Subsequent laparoscopic adrenalectomy for patients with previous adrenal surgery(report of 3 cases)

摘      要:Objective To evaluate the feasibility of subsequent laparoscopic adrenalectomy for patients with previous adrenal surgery. Methods From May 2005 to March 2007,3 patients(3 women) with primary aldosteronism underwent transperitoneal laparoscopic adrenalectomy, including 2 patients who had previously undergone retroperitoneal laparoscopic partial adrenalectomy and 1 patient who had previously undergone open partial adrenalectomy through lumbar incision. The mean patient age was 48 years(range 35~56years).The operative time, estimated blood loss, postoperative hospital stay, complications and the time of convalescence were recorded. Results The operation was successfully performed in all patients. The mean operative time was 90 min(range 75~110min).The mean estimated blood loss was 20ml(range 10~30ml), and no blood transfusion or conversion to an open procedure was required. The mean hospital stay was 5d (range 4~6d).No complication occurred. During a follow-up of 8~25 months, no tumor recurrence was found. Conclusion Although prior adrenal surgery greatly increases the degree of difficulties in the subsequent operation,the transperitoneoscopic approach to the adrenal can be used with prior retroperitoneoscopic or open adrenalectomy without significant increases in morbidity.
