The use of the Ligasure vessel sealing device in laparoscopic nephrectomy

摘      要:Objective To evaluate the value of the LigaSure vessel sealing in laparoscopic nephrectomy surgery. Materials and Methods From January 2004 to April 2008, 79 laparoscopic nephrectomy were performed with the LigaSure vessel sealing system, including simple nephrectomies, radical nephrectomies and nephroureterectomies. The operative time, estimated intraoperative blood loss, conversion rate, duration of postoperative drainage, total amount of postoperative drainage, postoperative hospital day as well as complication rate were recorded and studied retrospectively. Results Conversion to an open procedure was required in only one patient who had vena caval thrombus. There was no severe vascular complication or serious complication in other cases. The mean operative time was 158min (range, 35~265min);the mean blood loss was 187ml (range, 20~640ml);the mean time for postoperative drainage was 3d (range, 1~7d);the mean amount of postoperative drainage was 206ml (range, 27~435 ml). The postoperative hospital staying was 5~21d, with the averageof 9 days. The conversion rate was 0.01%. Conclusions The LigaSure vessel sealing system produces a consistent, reliable, permanent seal of veins, arteries, and tissue bundles. It can decrease operative time and blood loss. This new energy-based vessel-ligation device appears to be effective in advanced laparoscopic procedures.
