SIGNIFICANCE OF LIVER BIOPSY IN THE DIAGNOSIS OF INFECTIOUS HEPATITIS IN CHILDREN

摘 要:1. The pathologic changes of anicteric hepatitis are essentially the same as of the mild form of icteric hepatitis. However, they are quite different from those of liver enlargement caused by other diseases, and differentiation of the changes is not difficult.2. The principal criteria for a clinical diagnosis of anicteric hepatitis are contact history and hepatomegaly not explainable by other causes. Subjective complaints and liver function tests serve only as references.3. In this series the rate of missed diagnosis with anicteric form of hepatitis was rather high—approximately 30% of all hepatitic cases or 44% of hepatitic cases of anicteric form.4. Most cases of our series were clinically diagnosed as protracted and chronic hepatitis. Upon examination of biopsy tissue however, with exception of 1 protracted case, all were pathologically diagnosed as mild form of hepatitis. On the other hand, in clinically recovered cases an active hepatitic process remained. The above observations speak in favor of the assumption that a complete healing of hepatitis is not easily achieved, nor is a progression into liver cirrhosis. This is not only applicable to the anicteric form, but also to the icteric form. We are of the opinion that prognosis of icteric hepatitis in childhood requires further study.
