摘      要:慢性阻塞性肺疾病(COPD)和肺癌作为呼吸系统常见疾病,二者之间存在密切关联。本文综述了COPD疾病中肺癌风险的研究进展。首先,介绍了COPD和肺癌的流行病学联系以及研究现状,探讨了COPD患者易患肺癌的原因。随后,分析了COPD对肺癌发生的影响机制,探讨了两者共同的危险因素对肺癌风险的影响,以及COPD引起的慢性炎症对肺癌发展的影响。在COPD患者肺癌风险评估方面,本文总结了诊断标志物研究、影像学评估方法以及影响COPD患者肺癌预后的因素研究进展。此外,讨论了针对COPD患者的肺癌早期筛查和干预策略,介绍了针对合并肺癌的联合治疗策略和个体化治疗在COPD患者肺癌管理中的应用。综合研究现状和未来发展趋势,本文展望了COPD疾病中肺癌风险研究的未来方向,包括深入探讨两者之间的分子机制、开展大规模临床研究以及推动个体化精准医疗的发展。Chronic obstructive pulmonary disease (COPD) and lung cancer are common respiratory diseases, and there is a close relationship between the two. This article reviews the research progress on the risk of lung cancer in COPD patients. Firstly, it introduces the epidemiological connection and research status of COPD and lung cancer, discusses the reasons why COPD patients are more prone to lung cancer. Then, it analyzes the impact of COPD on the occurrence of lung cancer, discusses the influence of common risk factors on lung cancer risk, and the impact of chronic inflammation caused by COPD on the development of lung cancer. Regarding the assessment of lung cancer risk in COPD patients, this article summarizes the research progress on diagnostic biomarkers, imaging evaluation methods, and factors affecting the prognosis of lung cancer in COPD patients. In addition, it discusses early screening and intervention strategies for lung cancer in COPD patients, introduces combined treatment strategies for lung cancer in COPD patients, and the application of personalized treatment in lung cancer management. Based on the current research status and future trends, this article looks forward to the future direction of research on the risk of lung cancer in COPD, including in-depth exploration of the molecular mechanisms between the two, conducting large-scale clinical studies, and promoting the development of personalized precision medicine.
