Protective effect of ginsenoside Rb1 against ischemia/reperfusion injury in streptozotocin-induced diabetic rats

摘      要:OBJECTIVE:Diabetic patients with cardiovascular disease have a high mortality rate than patients without diabetes. The cardioprotective effect of ginsenoside Rb1 present in ginseng on myocardial ischemia reperfusion (I/R) has not been examined in diabetic myocardium. The present study was designed to investigate whether ginsenoside Rb1 can protect the heart against I/R injury in diabetes models and search the underlying mechanism. METHODS:Ginsenoside Rb1,wortmaninn were administered prior to I/R procedure. Rats were divided into 7 groups:(i) diabetic sham,(ii) normal,(iii) diabetic,(iv) diabetic+ginsenosid Rb1,(v) diabetic+wortmaninn,(vi) diabetic +ginsenoside Rb1+wortmaninn,(vii) diabetic sham+wortmaninn. After the treatment,hearts were subjected to 30 min of ischemia followed by 2 h of reperfusion. Arterial blood pressure,Heart rate were monitored throughout the whole procedure. Additionally,plasma creatine kinase isoenzyme MB (CK-MB) and lactate dehydrogenase (LDH) were analyzed to assess the degree of cardiac injury. Myocardial infarction,caspase-3 activity and myocardial apoptosis (TUNEL analysis) were also measured. RESULTS:Myocardial I/R caused significant cardiac dysfunction and myocardial apoptosis in diabetic rats. Diabetic group treated with ginsenoside Rb1 demonstrated significant improved cardiac function,reduced infarct size,cardiomyocyte apoptosis and caspase-3 activity compared to the diabetic group or the group treated with wortmaninn. Ginsenoside Rb1 significantly unregulated phosphorylated Akt (p-AKT) expression. Treatment with wortmaninn in ginsenoside Rb1-treated diabetic rats demonstrated significant downregulation of p-AKT. CONCLUSION:Ginsenoside Rb1 exerts cardioprotective effects against myocardial I/R injury in diabetic rats,including anti-cardiomyocyte apoptosis,which is partly through activation of PI3 kinase/Akt pathway. Thus this study shows a novel pharmacological preconditioning with ginsenoside Rb1 in the diabetic myocardium.
