摘      要：In this article we discuss the challenges of delivering a high quality Transition care. A good understanding of the adolescent needs with good communication between Transition care physicians and the patient is essential for good continuity of care. Despite availability of several guidelines, one model doesn t fit all and any transition service development should be determined by the local need and available healthcare facilities.
