The comparison of percutaneous and trans-spatium intermuscular pedicle screw fixation for the treatment of thoracolumbar fractures

摘 要:Objective:To access the clinical result of percutaneous pedicle screw fixation using Zina system in treating thoracolumbar ***:From September 2013 to February 2014,a total of 30 patients with thoracolumbar fractures(type A,loadsharing ≤ 6,without neurological deficit) underwent percutaneous pedicle screw fixation using Zina system(percutaneous group,PG) were retrospectively *** perioperative parameters,radiographic and clinical results were compared with another 30 patients underwent trans-spatium intermuscular pedicle screw fixation(trans-spatium intermuscular group,TSIG).Result:In PG,the length of skin incision was(7.35±0.76)cm,the operative time was(95.7±14.2)min,intraoperative blood loss was(53.7±19.9)ml and the VAS of postoperative first day was(3.7±1.3)point;while in TSIG,the data were(9.80±1.92)cm,(82.3±16.4)min,(122.0±28.2)ml and(5.8±1.3)point respectively,which showed significant difference between two groups(P 0.05).In PG,the anterior vertebral body height and Cobb angle at the pre-operation,post-operation and final followup were(60.8±12.1)%,(88.3±6.9)%,(86.5±7.2)%,(18.5±4.9) °,(4.9±2.5) ° and(5.2±1.7) °;While in TSIG,the data were(61.8 ±9.9)%,(91.1 ±4.3)%,(91.3 ±4.2)%,(19.0 ±3.7)°,(4.7±3.5) ° and(4.8±3.4) °,which showed significant difference between two groups(P 0.05).Compared to at the pre-operation,the anterior vertebral body height and Cobb angle at the post-operation were corrected significantly in both groups(P 0.05).Compared to at the post-operation,there were no significant losses of the recovery of the anterior vertebral body height and Cobb angle at the final follow-up in both groups(P 0.05).Conclusion:Under strict control of surgical indications,percutaneous pedicle screw fixation using Zina system in treating thoracolumbar fractures is comparable to trans-spatium intermuscular pedicle screw fixation,with the advantages of shorter incision,less blood loss and less postoperative pain.
