Impact of next-generation sequencing on antimicrobial treatment in immunocompromised adults with suspected infections

BACKGROUnD:Prompt pathogen identification can have a substantial impact on the optimization of antimicrobial *** objective of the study was to assess the diagnostic value of next-generation sequencing(nGS)for identifying pathogen and its clinical impact on antimicrobial intervention in immunocompromised patients with suspected ***:This was a retrospective *** January and August 2020,47 adult immunocompromised patients underwent nGS testing under the following clinical conditions:1)prolonged fever and negative conventional cultures;2)new-onset fever despite empiric antimicrobial treatment;and 3)afebrile with suspected infections on *** data,including conventional microbial test results and antimicrobial treatment before and after nGS,were *** were analyzed according to documented changes in antimicrobial treatment(escalated,no change,or deescalated)after the nGS ***:The median time from hospitalization to nGS sampling was 19 *** relevant pathogens were detected via nGS in 61.7% of patients(29/47),more than half of whom suffered from fungemia(n=17),resulting in an antimicrobial escalation in 53.2% of patients(25/47)and antimicrobial de-escalation in 0.2% of patients(1/47).Antimicrobial changes were mostly due to the identification of fastidious organisms such as Legionella,Pneumocystis jirovecii,and *** the remaining three cases,nGS detected clinically relevant pathogens also detected by conventional cultures a few days *** antimicrobial treatment was subsequently adjusted according to the susceptibility test ***,nGS changed antimicrobial management in 55.3%(26/47)of patients,and conventional culture detected clinically relevant pathogens in 14.9% of the patients(7/47).COnCLUSIOn:With its rapid identification and high sensitivity,nGS could be a promising tool for identifying relevant pathogens and enabling rapid appropriate treatment in immunocompromised patients w

Enhancement of low-temperature toughness of Fe-Mn-C-Al alloy by rare earth Ce-modified inclusions

Fe-Mn-C-Al alloys have been recognized as promising materials for certain low-temperature applications due to their exceptional mechanical properties and ***,their limited low-temperature toughness restricts their large-scale applications in specific *** influence of trace amounts of rare earth cerium(Ce)on the low-temperature toughness of Fe-18Mn-0.6C-1.8Al alloys was *** addition of Ce effectively alters the inclu-sions in the alloy,transforming large-sized irregular inclusions into fine ellipsoidal rare earth *** leads to a significant reduction in both the proportion and average size of the inclusions,resulting in their effective dispersion throughout the matrix and improved cryogenic *** presence of Ce-containing inclusions within the matrix reduces stress concentration,thereby inhibiting microcrack formation and improving impact absorption ***-cally,the addition of rare earth Ce alters the fracture behavior of the material at room temperature and low temperature,changing from brittle cleavage fracture to a more ductile failure *** impact toughness of the Fe-Mn-C-Al alloy is significantly improved by the addition of 0.0048 wt.%Ce,particularly at-196℃where the impact toughness reaches 103.6 J/cm^(2),representing an impressive improvement of 87.3%.

One potential solution to the problems of energy storage and conversion is the use of reversible protonic ceramic electrochemical cells(R-PCEC),which are based on the solid oxide fuel cell(SOFC)technology and offer a flexible route to the generation of renewable ***,the R-PCEC development faces a range of significant challenges,including slow oxygen reaction kinetics,inadequate durability,and poor round-trip efficiency resulting from the inadequacy of an air *** address these issues,we report novel B-sites doped Pr_(0.5)Ba_(0.5)Co_(0.7)Fe_(0.3)O_(3−δ)(PBCF)with varying amounts of Sn as the air electrode for R-PCEC to further enhance electrochemical performance at lower *** 600℃,R-PCEC with an air electrode consisting of Pr_(0.5)Ba_(0.5)Co_(0.7)Fe_(0.25)Sn_(0.05)O_(3+δ)has achieved peak power density of 1.12 W∙cm^(−2) in the fuel cell mode and current density of 1.79 A∙cm^(−2) in the electrolysis mode at a voltage of 1.3 ***,R-PCECs have shown good stability in the electrolysis mode of 100 *** study presents a practical method for developing durable high-performance air electrodes for R-PCECs.

To investigate the influence of structural parameters on the performances and internal flow characteristics of partial flow pumps at a low specific speed of 10000 rpm,special attention was paid to the first and second stage impeller guide ***,the impeller blade outlet width,impeller inlet diameter,blade inclination angle,and number of blades were considered for orthogonal ***,nine groups of design solutions were formed,and then used as a basis for the execution of numerical simulations(CFD)aimed at obtaining the efficiency values and heads for each design solution *** influence of impeller geometric parameters on the efficiency and head was explored,and the“weight”of each factor was obtained via a range *** structural parameters were finally chosen on the basis of the numerical simulation results,and the performances of the optimized model were verified accordingly(yet by means of CFD).Evidence is provided that the increase in the efficiency and head of the optimized model was 12.11%and 23.5 m,respectively,compared with those of the original model.

Heavy-atom engineered hypoxia-responsive probes for precisive photoacoustic imaging and cancer therapy

Photoacoustic agents combining photodynamic therapy(PDT) and photothermal therapy(PTT) functions have emerged as potent theranostic agents for combating cancer. The molecular approaches for enhancing the near-infrared(nIR)-absorption and maximizing non-radiative energy transfer are essential for effective photoacoustic imaging(PAI) and therapy applications. In addition, such molecules with high specificity and affinity to cancer cells are urgently needed, which would further decrease the side effect during treatments. In this study, we applied a heavy-atom engineering strategy and introduced p-aminophenol,-thio, and-seleno moieties into nIR heptamethine cyanine(Cy7) skeleton(Cy7-X-nH_(2), X = O, S, Se) to significantly increase photothermal conversion efficiency for PTT and promote intersystem crossing for ***, we designed a series of nitroreductase(nTR)-activated photoacoustic probes(Cy7-X-nO_(2),X = O, S, Se), and target hypoxic tumors with nTR overexpression. Our prostate cancer targeting probe,Cy7-Se-nO_(2)-KUE, exhibited specific tumor photoacoustic signals and effective tumor killing through outstanding synergistic PTT/PDT in vivo. These findings highlighted a versatile strategy for cancer photoacoustic diagnosis and enhanced phototherapy.

Effects of Additives on the Microstructure and Tribology Performance of Ta-12W Alloy Micro-Arc Oxidation Coating

Oxide ceramic coatings were fabricated on tantalum alloys by micro-arc oxidation (MAO) to improve their hardness and tribological properties. The MAO coatings were manufactured in a mixed silicatephosphate electrolyte containing naF and/or EDTA (ethylene diamine tetraacetic acid). The surface morphology,cross-sectional view, chemical composition, hardness, and wear performance of the coatings were analysed. As revealed by the scanning electron microscopy, silica-rich nodules appear on the MAO coating obtained in the silicate-phosphate electrolyte, but the formation of nodules is inhibited with naF and/or EDTA in the ***, they reduce the roughness and improve the compactness of the coatings, which are composed of Ta_(2)O_(5),(Ta, O), and TaO. A thick and hard coating is obtained in the naF-containing electrolyte, and the tribology performance is effectively improved. With additives, the nodule structure is detached from the coating surface and dissolved in the electrolyte. By using naF as an electrolyte additive, the abrasion performance of the MAO coating is enhanced by decreasing the nodule structure, increasing the size of micropores, and improving the coating hardness.

Voluntary wheel running ameliorated the deleterious effects of high-fat diet on glucose metabolism,gut microbiota and microbial-associated metabolites

Exercise training is critical for the early prevention and treatment of obesity and diabetes ***,the mechanism with gut microbiota and fecal metabolites underlying the effects of voluntary wheel running on high-fat diet induced abnormal glucose metabolism has not been fully elaborated.C57BL/6 male mice were randomly assigned to 4 groups according to diets(fed with normal chow diet or high-fat diet)and running paradigm(housed in static cage or with voluntary running wheel).An integrative 16S rDnA sequencing and metabolites profiling was synchronously performed to characterize the effects of voluntary wheel running on gut microbiota and *** showed that voluntary wheel running prevented the detrimental effects of high-fat feeding on glucose metabolism 16S rDnA sequencing showed remarkable changes in Rikenella and Marvinbryantia *** profiling indicated multiple altered metabolites,which were enriched in secondary bile acid biosynthesis *** conclusion,our study indicated that voluntary wheel running significantly improved glucose metabolism and counteracted the deleterious effects of high-fat feeding on body weight and glucose *** further found that voluntary wheel running could integratively program gut microbiota composition and fecal metabolites changes,and may regulate muricholic acid metabolism and secondary bile acid biosynthesis in high-fat fed mice.

BACKGROUnD Rhabdomyosarcoma(RMS)of the vagina in postmenopausal women is an extre-mely rare malignant tumor that was originally described as a unique group of soft tissue sarcomas originating from primitive mesenchymal *** was first re-ported in postmenopausal women in 1970,and fewer than 50 postmenopausal patients have been reported to *** SUMMARY A 68-year-old multiparous female was admitted to the hospital on October 11,2023,with the chief complaint of a mass causing vaginal prolapse with incomplete urination that had persisted for 4 *** vaginal mass was approximately the size of a pigeon egg;after lying down,the vaginal mass *** resection was performed,and vaginal pleomorphic RMS was diagnosed based on pathology and immunohistochemical staining *** patient is currently undergoing *** present study also reviewed the clinical,histolo-gical,and immunohistochemical features and latest treatment recommendations for vaginal *** abnormal vaginal mass should be promptly investigated through pelvic examination and appropriate *** current initial treat-ment for vaginal RMS is biopsy and primary *** When surgery is planned for vaginal RMS,an organ-preserving approach should be considered.
