a proximal gastric gIST of 13 × 8 × 7 cm, staged II. She received adjuvant treatment with imatinib for 36 months without recurrence and no reflux or stenosis. Case 3: A 55-year-old woman with intermittent hematemesis and right subscapular pain. The surgery was performed without complications. The histopathological report concluded that a gIST tumor of the cardia, 1.4 × 1.2 cm, staged IA. She was left in observation. At a 6-month follow-up, she does not report dysphagia, reflux, or stenosis, and no locoregional recurrence. Conclusions: Open and laparoscopic proximal gastrectomy is a safe therapeutic option for gIST. Furthermore, the reconstruction of the esophagus-gastro anastomosis in the cobra head after proximal gastrectomy is feasible and secure in our population, with good functional results in a short follow-up.

This paper presents a parameter estimation technique for the hot-spot thermal model of power *** proposed technique is based on the unscented formulation of the Kalman filter,jointly considering the state variables and parameters of the dynamic thermal model.A two-stage estimation technique that takes advantage of different loading conditions is developed,in order to increase the number of parameters which can be *** results are presented,which show that the observable parameters are estimated with an error of less than 3%.The parameter estimation procedure is mainly intended for factory testing,allowing the manufacturer to enhance the thermal model of power transformers and,therefore,its customers to increase the lifetime of these *** proposed technique could be additionally considered in field applications if the necessary temperature measurements are available.

Pollen analysis reveals the effects of uncovered interactions,pollen-carrying structures,and pollinator sex on the structure of wild bee-plant networks

Pollination networks are increasingly used to model the complexity of interactions between pollinators and flowering plants in *** methods exist to sample these interactions,with direct observations of plant-pollinator contacts in the field being by far the most *** the identification of pollen carried by pollinators allows uncovering interactions and increasing sample sizes,the methods used to build pollen-transport networks are variable and their effect on network structure remains *** understand how interaction sampling influences the structure of networks,we analyzed the pollen found on wild bees from eight communities across Mallorca Island and investigated the differences in pollen loads between bee body parts(scopa ***)and *** then assessed how these differences,as well as the uncovered interactions not detected in the field,influenced the structure of wild bee-plant *** identified a higher quantity and diversity of pollen in the scopa than in the rest of the female body,but these differences did not lead to differences in structure between plant-pollination(excluding scopa pollen)and bee-feeding interaction(including scopa pollen)***,networks built with pollen data were richer in plant species and interactions and showed lower modularity and specialization(H2'),and higher nestedness than visitation networks based on field *** interactions with plants were stronger compared to those of males,although not ***,females were more generalist(low d')and tended to be more central in interaction networks,indicating their more key role structuring pollination networks in comparison to *** study highlights the importance of palynological data to increase the resolution of networks,as well as to understand important ecological questions such as the differences between plant-pollination and bee-feeding interaction networks,and the role of sexes in pollination.

This paper reviews the current state of knowledge on psychological interventions with empirical evidence of efficacy in treating common psychiatric and behavioral disorders in people with intellectual disability(ID)at all stages of their *** begin with a brief presentation of what is meant by psychiatric and behavioral disorders in this population,along with an explanation of some of the factors that contribute to the increased psychosocial vulnerability of this group to present with these *** then conduct a review of empirically supported psychological therapies used to treat psychiatric and behavioral disorders in people with *** review is structured around the three generations of therapies:Applied behavior analysis(e.g.,positive behavior support),cognitive behavioral therapies(e.g.,mindfulness-based cognitive therapy),and contextual therapies(e.g.,dialectical behavior therapy).We conclude with some recommendations for professional practice in the fields of ID and psychiatry.

Morphology and distribution of Pronoctiluca (Dinoflagellata, incertae sedis) in the Pacific Ocean

The diversity and distribution of Pronoctiluca, a marine dinoflagellate of enigmatic systematic position, was studied in the vicinity of the Kuroshio and Oyashio Currents, the Philippines, Celebes, Sulu and South China Seas, western and central equatorial and southeast Pacific Ocean. The abundance of Pronoctiluca was high- er, with a wide vertical distribution, in eutrophic temperate regions, whereas it was nearly absent in surface waters of oligotrophic tropical regions. Most of the specimens corresponded to P. spinifera. Pronoctiluca pelagica, covered by hyaline layers and with no flagella, is considered as an encysted form. The bipartition was only observed in P. acuta-P, spinifera forms, that together with the occurrence of intermediate forms between P. spinifera and P. pelagica suggest that they may correspond to developmental stages of a single species. Pronoctiluca is essential to understanding the evolutionary history of the alveolates.

Strain engineering,as a powerful strategy to tune the optical and electrical properties of two-dimensional(2D)materials by deforming their crystal lattice,has attracted significant interest in recent years.2D materials can sustain ultra-high strains,even up to 10%,due to the lack of dangling bonds on their surface,making them ideal brittle *** remarkable mechanical resilience,together with a strong strain-tunable band structure,endows 2D materials with a broad optical and electrical response upon ***,strain engineering based on 2D materials is restricted by their nanoscale and strain quantification *** this study,we have modified a homebuilt three-points bending apparatus to transform it into a four-points bending apparatus that allows for the application of both compressive and tensile strains on 2D *** approach allows for the efficient and reproducible construction of a strain system and minimizes the buckling effect caused by the van der Waals interaction by adamantane encapsulation *** results demonstrate the feasibility of introducing compressive strain on 2D materials and the potential for tuning their optical and physical properties through this approach.

Dicroerisma (Actiniscales, Dinophyceae) in the open Pacific Ocean, a gymnodinioid dinoflagellate with an endoskeleton

This study is the first to describe and illustrate the dinoflagellate genus Dicroerisma in the open Pacific Sea and in the southern hemisphere. Eleven individuals ascribed to Dicroerisma psilonereiella were encountered in subsurface waters (>70 m depth) from 34°N to 33°S. Another specimen that differs from the type is also illustrated. These findings reveal that Dicroerisma is a widespread genus and that there is tentative existence of another species within the genus.

Lessons from International Students’ Reaction to the 2011 great East Japan Earthquake:The Case of the School of Engineering at Tohoku University

The objective of this study is to document the reaction of international students to the 11 March 2011 emergency in order to inform and improve disaster management strategies, both public and institutional. The study is based mainly on a questionnaire survey carried out in August 2011 and follow-up interviews with students and other stakeholders. It describes the background of Tohoku University’s School of Engineering and covers six different stages in the evolution of the emergency:(1) immediate response;(2) taking shelter in Sendai;(3) life in the city during the aftermath;(4) sheltering outside Sendai;(5) coming back;plus(6) an overview of the experience. Major findings include: the process of evacuation and safety confirmation was successful, yet the subsequent phases(2 and 3) went on mostly unmanaged;students relied mostly on secondary sources of information to make decisions, mainly family and friends of the same nationality, most of whom probably were not better informed than the students themselves. Based on the findings, suggestions for future disaster planning are advanced, as well as a discussion on the challenges of information provision during crisis.
